Sir(s),The Medical Superintendent invites sealed tender for “ Annual Repair & maintenance of Civil &

Electrical works of ESIC Model Hospital ,Rourkela-4.” as per specifications and / or quantities detailed in the Schedule attached. The “Tender Documents” comprising 8 nos. of annexures are enclosed herewith .If you are in a position to quote for the contract in accordance with the requirements stated in the attached schedule, please submit your quotation to this office.

The tender document may be obtained from the office of the Medical Superintendent in person by the bonafide applicant or his authorized representative by applying in their letter head at a cost of Rs.250/- (Rupees Two Hundred & Fifty Only) (By Demand Draft drawn in favour of ESIC Fund A/C No.I & payable at Rourkela).

The “tender documents” can also be downloaded from the web site www.esic.nic.i n & and in such case the same may be signed and submitted as per the procedures mentioned hereinafter along with the fee(s) for the “tender documents”@ Rs.250/- (Rs. Two Hundred & Fifty Only).

All the payment shall be made by Demand Draft; drawn in favour of “ESI fund A/C No. 1, ESIC Model Hospital, Rourkela – 04.” payable at Rourkela. Cheques /Cash will not be accepted.

Tenderers are requested that before quoting their rate or sending tender , the tender forms may please be read out thoroughly, otherwise the purchaser will not be held responsible for any error/oversight of his own.

The form is a standard form of tender hence certain clause/clauses may not be applicable in some case.So, tenderers are requested to ignore such clauses which are not applicable in the instant cases.

The Tender form containing the terms & Conditions & schedules should be returned in original, intact, after filling up the tender form and signing in full, on each page, whether you are quoting for any item or not, failing which the bid is liable to be rejected. If any item in the schedule is not being tendered for, the corresponding space against the item should be defaced by filling up as 'not quoting’.

In the event of the space on the chedule form being insufficient for the required purpose,additional pages may be added.Each such additional page must be numbered consecutively & be fully signed by you.In such cases, reference of the additional pages must be made in the Tender form.

If any modification of the schedule is considered necessary,you should communicate the same in separate letter sent alongwith the tender.

Medical Superintendent, ESIC Model Hospital , Jail Road, Rourkela – 04, does not pledge himself to accept the lowest or any tender and reserve to himself the right of accepting the whole or any part of the tender or portion of the quantity offered and you shall supply the same / execute thework at the rate quoted by you.

The Medical Superintendent ,ESIC Model Hospital, Rourkela reserves the right to relax/cancel any condition.

INSTRUCTIONS TO TENDERERS

1. All the interested parties are adivised to go through the Tender document carefully as it may contain some terms & conditions which may be different from other similar tenders.

2. Eligible & willing Agencies may visit the sites to get the assessment of the work on any working day between 10 a.m to 3.00 p.m & on saturday from 10.00 a.m to 12.00 noon by taking due permission from Medical Superintendent

4. SUBMISSION OF BID :

(a) The Tenderers are required to submit the tender in the form as defined below :-

The envelope no. 1 shall contain Technical Bid at Annexure-V duly filled in alongwith duly signed (all pages) Tender documents as well as self attested (in case of photocopy of document) supportive documents. The word/phrase “ Technical Bid for ARM work of ESIC Model Hospital & staff Qrs ,Rourkela- 4 “ must be superscibed on the top of the envelope .The Demand draft for Rs. 40,000/- (Rupees Forty Thousand Only) towards EMD & Demand draft for Rs.250/- (Rupees Two hundred fifty Only) towards cost of Tender, in case the Tender Document downloaded from website , shall be submitted with the Technical Bid.Without E.M.D & cost of Tender documents ,the tender is liable to be rejected.

The envelope no. 2 shall contain the Financial Bid at annexure –VIII and the word/phrase “Financial bid for ARM work of ESIC Model Hospital & Staff Qrs. Rourkela – 04 must be superscribed on the top of the envelope.

Both the envelopes shall also carry the name and address of the tenderer and all the three sealed envelopes shall be placed in a Third big envelope & should be superscribed “Tender for ARM workof ESIC Model Hospital & Staff Qrs , Rourkela – 04 “ on the top of the envelope as well as name and address of the tenderer at the bottom of the envelope. This sealed third envelope(containing the envelopes of Technical bid & Financial bid) shall be dropped in tener box placed in the office of the Medical Superintendent, ESI Model Hospital,Rourkela – 04 on or before 26.11.14 at 2.30 p.m.

