ELAINE: European Local Authorities Integration NetworkInvitation to the fifth International conference in Maastricht

Theme: health and integration of migrants and refugees

More specifically, there will be given presentations of:integrated approach for the benefit of health, (public) health system, physical and mental health during flight and stay in exile, socioeconomic factors related to health, support and health care to unaccompanied minor refugees, special healthcare in intensive supervised shelters for refugees with malfunctioning in one or more life areas, transcultural psychiatry, heath education, researches in relation to education and mental health issues, trauma and resilience, social work in relation to mental health and positive well-being, humanitarian corridors on integration and wellness of refugees, creative therapy, talent development.

IntroductionTogether with four local or regional organizations in Sweden, England, Denmark and Spain the municipality of Maastricht has applied for a subsidy for the project ‘ELAINE’ in the context of the ‘Europe for Citizens Programme 2014 – 2020’. At the end of 2015 this has been selected for receipt of a grant.

WhatThe ELAINE-project, European Local Authorities Integration Network, will connect five local and regional European organizations on the theme of integration of migrants and refugees. The project comprises five separate meetings on the topic of integration in five different participating member states. Each meeting will have its own specific focus on integration.

ConferencesThe conferences in Sweden, England, Spain and Denmark took place in 2016. In Sweden, the theme was integration through entrepreneurship and employment programmes. In England, the theme was integration through voluntary and public sector partnership. In Spain, the conference was about sport and cultural activities as a mean for the integration of immigrants and refugees and in Denmark it was about language acquisition and integration.

The fifth meeting in the Netherlands will be held in Maastricht from 19 to 21 April 2017.The topic is ‘health and integration of migrants and refugees’. The meeting will consist of lectures, workshops and other presentations of good and relevant examples, good practices, experience in and research on the topic, followed by workshops. Spoken language at all conferences is English.

In 2017 it is 25 years ago that the Maastricht Treaty was signed in Maastricht: the birth of the European Union and the Euro. The ELAINE conference is part of the programme of Europe Calling!

WhoThe meeting is for you who work with, or otherwise have an interest in (health and) integration of asylum seekers, refugees, unaccompanied minors and migrants on the local level in the municipality of Maastricht and the province of Limburg, on a national level in the Netherlands, and on an international level for our foreign participants. Among you are professionals in healthcare, researchers, policymakers, students and volunteers.

The second and main conference day will be held at the city hall. Maastricht’s city hall is located at the centre of Maastricht.

In one room of the city hall, the Council chamber of the Mosae Forum, the City Council normally meets once every month at 5 PM. The City Council is the highest governing body in the municipality and is elected by the residents. In this council chamber the conference will start at 9.30. Between 9.00 - 9.30 arrivals and registration will take place.

Civic integration of refugees requires an integrated approach. Employment, participation, education, language acquisition are not only interrelated with each other, but are also related to the health of people. For example, language competence and a good health are conditions for employment. Health is no longer considered as a static condition of the “complete physical and mental wellbeing” but rather as the dynamic ability to adapt and to self-manage one’s own well-being, according to Machteld Huber. She named this “positive health”. Pursuing goals, participation and employment can promote health. So, the circle will be finalized. During this presentation you will get insight in how the themes of the 5 international conferences are related and can only be successfully addressed when combined.

10.30 - 11.00 An overview of the (public) health system in the Netherlands for asylum seekers and refugees

The current procedure for an asylum seeker to receive refugee status in the Netherlands requires refugees to be housed at several locations during the waiting process. Upon, receiving a positive refugee status, they are provided permanent housing in a municipality. During this process both asylum seekers and refugees have the possibility to make use of health care and public health care. In this presentation, you’ll get an overview of the organization and financing of the (public) health system in the Netherlands, compared to the system in the other participating EU-countries.

How is their health in different stages of their flight and stay in exile, which socioeconomic factors are of influence and what could a local government do to prevent health disparities in refugees?

