INCIDENT SUMMARY
Last week Dennis Ingram of DOGM alerted me about a pipeline spill that occurred on 5/14 near the El Paso Rudy 1-11B3 well location south of Bluebell. Last Friday 5/16 I made a follow-up visit, attached are photos. I met with Val Oman of El Paso at the site, his phone number is 435-454-3394.
This was a produced water line that developed a leak (photo 013). There is a small pond next to the leak location, and the leaking production water had run into the pond, the pond overflowed, and the leak ran maybe 300-400 yards down the farmers field and soaked into the ground (photo 007, you can see where the leak ran, the non-green area in the middle of the field). There was no surface water impact other than the small pond. By late last Thursday El Paso had pumped the water out of the pond (ten trucks worth or 1,300 barrels) and trucked it to their nearby disposal facility. Their plan is let the pond and remaining wet spots dry out, then they will come in and skim the contaminated dirt and dispose of it. The landowner is working with El Paso and is apparently satisfied with the proposed cleanup, which may take several weeks or even a month. El Paso has repaired the production line (photo 008) and it is back in service.

They had a rupture in the Brotherson 210 B4 production line that goes to the Brotherson 110 B4 Battery. They released ~15 barrels (720 Gallons) of production fluid (Oil, Gas, Water mixture) to the soil. The fluid flowed 50 yds to a dry ravine. Some went into the ravine, most went to the soil. At the same time their trace system was ruptured and released approximately 300 gallons of Glycol. Cleanup is scheduled for tomorrow morning.

Production water spilled from a ruptured, pressurized pipeline between their fixed facility and an injection well. ~9.5 barrels (~400 gallons) spilled into a dry wash area impacting ~75 yards into the dry wash. Facility was pressure testing a line using fresh water. Effluent was discharged into a Frac-Tank which overflowed an estimated 25 barrels. Frac-tank is located next to irrigation ditch. Discharge water presented with contaminant by-product of corrosion described as rusty-black in appearance. Representative walked about one mile downstream. With exception to the residual observed at the point-of-entry, there were no apparent visual impact.

Buried steel salt water disposal line crosses beneath irrigation ditch. Believe the line failed. Release volume is estimated at six barrels of produced water and three barrels of a heavy slop-crude oil. Approximately one barrel of slop-crude entered the irrigation ditch and migrated 1-1/4 miles from point of release. Irrigation ditch splits downstream at 1-3/4 miles from point of release and services agricultural fields. Natural grow willows and tumbleweeds esatablished natural blockade preventing further migration. Most adhered to the banks and shows minimal sheen. Land owner discovered release and made notification. PRP was able to quickly shut-in flow. PRP crew deployed at 17:00 to begin cleanup. Booms and sorbents were deployed. PRP continues these actions today and is excavating the line to determine repairs.

INCIDENT SUMMARY
El Paso reported a line rupture at the East Compressor Station while performing a hydro-test to 1100 psi. The line leaked at 1050 psi. Approximately 100 barrels of fresh water was released into Big Sand Wash Creek. The line was filled with about 1500 barrels of fresh water. No sheen was reported visible. The DEQ District Engineer (DE) was in the area and visited the site. The DE reports that the PRP appears to have the incident under control and is diligently repairing the line. DE reports that the creek does not present evidence of a sheen, contamination, residue or odor. Sand Wash Reservoir is approximately 3 miles downstream. Several monitoring wells associated with the facility are nearby.