The tender must reach this office not later than the time & date notified in the tender form stated in the schedule of the tender. In the event of tender received after schedule date & time, the tenderer will not be allowed in bidding and it will be rejected summarily.

(b) All entries in the tender form shall be legible and filled clearly. Any overwriting which is unavoidable shall be signed by authorised signatory.

© Tender incomplete any form, will be rejected outrightly. Conditional tender will be rejected outrightly

(d) No tenderer will be allowed to withdraw after submission of the tender within the bid validity period, otherwise EMD submitted by tendering firm will stand forfeited

(e) Each page of the tender Document shall be signed and stamped by authorised representative

(f) The financial bid and technical bid received without the signature of the authorised person will not be entertained and will be rejected summarily.

(g) Rates quoted in financial bid shall be in indian currency and as per detail furnished in financial bid i.e. Annexure VIII.

5. Earnest Money Deposit (EMD)

EMD of Rs. 40,000/-(Rupees Forty Thousand only) in the form of DD in favour of ESI Fund Account No. 1, payable at Rourkela. EMD shall be submitted along with the duly filled in tender document. In the absence of EMD, the tender shall be rejected summarily. EMD shall be refunded without interest to the unsuccessful tenderer through ECS after finalisation of thecontractand realization of the amount by ESI Model Hospital, Rourkela – 04. In the event of the withdrawal/ revocation of tenders before the date pecified for acceptance, the EMDsshall stand forfeited.

6. LATEST HOUR FOR RECEIPT OF THE TENDER:

The tender must reach this office not later than the date and time notified in the Tender Notice stated in the schedule of tender. Any tender received after that shall be rejected. In the event of the stipulated date of opening of the tender being declared a closed holiday , the date of opening of the tender (s) will be the next working day.

7. BID / TENDER VALIDITY:

The price quoted shall remain valid for acceptance for a period of Ninety (90) days from the date of opening of the bid & in respect of accepted bid, the prices quoted shall remain valid for 180 days or for such period as stated in Terms & Conditions.

8. OPENING OF BID

(a) The Technical bids will be opened at 3.00 p.m on 26.11.14 in the presence of such tenderers or their authorized representatives who may choose to be present at the time of opening of technical bids.The Technical bids will be evaluated by a Technical Evaluation committee as constituted by Medical Superintendent to check all requisite & relevant documents & their authenticity. & the eligible bidders (technically suitable) will be shortlisted.The financial bids of only the technically qualified bidders shall be opened later on.The date and time of opening of financial bid shall be communicated to all technical qualified bidders.

(b) The financial bid shall be opened in presence of technical qualified bidder or their authorised representative who choose to be present on the date and time of opening of financial bid

9. ELIGIBILITY CRITERIA

a) The bidder must be registered with PWD/C.P.S.U/S.P.S.U as Civil contractor/Electrical Contractor.

b) The bidder must possess valid electrical contractor's licence.

c) Experience of having succefully completed similar work in last 3 Years as on 31.3.14 should be either of the following :

I) One similar completed work costing not less than Rs.16 Lakh (Equals to 80% of the value of work to be executed)

ii) Two similar completed works costing not less than Rs.10 Lakh (Equals to 50% of the value of work to be executed)

iii) Three similar completed works costing not less than Rs.8 Lakh (Equals to 40% of the value of work to be executed)

c) The similar nature of work means that the applicant should have completed the building works including water supply ,sanitary installations & drainage work or repair & maintenance works & electrical works or repair & maintenance works.

d) Average annual financial turn over during last three financial years ending 31st march 2014 (i.e. 2011-12,2012-13,2013-14) should be at least 30 % of the estimated cost i.e., Rs. 7 Lakh (Copies of Balance Sheet & income & Expenditure Statement (P&L A/C) to be enclosed.)

e) The tenderer must be registered with following statutory authorities & must also furnish self attested copies of supporting documents

(i) ESI,EPFO,Income Tax,Service Tax

(ii) Labour Licence

(iii) Any other registration which is mandatory for such agencies stipulated by concerned authorities from time to time.

f) The following documents must be submitted alongwith the Tender documents:

(i) Audited Balance Sheet of Last three years (2011-12,2012-13,2013-14) alongwith Income & Expenditure Stement (P&L Account & Audit Reports)

(ii) Income Tax returns for last three years

(iii) ESI/PF challans for last 6 months (10/13 to 3/14) & service tax challn for 3/14.

g) The Tenderer must have at least 10 nos. of captive manpower on his pay roll on the day of filling tender.The Tenderer having captive manpower less than 10 will be rejected. Details of captive manpower should be submitted in the prescribed format alongwith Technical Bid.

h) Medical Superintendent reserves the right to withdraw /relax the eligibility criteria & in such a situation the Tenderers will be given sufficient time to take the changes into account..

