• Goals/Objectives
• Expected Outcomes

• Practices
What needs to be done? Develop a Plan

Assign Responsibility and ensure Accountability:
What,
Who,
When,
Where,
How
Implement the Plan
Review the outcome
Planning What is Situation
What is our Mission (Objective)
How do we Implement the plan (Who, What, Where, When, How)
What other Support do we need? (Who else?)
What Controls are we putting in place
What Reports do we need to generate and who needs it.
Implement the plan...
• Activities to be done
• Who will do it
• When do we need it? (Timelines)
• What are our constraints? (Time
constraint etc.)
• On-going evaluation to ensure
that it is going as planned
• If not, what changes are
required
The Four Questions:
What did we intend to do?…what was our goal/mission/expectations?
What really happened?…what did we do? (process/steps etc.)
Why did it happen?…what went well?…what went wrong?…what can be improved?
What are we going to do about it?…what actions or changes are needed to
improve?
