End of Life Planning Part One:

Understanding Death

Living things, including people, cannot be here

forever. There will be a time when your body

will not work as it should. Like plants and

animals, old ones or sick ones cannot live

forever. It is sad but it is a part of life. It is

called death or dying.

Did you ever know someone who died?

Grandparents

Moms, Dads, Brothers, Sisters

Friends

Doctors

Pets

Uncles or Aunts

The people who died are not with us anymore. We miss them and remember what is was like when they were here.

We cannot see them or talk to them or visit them. They are dead.

Dying

Sickness

Accident

People die in different ways. While some people who get sick do get better, others can die from being sick. Some die from accidents like choking or car accidents.

People die in different places. Some die at home. Some die in a hospital. Some people even die when they are on vacation or far away from home.

Sometimes you know that someone is going to die and other times it may be a surprise or shock

because they were not sick.

After Someone Dies

You may be very sad. It may help to think about all the good

things you remember about the person.

It may be hard to do the things you

normally do everyday. You

may not feel like eating or going to

your job.

Going to the funeral or memorial service for the

person can honor the person and may help

you too. You probably will see other people who feel the same as

you.

It may help to talk about how you are feeling

with other people.

If you are feeling sad, angry or upset for a long time and you are not able to do things

you used to do, you should talk to your nurse or doctor.

End of Life Planning Part Two: Making a Plan

have a plan. Better for you…

Better for those who love or support you...

What Are Some Things You Plan For?

What should I wear?

A Trip

A Party

You plan for things so that people know what

you want.

Think about these things….

Where should we eat?

Where will we go?

How much money will I

need?

Making a Plan For The End of Life

When You Are Sick

Before you get very sick, in case you don’t get better,

it is really important to make a plan, so people know what

your wishes are.

Making a plan for your end of life wishes is a

personal matter. You can make a plan by yourself or with a friend, family

member, or support staff.

Would you like people to visit you?

Aunts, Uncles, Friends, or Family?

Who can help?

Friends? Family?

Where would you like to be?

Do you want a funeral

service or something else?