10. AWARD OF CONTRACT

(a) The selection of the Agency will be at the sole discretion of ESIC who reserves the right to accept or reject any or all the proposals without assigning any reason thereof.

(b) The contract for the annual repair & Maintenance shall be awarded to the qualified responsive tenderer who has quoted lowest.

11. In case the successful tenderer declines the offer of contract, for what so ever reason(s) , his EMD will be forfeited.

12. PERFORMANCE GUARANTEE

The successful tenderer will have to submit a performance guarantee at the rate of 5% of the contract value for proper performance of contract in the format enclosed (Enclosure-III ) within 15 days of award of contract.The performane guarantee shall be valid for the duration of

contract plus the default liability period.The performance security can be encashed by the Employer to recover any amount which is payable by the contractor to the Employer on account for a clause arising out of the contract.

13. An agreement between Medical superintendent & successful bidder will be entered into on Rs.100/- non-judicial stamp paper .Stamp Value will be borne by the successful bidder.

SCOPE OF WORK/SERVICES

The agency will be responsible for Annual Repair & maintence of both Civil & Electrical works of ESIC Model Hospital & staff Quarters as detailed below :

(A) Civil :

1. Attending blockages of drains,gully traps ,manholes etc.2. Attending leakages of taps,valves,sanitary fittings for water supply.3. Attending rectification & malfunctioning & easing of fittings on doors,windows.4. Replacement of glass panes of windows.5. Attending rectification of wall plasters & floors on patch work.6. Periodical whitewashing, colour washing ,painting etc.7. Periodical cleaning & disinfection of Overhead water Tank & under ground water tanks.8. Diluting & injecting chemical emulsion for anti-termite treatment9. Attending addition/alteration & new constructions as & when required.10. Maintenanace of fire fighting systems.11. Horticulture work.

(B) Electrical :

1. Replacement of fuses (all type),repairing/replacement of faulty switches,holders etc.2. Oiling,greasing of fans3. Repairing,replacement of wiring,tube lights,bulbs,chokes,starters etc.4. Checking of earthing systems,& making it functional.5. Electrical maintenance of water pumps,bore wells,motor,submersible pumps & general

electrical equipments.6. Electrical maintenance works including HT/LT panels,Relays,internal wiring.7. Operation of electrical substation, HT/LT Transformers 8. Maintenance of fire fighting systems9. Maintenance of D.G Set (One 63 KVA )including supply of diesel for running of D.G (Approx.

monthly requirement 200 Litres)10. Repairing & maintenanace of Air Coolers,Water Coolers, Air-conditioners etc.

NOTE : 1. All tools, tackles,plants ,machineries,equipments & materials required to carry out the work at

site shall be provided by the Agency at his own cost.2. The Agency shall ensure all safety precautions for its labourers.

GENERAL (COMMERCIAL) TERMS & CONDITIONS

1. Definitions :

a) The contract ( as herein after defined) the following words & expressions shall have the meanings hereby assigned to them except where the context otherwise requires.

1. “Employer “ means the Medical Superintendent ,ESIC Model Hospital ,jail Road ,Rourkela-

2. “Engineer “ means the person appointed by ESIC to act as Engineer for the purpose of contract.

3. “Contractor” means an individual or firm (Proprietary or partnership) wheather incorporated or not ,that has entered into contract (with the employer) & shall include his/its heirs ,legal representatives,successors & assigns.Changes in the constituion of the firm, if any shall be immediately notified to the employer in writing & approval obtained for continued performance of the contract.

b)

1. “Contract” means these conditions, the specifications,the bill of quantities,tender,the letter of acceptance, the contract agreement (if completed) & such further documents as may be expressively incorporated in the Letter of Acceptance or Contract Agreeement (if completed)

2. “Specifications “ means the specification of the works included in the contract & any modification thereof.

3. “Drawings” means all the completion drawings,calculation & technical information of a like nature provided by the Engineer to the contractor under the contract & all drawings, calculation,samples,patterns,models,repair & maintenance manuals & other technical information of the like nature submitted by the contractor & approved by the Engineer.

