EVIDENCE-BASED SELF-MANAGEMENT PROGRAMS FOR
ADULTS WITH CHRONIC HEALTH CONDITIONS
Presentation Objectives:
At the conclusion of the training, participants will be able to demonstrate their knowledge by: • Explaining the core components of Stanford University
School of Medicine Patient Education Research Center’s evidence-based Chronic Disease Self-Management Education (CDSME) Programs
• Discussing the effectiveness of the CDSME Programs. • Listing the available CDSME Programs in New Mexico.

No Personal Conflicts of Interest as this is a program delivered through the City of Albuquerque Department of Senior Affairs to the general public for no charge
Principle funding is through a federal grant from the ACL
Some contributions and support provided by various health care organizations for program delivery and participant referrals
Stanford University’s CDSME Programs: 4
Peer led small groups Standardized training for leaders Highly structured teaching protocol Standardized participant materials Several topics per session Well researched and studied
• Evaluated in randomized trials to establish the evidence base
Available in English and Spanish
What is Program Fidelity and Why Bother?
At the community and organizational level, program fidelity refers to the how closely individuals and staff follow the program that the developers provide. This includes consistency of delivery as intended as well as program timing and costs.

The Stanford CDSME programs are designed to be as near as possible culturally neutral.

Workshop Leaders - are authorized to only facilitate community workshops.
Master Trainers - are authorized to train individuals to become certified Leaders. Can also facilitate the six-week workshop.

What Makes the Programs Effective?
It is the process in which the program is taught that makes it effective. Classes are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives.
