Healthy Lifestyles is a voluntary health and wellness program for Spectrum Health employees. The program is administered and managed by Priority Health Wellness, an organization that works with local businesses to establish effective health promotion and disease prevention programs.

The Healthy Lifestyles program is a three-step process: enroll, biometric screen, and participate and evaluate.

Healthy Lifestyles offers reduced health insurance contributions for enrolling to those who have Priority Health benefits through Spectrum Health.

Cash incentives/rewards also are given for biometric screens and for participating in various program components.

Healthy Lifestyles provides opportunities for employees to improve their quality of life through preventive, physical, and mental and holistic health actions.

Even if you do not take medical benefits through Spectrum Health, you are encouraged to enroll based on the program’s many benefits.

Key Program Dates

Enroll• Complete the online Health Risk Appraisal from September 1, 2012, to

October 31, 2012.• Receive reduced health insurance contributions for 2013.

Participate and Evaluate• Complete your preventive health actions.

− Physical exam: August 1, 2011, to July 31, 2013 − Dental exam: August 1, 2012, to July 31, 2013 − TB test

• Complete all physical and mental and holistic health actions between September 1, 2012, and July 31, 2013.

• Complete the online program evaluation in August 2013.• Receive $50 for documenting the completion of all actions and submitting

the evaluation.

Note: To be eligible for the cash incentive, you must be an active Spectrum Health employee and enrolled in the Healthy Lifestyles program when the incentive is paid.

Biometric Screens• Complete a biometric screen on-site or with your physician.

− On-site screenings are available August 2012 through November 2012. − Screenings done by your physician must be completed between March 1, 2012, and November 30, 2012.

• Employees must enroll by October 31, 2012. New hires will have 90 days from the date of hire to complete enrollment.

• If you do not have Internet access, you may use the computers in the Blodgett and Butterworth hospitals’ health sciences libraries. You will need to check out a user ID from the library front desk. Public computers also are available at many local libraries and coffee shops.

How to Enroll

Visit priorityhealth.com and follow the instructions below for the scenario that fits you best.

A: If you already have an online account1. Enter your username and password and click “Go.”

B: If you have an online account but do not know your login information1. Click “Forgot user name or password?” 2. Select “A member.” 3. You will need your Priority Health contract number to reset your password.

C: If you have not yet set up your online account1. Click “Create account.”2. Under “Member Accounts,” click “create your account.”3. You will need your Priority Health contract number to create your account.

After you have logged in or created your account, you will be directed to your Member Center. Click “GO TO WEBMD.”

From your Healthy Lifestyles Rewards page, under the Enroll section, click “Complete your Health Risk Appraisal.”

• If you completed the health risk appraisal last year, you may simply need to update the information that is already saved in your health risk appraisal.

• If this is your first time enrolling, answer the questions to the best of your knowledge.

• After completing the health risk appraisal, you will receive a score out of 100.

Please note: if there is certain information that you do not know (e.g., waist circumference or the dates of your recent exams), you may leave it blank. You do not need to complete the section that asks about your lab work. Results from your biometric screen or most current blood draw will be automatically uploaded into this section.

Priority Health Contract Numbers

Which of the following applies to you?

You have Priority Health insurance and you are the primary carrier.Your contract number* is located under your name on your Priority Health insurance card. If you do not have your insurance card available, call customer services**.

You have Priority Health insurance but it is through a spouse or other family member.You will receive a second Priority Health ID card that is for Wellness only and specifically for Healthy Lifestyles. Your contract number* will be located under your name. If you have not received this card in the mail, call customer service** and ask for your Wellness contract number. You also may request that a second Wellness card be sent to your home.

You do not have Priority Health insurance.You will receive a Wellness card that is specifically for Healthy Lifestyles. Your contract number* will be located under your name. If you have not received this card in the mail, call customer service** and ask for your Wellness contract number. You also may request that a second Wellness card be sent to your home.

*Note: All contract numbers will begin with 947 and end in -00.

About Your Biometric ScreenChoose to participate on-site or with your physician. You may not participate in both. Only the first results received will be used for the purposes of this program.