4. “Bill of Quantities” means the priced & completed bill of quantities forming part of the tender.

5. “Tender” means the contractor's priced offer to the employer for the execution & completion of the works & remedying of any defects therein in accordance with the provisions of the contract, accepted by the letter of acceptance.The word Tender is synonymous with “bid” & the words, tender documents with bidding documents.

6. “Letter of Acceptance “ means the formal acceptance of the tender by ESIC.

7. “Contract Agreement “ means the contract agreement, if any, referred to in b (I) above.

8. “Appendix to Tender “ means the appendix comprised in the form of tender annexed to these conditions.

c)1. “Commencement Date “ means the date upon which the contractor receives the notice to start

the works.

2. “Time of Completion “ means the time period for which the contract of Repair & maintenance has been awarded by the employer to the contractor.

d)1. “Contract Price” means the sum stated in the letter of acceptance as payable to the contractor for

the execution & completion of the works & the remedying of any defects therein in accordance with the provisions of the contract.

.”Retention Money” means the aggregate of all money retained by the employer.

e)1. “Works” means the permanent works & the temporary works or either of them to be executed in

accordance with the contract.

2. “Site” means the places provided by the employer for repair & maintenance of ESIC Model Hospital ,Jail Road,Rourkela.

3. “Cost” means all expenditure properly incurred or to be incurred ,wheather on or off the site including overhead & other charges but does not include any allowance for profit.

2. Engineer's Duties & Authority -

a) The Engineer shall carry out the duties specified in the contract.

3. Custody & Supply of Drawings & Documents -

The drawings shall remain in sole custody of the Employer/Engineer, but copy as rerquired shall be provided to the contractor free solely for the purpose of the contract.

4) Sufficiency of Tender-

The Contractor shall be deemed to have based his tender on the data made available by the employer & own inspection & examination of this site conditions.

5) Contractor's Employees -

The contractor shall provide on the site qualified & experienced technical staff in connection with repair & maintenance of the works & the remedying of defects therein.The minimum staff shall as per description of work mentioned in BOQ.

6) Engineer's at liberty to Object -

The Engineer shall be at liberty to object to & require the contractor to remove forthwith from the works any person provided by the contractor who, in the opinion of the engineer, misconducts himself,or is incompetent or negligent in the proper performance of his duties or whose presence on site in otherwise considered by the engineer undesirable,& such person shall not be again allowed upon the works without the consent of the engineer.Any person so removed from the works shall be replaced as soon as possible.

7) Safety,Security & Protection of the Environment-

The contaractor shall throughout the execution & completion of the works & the remedying of any defects there in :

(I) Have full regard for the safety of all persons entitled to be upon the site & keep the site (so far as same under his control ) & the works (so far as the same are not completed or occupied by the employer ) in an orderly state appropriate to the avoidance of danger to such person and

(II) Provide & maintain at his own cost all lights,guards,fencing,warning signs & watching when & where necessary are required by the engineer are by any duly constituted authority for the protection of the works or for the safety & convenience of the public or others ,and

(III) Take all reasonable steps to protect the environment on & off the site & to avoid damages or nuisance to persons or to property of the public or others resulting from pollution,noise,or other causes arising as a consequence of his methods or repairs.

8. (a) Insurance of Work by the Contractor for his Liability.

(I) During the repair & maintenance period for loss of damges to property & life arising from a cause for which contractor is responsible.

(II) For loss of damges occasioned by the contractor in the course of any repairs carried out by him for the purpose of complying with his obligations.it shall be the responsibility of the contarctor to notify the insurance company of any change in the nature & extent of the works & to ensure the adequacy of the insuarance cover at all times during the period opf contract.

(b) Damages to persons & Property :

The contractor shall, except if & so far as the contract provides otherwise ,indemnify the employer against all losses & claims in respect of :

(I) Death of or injury to any person or

(II) Loss or damage to any property (Other than the works) which may arise out of or in consequence of the repair & maintenance of the works & the remedying of any defects therein

& against all claims,proceedings,damages,costs,charges & expenses whatsoever in respect thereof.

9. Accident or Injury to workmen :

The employer shall not be liable for or in respect of any damages or compensation payable to any work man for death or injury resulting from any act or default of the contractor.The contractor shall indemnify & keep indemnified the employer gainst all such damages & compensation & expenses whatsoever in respect thereof or in relation thereto.