Per NRC report: Release from a condensate above ground storage tank at El Paso E&P Compressor Station was caused by a one-inch valve. Approximately 8 barrels of condensate leaked into an unknown tributary of the Duchesne River. The material has been contained in a ditch using booms and is currently being cleaned up with VAC trucks. A gasket on a threader failed during a wind storm. A mist of oil was blown into a reservoir. An estimated 10 barrels of crude oil is reported to have been released and approximately 0.5 barrels were blown into Lake Boreham. The well site is being cleaned and an environmental contractor has been hired to cleanup the release to the water.
UPDATE provide by Scott Hacking 10/18/12@7:57 AM: Things are under control for this release, and it was not a major spill (water-quality wise) with only an estimated 20 gallons reaching water. And most of that is in solid wax form, collected on the north shoreline, enabling an easier cleanup. Cleanup was under way yesterday late afternoon and evening. Lake Boreham is an agricultural reservoir, in a very rural area with few residences nearby. There is one residence downwind about a half mile away. The product released is not your classic crude oil, it is a paraffin-like yellow waxy Uintah Basin crude. At ambient temperature it appears to be yellow candle wax. It likely has a very limited solubility in water, especially in colder temps. The photos really show the character of the oil (which is not like "normal" crude oil at all). The release at the well site (the well site is not shown on the Google Earth imagery, as the aerial photography is dated in that area) was sprayed up into the air, and a high wind (30 mph plus) was blowing which carried the aerial plume almost directly east. Only a small part of Lake Boreham was impacted along the north shoreline (see map), and I think the company estimate of half a barrel (about 20 gallons) is reasonable. Envirocare personnel were at the site cleaning up the visible contamination (which kind of looked like a heavy yellow pollen, very small waxy particles that were collecting in the reeds in the shallows on the north shoreline). The wind was blowing the contamination due east so that it basically plated out on the north shoreline. Really a pretty limited impact water-quality wise. Not a major spill in the water at least, and not a lot of cleanup to be done (as far as the water). There is significant land cleanup to be done, and it was estimated that Envirocare personnel would be there for at least 3 days. They had mobilized their command center and located it at the site (see photos). Tim Eaton with Newfield (435-646-3721) and Tony with Envirocare (801-299-1900) are the contacts in the field. I was able to collect samples last night, however I think we need to consider whether we need to analyze them or not. Caller reported a release of production water (salt water) into irrigation pond through a catch basin at the site. Sheen was observed on the pond and the caller reported that they were unable to obtain an accurate volume but estimated approximately 1 gallon of oily sheen observed on the water surface. The called reported that the water in the pond discharged into an irrigation ditch which dissipates approximately 1000 feet downgradient of the pond. Absorbent booms were placed at the discharge of the pond and along the irrigation ditch to absorb the oily sheen. The caller notified NRC and I provided contact numbers for the TriCounty Health Department.

I just talked to Chris Jensen with DOGM who was at the site yesterday. The spill volume has been minimized by Newfield. From what I am hearing it was probably at least a hundred barrels and probably in the hundreds. Chris Jensen alerted me to that. A stock pond was filled up and overflowed, and a road was severely eroded by the flowing water. It did get into surface water (the stock pond). I will visit the site today and document what I can and take samples.

AT APPROXIMATELY 8 AM 2/4/2013 A WATER INJECTION LINE LEAK WAS DISCOVERED NEAR THE 5-27-8-17 WELL SITE. THE LINE WHICH CARRIES INJECTION WATER BROKE ON THE NORTH SIDE OF THE ROAD RELEASING APPROXIMATELY 20 BBLS OF WATER WHICH RAN SOUTH ACROSS THE ROAD AND INTO A DRAINAGE WHERE IT MADE IT'S WAY DOWN TO A STOCK POND. IT THEN FILLED THE STOCK POND AND CONTINUED TO FLOW SOUTHEAST DOWN THE DRAINAGE TO THE TERMINUS AT N 40° 5' 21.05" W 109° 59' 57.99". THE LEAK WAS ISOLATED UPON DISCOVERY. WATER SAMPLES OF THE POND AND A NEAR BY SPRING HAVE BEEN TAKEN FOR COMPARISON. UPON AGENCY APPROVAL, FRESH WATER WILL BE FLUSHED DOWN THE DRAINAGE IF NECESSARY TO BRING TDS BACK INTO NORMAL RANGES. BLM, UDOGM, & TRICOUNTY HEALTH DEPARTMENT HAVE ALL BEEN NOTIFIED. UDOGM LOOKED AT THE SITE AT APPROXIMATELY 12:00 PM.

Caller reported that a leak in an inject line was discovered this morning. The leak was the result of internal corrosion. The injection water made it to a dry drainage ditch. The line was secured and the ditch was dammed up stopping the water. They will flush the ditch with clean water and recover the released fluid. Samples have been pulled. One set is available for state analysis. They have notified Chris Jensen with DOGM and he will visit the site Monday. Location is aproximatte for well area.

Injection line broke releasing about 10 barrels of water. An estimated 5 barrels flowed across the road into a waterway fed by an emergent spring. This flows to a stock pond. The caller reported that 692 barrels of injection water was released over the course of the day. In jection water is a mix of treated production water and fresh water. The caller stated that their equipment allowed them to determine that the 692 barrels was made up of 512 barrels of fresh water and 180 barrels of treated production water.
At approximately 4 am they determined that there was a leak. The located the leak and stopped it. There were actually two leaks and the second leak went unnoticed from 4 am until 5 pm. They were able to shutin the release at 5:30 pm.
The water ran down a leased access road and into a small stream which eventually flows into the Pariette Wetlands. The caller did not know how far down the stream the water flowed. The company took six water samples from the stream and plan to take some upstream in the morning to determine background levels.
Their current plan is to dam the stream and flush the spill from the source to the dam with fresh water. The water will then be sucked from the dam area and disposed. The dam will eventually be removed and the stream allowed to flow again.
The leak occurred near well site number 60-26-8-17. The coordinates of the site are: 40degrees 5minutes 21.59 seconds North, 109 degrees 58minutes 34.86seconds West.