Several armed conflicts in the world have resulted in unprecedented mass displacement and forced migration, with many people seeking refuge outside their region of origin. In the Netherlands, by February 2017, 25,485 asylum seekers would live in special Centres (AZC), being 63% of that population consisting of males. The asylum-seeker and refugee population have faced many acculturation challenges since their arrival. They are often keen to maintain their heritage culture but also to identity and seek contact with the host culture. The success of the acculturation

process depends on many variables, related to the individual, the context, but also on

the historical and political scene of the host country and the support received by that vulnerable population. Also the acculturative challenges that these individuals have to face can be influenced by several factors, such as gender, background, age, education, previous experiences and others. Some of these issues have been investigated, in the fall of 2016, by the University College Roosevelt, a liberal arts and science honors college of Utrecht University, in Middelburg, Zeeland, the Netherlands.

From the conducted researches on asylum-seekers and refugees, in relation to education and mental health issues, the results of the following investigations will be presented and discussed: Mental Health resilient factors of adult Syrian refugees in the Netherlands (Kristen

Workshop 1:Unaccompanied minor refugees with rejected asylum application - 45 minutes; 15 persons

An unaccompanied minor refugee is defined as someone who is under the age of 18 and has travelled without a parent/guardian to seek asylum in the Netherlands. Furthermore, their country of origin is outside of the EU. Nearly 90.000 unaccompanied children sought asylum in Europe in 2015. In the Netherlands, they are allowed to stay until they turn 18 years old. In most cases, the minors will know the result of asylum application by the age of 16 or 17. If their asylum application is rejected, they have to return to their country of origin direct after their 18th birthday. It’s a challenge for the government, school and professionals to facilitate appropriate support and health care to these minors. Currently there is a rise in unaccompanied minor refugees from Afghanistan with a rejected asylum application. This rise is accompanied by increased psychological problems such as sleep problems, depression, auto mutilation and aggressive behaviour. Lack of concentration and motivational may also occur, which can lead to troubles at school. In this workshop, we will focus on this problem and exchange (possible) solutions. What possibilities are there on the level of policy, school, health care workers and housing counselling?

The goal is to create a mindmap together and get inspired for change!

Workshop 4:Humanitarian Corridors and Wellness - 45 minutes; max. 25 persons

Currently in the Europe are approaches to refugee settlement where asylum seekers and refugees face long waiting periods, in large-scale camps, with little to no opportunity to structure their lives, or participate in society (Korac, 2003). Parallel to the current approaches are alternative approaches to refugee settlement, facilitated through charities and citizens. These alternatives focus on integration and inclusion in the community. As well, European countries are now exploring alternatives to current procedures. These alternatives, often known as Humanitarian Corridors, are being trialed in countries such as Italy (Santegidio, 2016), while well-established examples are seen elsewhere, such as Canada’s “private sponsorship” model (Carter et al., 2008). The workshop leader examines these alternatives through the lens of occupational science. In occupational science and occupational therapy, the core concept is occupation: the meaningful activities individuals do which give structure and meaning to their daily life. In this area of science, there have been many studies which examine the lack of engagement in meaningful occupations of populations such as refugees and asylum seekers, and the impact this has on their mental health and eventual inclusion in society (Townsend & Wilcock, 2004; Whiteford, 2005; Kronenberg & Pollard, 2005). During this workshop the participants will explore and discuss the impacts of humanitarian corridors and possible humanitarian corridors on integration and wellness of refugees.

Workshop 7:Movement without boundaries, dancing across border - 45 minutes; max. 30 persons

We will share our experience and knowledge about dance/movement therapy with refugees. In the workshop people can experience for themselves how dance therapy works, based on a few activities that worked really well with refugees.In this presentation, the characteristics of dance movement therapy with refugees and its potential will be discussed and clarified by introducing best practices. It is a challenge to link the fields of research, education, and clinical practice. But also to link the field of refugee, with or without psychiatric problems. These fields have different basic principles, goals, and emerge from different cultural landscapes, which makes it difficult to bring these fields together. ‘Together’, ‘look for a similar focus’, ‘share’, relax’ and ‘express yourself’ are essential action to bridge these cultural landscapes.

An introduction into the Dutch health care system will be presented. It is a complicated system, so we try to be as clear as possible on how it works. After that we will describe a disease, such as diabetes, in a way that is understandable for people who speak not Dutch nor English.These workshops are given to immigrants who have already entered the Netherlands. With these workshops we hope they understand how the Dutch health care system works and how they can get health care if they need it. With the description of the disease we hope they get an understanding in how it works and how to prevent it or prevent it from getting worse.