10. Evidence & Terms of Insurance :

The contractor shall take out appropriate insurance to cover his work & workers & staff employed by him fully.The contractor shall provide evidence to the Engineer/Employer as soon as practiacble after the respective insurance have been taken out but in any case prior to the start of the work at site that insurance required under the contract have been effected.

11 (A) Compliance with statute,Regulations :

The contractor shall confirm in all respects,including by giving of all notice & the paying of all fees with the provision of :

(a) Any national or state statute,ordinance or other law,or any regulation ,or by law of any local or other duly constituted authority in relation to the execution & completion of the works & remedying of any defects therein, and

(b) The rules & regulations of all public bodies & companies whose property or rights affected or may be affected in any way by the works & the contractor shall keep the employer indemnified gainst all penalties & liability of any kind for breach of any such provision.

© Any changes required for approval due to revision of local laws.

(B) Default of Contractor in Compliance :

In case of default on the contractor in carrying out such instruction within the time specified therin or if none one,within a reasonable time,the employer shall be entitled to employ & pay other persons to carry out the same & all cost consequent thereon or incidental thereto shall, be determined by the Engineer shall be recoverable from the contractor by the employer, & may be deducted by the employer from any monies due to or to become due to the contractor & the engineer shall notify the contractor accordingly.

12. Time of Completion :

The repair & maintenance work shall be for a period of 12 months or as mentioned in the letter of commencement & shall start from the date of issue of letter of commencement & shall stand terminated after the expiry of twelve months.

13. Extension for time of Completion :

The repair & maintenance contract may be extended on the written mutual consent of both employer & the contrcator for a further period of one year.However,employers reserves its right to terminate the repair & maintenance contract by giving 15 days notice at any time during the currency of the contract if the service of the agency are not satisfactory as per the opinion of the employer or its representative.

14. Defect Identification & its rectification :

The Agency shall immediately attend the defects & complaints notice at site.The agecy shall provide & develop a system for regular repair & maintenance of all services which include defects identifications & its immediate rectification so that services are not affected.It shall be the sole responsibility of the repair & maintenance agency that all the services are kept in functional condition round the clock during the currency of the contract.Defect Liability period shall be 12 months from the date of completion of the work under BOQ measurable works.The contractor shall rectify at his own expenses any defects in the work carried out by him during this period. On failure of the contractor to do so ,the same shall be completed by the employer at the risk & cost of the contractotr.

15. Penalty for Delay (Liquidated Damges for Delay)

If the contractor fails to attend nay complaint or defect in due course of time & if in the opinion of the Engineer delays on the part of repair & maintenance agency,the employer can impose liquidated damages on the contractor as mentioned in the Tender documents.

16. Contractor's failure to carry out instructions :

In case of default on the part of the contractor in carrying out defect rectification works ,the employer/engineer shall be entitled to employ & pay other persons to carry out the same & if such work in the opinion of the engineer,the contractor was liable to do at his own cost under the contract,then all costs consequent thereon or incidental thereto shall be determined by the Engineer & shall be recoverable from the contractor by the employer, & may be deducted by the employer from any monies due to or become due to the contractor.

17. Instructions for variations :

Quantities given in the BOQ rae estimated quantities.The quantity of any particular item may vary to any extent,variation in quantity in particular items or overall cost,does not entitle the contractor to claim any extra cost.

18. Works to be measured for BOQ items :

The Engineer shall determine by measurement of the value of actual work done in accordance with the contract & the contrcator shall be paid proportionately.Part rate shall be made for nay part of BOQ items not fully executed.Engineer shall be at liberty to decide the break up of lumpsum items & to decide the part rate of nay particular item.In case overtime working of staff under is required, specific instructions of the Authorised Officer/Engineer is to be obtained.Payment shall be made at applicable overtime rates.

19. Method of Measurement:

The work shall be measured net, notwithstanding any general or local custom,except where otherwise provided in the contract.

CERTIFICATE & PAYMENTS :

20. Monthly Statements :

The contractor shall submit a bill in three copies to the Medical superintendent by 7 th of each month for the work executed upto the end of previous month in tabulated form duly verified by the Engineer,showing the amounts to which the contractor considers himself to be entitled.The bill must be supported with the following documents :

(a) Attendance sheet alongwith salary cerificates, wage sheets of all the workers deployed .