Sign up for an on-site biometric screen.

• On-site screens are conducted at various locations by specially trained professionals from employee health services.

• Schedule your appointment through InSite by selecting the link under Tools and Services, or call 877.548.3871.

• You will need to fast for 12 hours prior to your screen. Drink plenty of water during the fasting period and remember to take any prescription medications.

• Bring your Priority Health identification card and reading glasses, if needed.

Have your physician complete your biometric screen.

• Your physician must verify all of the screening components listed on the Physician Documentation or Medical Exception Form*.

• If you are diabetic, you may want to consider having your physician complete the Physician Documentation or Medical Exception Form* for the entire screen, including the A1C value. This will save you a step compared to completing the screen onsite and then going to your personal physician for the A1C value.

• If you are pregnant, within six months post-partum, or breastfeeding, ask your physician to complete the Physician Documentation or Medical Exception Form*.

• Results must be from March 1, 2012, to November 30, 2012.

• Priority Health Wellness must receive your form by December 10, 2012. Late forms will not be accepted.

*This form is available on the Healthy Lifestyles page on InSite or on your WebMD Rewards page at priorityhealth.com.

How to Participate

Preventive Health Actions—Physical and Dental Exams and TB Test

• If you are enrolled in medical or dental insurance through Spectrum Health, your physical and dental exam completion will automatically be uploaded to your WebMD Rewards page.

• If your physical or dental exam is not covered by insurance through Spectrum Health, or you are not the primary carrier, submit a Preventive Health Verification Form. This form is available on the Healthy Lifestyles page on InSite or on your WebMD Rewards page at priorityhealth.com. The form must be submitted to Priority Health Wellness by August 7, 2013.

• Your TB test completion will be uploaded automatically to your WebMD Rewards page.

Physical and Mental and Holistic Health Actions

• Report your actions using the online participation verification form by August 7, 2013. This form can be found at priorityhealth.com. Log in to your Member Center. Under “Your events,” click “Verify your participation.” You also can access the form from your WebMD Rewards page.

• You may either submit the online form each time you complete an activity or submit one form once all of your activities are complete.

• Your completion status is reflected on your WebMD Rewards page within two weeks of submission.

How to Evaluate

• Log on to your WebMD Rewards page in August 2013 to complete the online evaluation.

• Based on feedback in your evaluation of the Healthy Lifestyles program, we are able to consider program adjustments for the following year.

How to Earn Your Incentives/Rewards

• If you are a Priority Health member through Spectrum Health and enroll in Healthy Lifestyles by the deadline, you will contribute $25 less per pay period for the 2013 calendar year, compared to employees who do not enroll.

• New employees have 90 days from the date of hire to enroll. The reduced health insurance contribution will be effective when Spectrum Health is notified of your enrollment.

• Biometric screen and participation incentive payout dates will occur in the first quarter of the 2013 calendar year.

• To be eligible for the cash incentive, you must be an active Spectrum Health employee and enrolled in the Healthy Lifestyles program when the incentive is paid.

• Under federal law, the incentive/reward payments must be taxed because they are considered income.

Reimbursement for Spectrum Health Sponsored Programs

Healthy Lifestyles participants may be eligible for up to 50 percent reimbursement for Weight Watchers® at Work, on-site exercise classes and holistic services (if offered through Healthy Lifestyles at your specific entity).

Encompass

Encompass strives to strengthen people, performance and productivity by offering employee assistance services including assessments, consultation or resources, and emergency or crisis management. Using Encompass services can count toward the mental and holistic health actions. For more information, call Encompass at 800.788.8630 or visit encompass.us.com.

Health Coaching

Health coaching is a benefit offered to Healthy Lifestyles participants who have certain health risk factors. Spectrum Health provides this benefit at no cost to you if you qualify. A health coach or a representative from Encompass will contact you if you meet the criteria. To maintain confidentiality, they will facilitate health coaching referrals.

Behavior Change Programs

Get motivated to improve your eating habits, lose weight and increase your physical activity with programs that help you change behavior and improve your health. Participating in these programs can count toward your physical health actions. You may be eligible to win prizes.
