2015 Benefits UpdateWe’ve just kicked off a new year with our benefits options, so let’s review what’s new and what stayed the same. Here are a few frequently asked questions to help guide you during 2015. For a quick review of all of your benefits, please see the Human Resources page on CHIP to get the 2015 Benefits Guide.

Has our insurance company changed? No, we are still with UMR which provides administration for our medical insurance claims, but you will see a new prescription drug provider on your UMR insurance card, Catamaran. Your card should look like this:

Have our benefits changed? Yes. Prescription drug benefits have changed, and many services provided in Tier 1 at a CaroMont Health facility or an approved provider with privileges will be covered at 90%.

I misplaced my benefit card. How do I get a new one? You can request a new card or print a temporary card by logging into www.umr.com. You may also call 1.800.758.4910 to request a new card. You will need your Employee ID number ready when you call or login.

What pharmacies are in the Catamaran network? 2015 Benefits Update (continued)

Why do some of my brand-name drugs cost more this year? Our pharmacy plan now has a tiered formulary based on medication type.

Generic medications – (lowest out-of-pocket costs) – 20% up to a max of $20 Preferred brand medications – 30% up to a max of $75 Non-preferred brand medications – 45% up to a max of $100

The price and save tool at www.catamaranrx.com can help you find the best price for your medication. It can also help determine if a generic option or preferred brand is available, or if mail order could save you additional money.

What is Step Therapy? Catamaran’s Formulary Advantage Program employs a step therapy approach, requiring generic or preferred brand medication be used first before prescribing higher-cost medications. Your pharmacist will let you know if your prescription requires step therapy, and your doctor must contact Catamaran for prior approval for the next step medication to be considered. Visit the HR page on CHIP for more information about the Formulary Advantage Program.

How does mail order work? Will it save me money? Filling your 90-day prescriptions through mail order can save you money. You need to enroll in the Catamaran Home Delivery Program and mail your prescriptions along with the order form available at www.catamaranrx.com. Call Member Services anytime at 1.888.637.5121 with questions.

What Are Specialty Drugs? Specialty drugs are high-cost injectable, infused, oral or inhaled medications that are typically prescribed to treat chronic or long-term conditions that have few or no alternative therapies.

This includes (but is not limited to) medications for cancer, HIV/AIDS, hepatitis C, multiple sclerosis and others. All specialty drugs must be filled through BriovaRx, Catamaran’s Specialty Pharmacy, and includes drugs such as Humira, Atripla, Betaseron, Remicade, Norditropin, Copaxone, Sovaldi, Tecfidera and Pulmozyme. For more information and for a full list of specialty drugs, visit www.catamaranrx.com, BriovaRx.com or call 1.855.427.4682.

Clostridium Difficile Clostridium Difficile (C. diff) is an antibiotic-resistant bacteria and the most common cause of contagious diarrhea in hospitals. Controlling this bacteria in our hospital is extremely important. During February, the Infection Prevention and Control department will be working to educate staff through inservice meetings and employee education about the proper precautions and ways to reduce the incidence of this hospital-acquired infection.

Who is most likely to contract C. diff? Patients who: Require prolonged use of antibiotics Have had gastrointestinal surgery or procedures With a weak immune system With a chronic condition Who have been in the hospital for an extended period of time Who are elderly

If C. diff is suspected, laboratory testing should be performed immediately. It is important to practice proper hand hygiene and obey all contact precautions when caring for patients with C. diff. Wearing a gown and gloves, and handwashing with soap and water is the best defense against transmitting C. diff to other patients, alcohol-based hand sanitizers are not effective against the bacteria. C. diff spores can live for months on hard surfaces and must be cleaned with a bleach solution.

Please visit the Infection Prevention and Control page on CHIP to learn more or call Eric Lee, Infection Preventionist at extension 2930.
