ofthesharedplans,isanumberindicatingthenumberofformsthereareforaparticularplantype.Pleasenote,EndofLifeplanswillonlyeverhaveoneplanwithallcarerscontributingtothesameplan.Wheretherearenoplans,clickthe+buttontocreateanewplanorclickthe‘eye’buttontoamendorviewanexistingplan:

practices.Onsavingtherecords,theinformationissavedtothecentralrepositoryandanotificationissentbacktotheGPPracticetoreviewthechangesandacceptbacktotheGPrecord.

ToacceptamendmentsbackintoaGPrecord:1. WhereHCPsoutsideoftheGPpracticehavemadeamendments,anominated

emailaddresswillreceiveanotificationtoindicatethatarecordhasbeenadjusted.