Presentation about the background of the project ‘refugees in motion’. Why this project is started, the importance of doing and using movement at a refugee camp. Explanation by the coaches in movement how they work at the AZC. They will share some of their best practices they had at the different camps they worked in. Besides that they want to let you feel what moving does with your body and psychological state.

Presentation and explanation about the Erasmus+ programme and in which way participating in a Erasmus+ project can contribute to the physical and mental health of refugees. A short movie of 10 minutes, made by some refugees during the last project in the Netherlands, will be shown. It’s a personal story of a refugee (subtitled in English).

Workshop 11: Infant Mental Health: Parents/child interaction risks in the context of forced migration – maximum 45 minutes; as many persons as possible

Interaction can be defined as the reciprocal parent/child reaction, which happens in a continuous developmental process, and implies in physical and psychological regulation mechanisms. This process can be affected by several factors and contexts, from the infant, the parents, the environment and the circumstances. The model of assessment and intervention to be presented and discussed in this symposium was developed in the Netherlands, for diagnosis, prevention and treatment of infant’s functional and developmental disorders in relation to parents/child interactions, focusing in the risks and concerns related to the context of forced migration in the population of asylum seekers and refugees.

The parent’s psychological trauma can have profound effect in the infant’s mental health development, determining heterogeneous and complex manifestations and symptoms, characterizing a secondary trauma. In that context the influence of culture must be taken into account in shaping the traumatic experience and its resilience. Throughout the parents/child emotional communication and interaction, the infant, directly or indirectly, is affected by the parents’ trauma, as well as by the traces trauma leaves in their relationship. These traces can negatively influence the child’s development, not only in the beginning of his/her life, but also in his/her future as adolescent and adult. The mother/child psychotherapy presented (also in video) integrates dynamic, systemic and transcultural approaches in psychological trauma treatment of parents and infants, combining treatment of the trauma as well as the parents/child interaction. Therefore, in the treatment pretends to: foster realistic responses to threat; help to recover/maintain regular levels of affective arousal; help to build reciprocity in the interactions; normalize traumatic responses; encourage differentiation between reliving and remembering; place the traumatic experiences in perspective. In parallel, the treatment focus in the parents/child’s interaction aims to develop the parents’: sensitivity; communication; attuned emotional exchanges; involvement and responsivity; structure and positive regulation of the interaction and play; positive representation of the child; as well as synchronicity in the relationship. These aspects should also be developed by the child in interaction with the parents. The techniques discussed will be illustrated by cases presented in video.

Workshop 12:QCC Resilience model: A psychological and pedagogical working model to enhance resilience in refugee children – 45 minutes; number of persons undefined The workshop will entail a short introduction into the "QCC psychological/pedagogical resilience model", a workingmodel regarding the reinforcement of resilience in refugee children.The QCC resilience model is a model based on research findings regarding resilience related factors in children and their families. The QCC resilience model is an interactional model that places strong emphasis on child-related and environmental related (parent, community, ...) factors, as the interaction between both, in order to reinforce resilience in children of different cultural background including refugee children and their families. The model provides several concrete working domains in order to reinforce resilience that can easily be applied on the working floor for people working with refugee children/unaccompanied minors in different settings (therapy, schools, asylum centres, ...). During the workshop we will be focussing on describing the model, sharing experiences in working with the model in asylum centres in Belgium as sharing some concrete tools, sharing the QCC working guide as sharing some material that can be used in applying the model.

Continuation of ‘ELAINE’The conference in Maastricht will be the last of the five international conferences. Next will be the Dissemination Seminar in Brussels, 17th - 19th October 2017.

The dissemination seminar in Brussels is in order to reach a large geographical representation and to reach European politicians and officials. It’s also to reach networks such as the European Migration Forum and ERLAI. The purpose is to spread the results of the project through these platforms. The dissemination will also target representatives in the European Economic and Social Committee, the Committee of the Regions, the European Parliament and the EU Commission.The expected result is to deliver the experience, good and bad, from the municipalities to higher European level and policy makers. The purpose is to provide the policy makers with the local levels opinions and experience in the field of integration of migrants and refugees.