INCIDENT SUMMARY
Caller reported a 30 barrel spill of injection water (described as a mix of produced water and fresh water) that flowed towards an ephemeral drainage area. Only 10 barrels actually entered the drainage area. No other waterways were impacted. The injection water soaked into the ground. Caller said there were no plans for clean-up. This incident took place on BLM land. The incident location on the attached map is approximate. UPDATE (9/20/13, 17:35, KV): Tim Eaton of Newfield Exploration called to report a break in a line that happened last night causing an additional 360 barrels of injection water to spill into the dry drainage area. He took samples for lab analysis and will have the results next week. Some of the water was retrieved with a vacuum truck. Caller said that he would flood the impacted area with clean water to dilute the spill if that is required.

Caller reported a release of produced water and oil from a 4-inch steel Salt Water Disposal line. The release occurred approximately 9550 feet of the Jensen 2-29A5 drill site. A total of 1500 bbls of produced water and 5 bbls of oil were released during the incident. An estimated 5 gallons of oil and 1500 gallons of produced water flowed into a nearby catch basin used for livestock watering. Landowner was notified of the release and a fence was erected around the basin to prevent livestock access. Released oil becomes a waxy substance when exposed to current temperatures. 1300 bbls of produced water was recovered. Line repaired and fresh water was used to flush the line. 11/26/2013 Other Update All, Here is the clean up details that we have completed on the spill that occurred October 26th for the Jensen 2-29A5 Salt Water Disposal Line leak in Talmage. I have no email address for Darrin Brown with Tri County Health Department and they are requesting a cleanup report within 30 days of the incident. We have cleaned a total of 45 yards of contaminated soil from the leak site at the field and pond. All contaminated soil was hauled to the Duchesne County Landfill. The landowner has requested that we do not haul anymore soil than necessary out of his pond. Since the leak, we have steamed the entire line out with a hot oiler. The leak in the pipe was cut out and determined to be severe internal corrosion on the pipe wall. As part of our company policy, the line was hydro tested before being put back in service. We could never get a good hydro test after repairing two leaks and have left the line out of service. The landowner has not been met yet to get his approval, but we are going to set up a time with him. If there are any other questions, let me know back. Thanks.
11/6/2013 DEQ DE Site Visit A followup visit was made on Nov. 6th to check the cleanup. There was nobody at the site at the time, but the cleanup equipment was still there. It appears the contractor was waiting for the remnant water/mud in the small pond to dry out so they could excavate that area, which is all that is left to clean. The rest of the site has been excavated, and the entire site is still fenced off from cattle (this spill was on private farmland). The cleanup so far looks good, except they still need to address the pond area that is drying out
11/1/2013 DEQ DE Notes There has been a rash of oil/gas spills recently, and Darrin and I had 6 spills to investigate this week. Three were minor, one was major (see #11599 below), two were "medium" (#11600 and #11597 below). Two were minor spills reported by the public to Darrin (in Hanna and Jensen), and one was a minor crude spill on pavement (on the highway) reported by DOGM staff (outskirts of Duchesne) that were all basically "side of the road" type minor oil/diesel spills, and when we visited them last Monday they were already cleaned up, so incident reports for those minor spills were not generated. One main reason for that is Darrin cannot generate the spill reports himself, and if he had full access 24/7 to do that on his own he probably would - we would like to bring that issue up at the WQ spills Kaizen on Nov. 13th).

10/30/2013 DEQ DE Site Visit Incident #11599- EP Energy 1500 bbl Production Water Spill near Talmage (north of Duchesne)An email report on this one has been sent to WQ staff, and I have more photos to send as followup later. This cleanup is ongoing and more followup site visits will be needed. An EP Energy production water pipeline ruptured and 1500 bbl of water was released, most all of it collecting in a small farm pond that was immediately downhill. The pond is pretty small, and I was told most of the year it is dry, except when it rains (or during snowmelt) it does fill up and hold water for a few weeks. This is on private farm land and cattle are present (also this is not "Indian Country"). The pond was pumped out and then fenced so the remnant water in it is not a threat to the cattle. Cleanup is ongoing, excavation is occurring and they are trying to dry the pond out to facilitate cleanup. It appears to be a good cleanup so far based on my site visit on the 30th. No surface water was affected, except for the release collecting in the the small pond (which is normally dry in the fall, however it may have had some water in it due to recent rains - it is unclear how full it was before the release).