(b) Certified bill for miscellaneous materials purchased under different heads.

© Details of defects/complaints attended & rectified within time.

(d) Details of complaints attended late.

(e) Proof of credit of wages to the individual bank accounts of workers through ECS.

(f) ESI/PF/Service Tax challans must be enclosed.

21. Deduction of Income Tax :

The amount to be deducted towards the income tax shall be at the rate applicable.

22. Monthly payments :

After submission of monthly bills complete in all respects by the contractor,engineer shall check the bills with the help of contrctaor & certify for payment within 15 days of submission of the bill.

23. Correction of Certificates :

The Engineer may at any interim payment certificates make any correction or modifications in any previous interim payment certificates which has been issued by him & shall have authority if any work is not being carried out to satisfaction ,to omit or reduce the value of such work in any interim payment certificate.

24. Final Certificate :

Within 28 days after receipt of the final statement, & the written discharges,the Engineer shall issu to the employer (with a acopy to the contractor) a final cer tificate stating :

(a) The amount which,in opinion of the Engineer is finally due under the contract ,and

(b) After giving credit to the employer for all amounts previously paid by the Employer & for all sums to which the Employer is entitled under the contract.

25. Default of Contractor :

(I) If the performance of the contractor is not satisfactory & not corrected within 15 days of receiving notice,then employer shall be at liberty to terminate the contract & get the work executed through other means at the risk & cost of the contrcator.

(II) In the event of termination of the contrcat,the employer shall be at liberty to get the balance work done at the risk & cost of the contractor & due payment of contractor, if any,shall be releaesed after the completion of whole of the works.

26. Amicable Settlement of Disputes :

The parties shall use their best efforts to settle amicably all disputes arising out of or in connection of this contract or the interpretation thereof.

27. Arbitration :Any dispute or differences relating to the meaning of the specifications,designs,drawings & instructions herein before mentioned and as to the quantity of workmanship of materials used in the work or as to any other question ,claim,right,matter or thing whatsoever in any way arising out of or relating to the contract,designs,drawings,specifications,estimates,instructions,or these conditions or otherwise concerning the works or the execution or failure to execute the same wheather arising during the progress of the work or after the completion or abandonment thereof in respect of which amicable settlement has not been reached shall be referred to the sole arbitration of the Chief engineer,ESIC who shall proceed as per the Arbitration Act,1996.

SPECIAL CONDITIONS OF CONTRACT

1. CPWD specificatoions shall be followed.where not available,BIS/Engineering practice as directed by the Engineer shall be followed.

2. Complaint shall be made in the format (Enclosure-I)

3. A complaint register (format at Enclosure-II) shall be maintained in the repair & maintenance office of the contractor in which all complaints received shall be documented.

4. All repair & maintenance related complints shall be attended to within 2 hours failing which a recovery of Rs.100/- per event per hour shall be made from the subsequent payment certificate of the contractor.

5. The sewer man shall ensure that all lines are maintained in a clean condition by preventing cleaning of all lines at least once in each month.

6. As the work will have to be carried out in building and in use, the contractor shall ensure :

i) That the normal functioning of ESIC activity is not affected as far as possible.

ii) That the work is carried out in an orderly manner without noise & obstruction to flow of traffic.

iii) That all rubbish etc. is disposed off at the earliest & the place is left clean & orderly at the end of the each day's work.

iv) The contractor shall ensure that his repair & maintenance staff is qualified & licensed for their part of work.He shall be responsible for their conduct.The staff should behave in courteous manner.the contractor shall be held responsible for any loss or damage to ESIC property.

v) The contractor shall ensure safety of his workers & others at site of work & shall be responsible for any consequences arising out of execution of the repair & maintenance work.

vi) When instructed to do so,the contractor shall ensure proper record keeping & storing of irrepairable/dismantled material.

vii) Water & electricity shasll be made available free of cost at the nearby source of work.The contractor has to make his own arrangement for use of the same including drawing temporary

6) The contractor, to whom the work is awarded,will execute the work himself.He will not be allowed to sublet the work to any other agency.

7) The workers/staff of ARM Contractor should be issued uniforms with badges which will be worn by all workers/staff.

8) If there is any item not mentioned in the Tender Schedule ,the work will be done as per DSR or Loacl Market rate as the case may be.DSR 2013 shall be followed.

9) The agreement shall be for a period of 12 months.However, it may be extended for further for a maximim period of another 12 months on the same terms & conditions.