From NRC ReportCALLER STATED THAT A TRACTOR TRAILER WITH A PUP HIT BLACK ICE AND ROLLED ONTO A PICK UP TRUCK THEN DOWN AN EMBANKMENT. THERE WAS A RELEASE OF 40 BARRELS OF CRUDE OIL ONTO THE GROUND AND INTO A CREEK. THE RELEASE IS FROM THE PUP. DRIVER OF THE PICKUP TRUCK WAS FATALLY INJURED.
Caller reported to DWQ that they had police anf fire onsite. The y also dispatched Envirocare to the scene. The truck is being offloaded, but all 40 barrels in the pup were released. The waterway is nopted in maps as Gray Mountain Canal. On scene person rep[orts that the canal is frozen on the surface and crude was waxy crude, so it may set up an not migrate. Fire aparently notfied Tri County Health of the incident.
Location is on Sand Wash Road about 1 mile south of Highway 40 at top of hill. Some maps show this may or may not be on Tribal land.

3/14/2014 DEQ DE Update Incident #11696- Crude Oil Tanker Spill near Myton. The ice has melted, exposing some lumps of crude oil that did get under the ice. Contractors were on-site Wed and Thursday for final cleanup. Darrin reports that as of last Wed late there was still cleanup to be finished. Darrin and/or I will make a final cleanup inspection this week.
3/7/2014 DEQ DE Update Incident #11696- Crude Oil Tanker Spill near MytonThe ice on the canal has finally melted. I visited the site on March 4th, and it is apparent there is still a minor amount of crude oil globules of varying size (some are as big as a fist) in the canal water that need to be cleaned up. There is only about a 100-foot length of canal and canal banks to be cleaned. The flow in the canal is a very small stream of water at the moment, about the same as 2 garden hoses flowing. I contacted the cleanup contractor (Levi Pratt with Pacific West) and they are scheduled to return to the site early next week for final cleanup. After final cleanup, Darrin and I will visit the site one more time to confirm
2/28/2014 DEQ DE Update I visited the site on the 25th, and John Kennington was able to see the site also on the 19th. The site is pretty much cleaned up now and they did a good job (after having to come back to the site twice for more cleanup). The only question remaining, is that there was still some ice covering the canal, and it is possible (not likely but possible) that there is still some crude oil that was pushed down beneath the ice. The ice was melting quickly, and the canal should be ice-free by next week. I will be meeting the cleanup contractor next week for a final inspection (with Darrin Brown if possible) to make sure that there was no crude oil under the ice that still needs some cleanup.
2/24/2014 DEQ DE Update The cleanup contractor worked several more days last week to finish the cleanup. I was able to visit the site twice to document the progress. During the cleanup of the area near the canal they did discover that there was some crude oil that managed to make it under the ice and into the canal (it was hot when it spilled, and some of the oil apparently melted the ice and was pushed under the ice into the water below). Cleanup of the oil in the canal will occur this week. The black wax crude does not spread in the cold water, and it is sitting under the ice in solid layers and clumps which should facilitate cleanup. There was still some ice covering the canal that was preventing cleanup last week but it is melting fast and final cleanup should be able to occur this week.
2/3/2014 DWQ Update Rich Harness called on Feb 3 2014. Canal is Tribakl canal. The spill cleanup is completed and samples have been taken. The canal was frozen solid, workers were able to remove the solidified crude from the ice and soil quickly. Reports and results are being submitted.

Caller reported that Newfield Production Company has been drilling an oil well within a few hundred feet near his property. Based on the provided address information it is assumed that the oil well is Jorgensen 2-4-9-3-2WH (API 4301352107). During the drilling process the caller noticed changes in the flow to the private well (depth~225') on his property. Caller has also siphoned water from the well to irrigate a pond (~12'x12'x2') stocked with gold fish. On the incident discovery date caller observed that the pond had a sheen and the the fish were dead. Also, Newfield appears to have completed the well based on observations by the caller.

4/7/2014 DEQ DE UpdateProduction Water Release 10 miles West of Duchesne (Incident #11772).I visited site on April 2nd, and the pipeline break had been fixed and the repair trench was still open. There was a cleanup crew on-site (Chapman Construction) that was removing the contaminated soils, with a small Bobcat and hand work with shovels. The production water ran maybe 300 feet or so north into the nearby range land (owned by DWR) , and two containment berms/excavations had been constructed at the terminus. No surface water was affected. The cleanup was looking good. I called Scott Ghan of BBC while I was at the site, and he has hired LTE Environmental to take soil confirmation samples of the cleanup. The DWR was notified by BBC, and DWR gave permission to construct the containment berms/excavations. The minerals are Tribal, and the Tribe was also notified by BBC.An email report with photos was sent to DWQ staff.
