Gweithio trwy’r deunyddiau dysgu ‘rhagoriaeth wrth gynllunio gofal a thriniaeth’
Mae’r deunyddiau dysgu i gefnogi rhagoriaeth wrth gynllunio gofal a thriniaeth yng Nghymru yn ceisio bod yn hyblyg. Mae hynny yn rhoi cyfle i hwyluswyr ddewis adnoddau dysgu sy’n addas i’r gynulleidfa.
Mae’r deunyddiau wedi eu rhannu’n Unedau Craidd (UC) ac Unedau Ychwanegol (UY). Mae’r unedau craidd yn dilyn ei gilydd ac yn anelu at adolygiad cynhwysfawr o gynllunio gofal a thriniaeth ynghyd â’r ddeddfwriaeth a pholisi ategol Mesur Iechyd Meddwl (Cymru) 2010. Dylid cwblhau’r unedau craidd yn y drefn y maen nhw’n ymddangos. Ond, gall hwyluswyr dysgu (ar ôl iddynt ddod yn gyfarwydd â’r adnoddau hyn) addasu a newid trefn yr unedau yn dibynnu ar y gynulleidfa dan sylw. Navigating the ‘excellence in care and treatment planning’ learning materials
The learning materials to support excellence in practice for care and treatment planning in Wales are designed to be flexible. This will allow facilitators the opportunity to match learning resources to different audiences.
The materials are separated into Core Units (CU) and Additional Units (AU). The core units are sequential and are designed to provide a comprehensive review of care and treatment planning and the legislation and policy supporting the Mental Health (Wales) Measure 2010. The core units are designed to be completed in the order in which they are presented. However, we believe facilitators (once familiar with these resources) can adapt and re-order these units depending on particular audiences. Core units may be delivered in single blocks or joined together to form larger units. In circumstances where units are joined we ask facilitators to think carefully about adapting the case studies/personal scenarios in these materials in order that participants may follow the same personal scenario throughout the combined unit.
The additional units are designed for particular audiences, such as those requiring a general awareness about care and treatment planning and care coordination in the Mental Health (Wales) Measure (Additional unit 1:‘An overview of the Mental Health Measure - care and treatment planning’); those people working in Child and Adolescent Mental Health Services (CAMHS) (Additional unit 2: ‘Children & young people and care and treatment planning’); practice teams (Additional unit 3: ‘Leadership and team development) and those with senior management responsibility (Additional unit 4: Care and treatment planning: organisational responsibilities).
Additionally the units are divided into blocks of time and again can be delivered in a flexible manner. Therefore there is no requirement that these resources are delivered within a formal training environment. We would encourage people to think creatively about how elements of these units can be provided in team meetings or similar situations.
Presentation of these Resources
These resources are fully bilingual and we consider the presentation of Welsh & English versions sends an important message on the equal standing of both languages in Wales. The Welsh language has a special place in the legislation and we intend that these resources reflect that position.

‘Gwellhad, dealltwriaeth ddiwylliannol, cymhlethdod wrth gynllunio gofal a thriniaeth’
Bwriad yr adnoddau hyn yw cefnogi gwelliant parhaus ym maes gofal iechyd meddwl a chefnogi prif elfennau Rhannau 2 a 3 o Fesur Iechyd Meddwl (Cymru) 2010. O ganlyniad, maent yn cynnwys nifer o brif themâu:
• Gwellhad.
• Dealltwriaeth Ddiwylliannol.
• Cymhlethdod.
Nod cyffredinol yr adnoddau yw cefnogi’r dasg o gynllunio gofal a thriniaeth mewn ffordd safonol a chyson yng Nghymru i bawb sy’n derbyn gwasanaethau iechyd meddwl eilaidd. Mae hefyd yn helpu i ateb y gofynion deddfwriaethol sy’n deillio o Ran 2, Mesur Iechyd Meddwl (Cymru) 2010.
Bydd yr adnoddau hyn yn:
• Rhoi cyfle i gyfranogwyr ddeall gofynion Mesur Iechyd Meddwl (Cymru) 2010.
• Cynnig addysg a hyfforddiant ar gynllunio gofal a thriniaeth i gynulleidfa eang yn cynnwys defnyddwyr gwasanaeth, gofalwyr, clinigwyr, partneriaid y 3ydd sector, rheolwyr gwasanaethau iechyd meddwl a phartïon eraill â diddordeb.
• Adlewyrchu’r cyd-destun Cymreig unigryw ar gyfer cynllunio gofal a thriniaeth.
• Helpu i ddatblygu cynlluniau gofal a thriniaeth sydd â phwyslais ar wellhad a dealltwriaeth ddiwylliannol yn y gwasanaethau iechyd meddwl eilaidd i bobl o bob oed.
A) Gwellhad
Credir yn gryf bod dull sydd â phwyslais ar wellhad yn cynnig ffordd integredig a defnyddiol i ddarparu gofal, triniaeth a chefnogaeth effeithiol i ddefnyddwyr gwasanaeth, gweithwyr a gwasanaethau iechyd meddwl, ac mae’r unedau astudio canlynol yn seiliedig ar y gred honno. Mae mwy nag un dehongliad yn bosibl o’r cysyniad o ‘wellhad’, ond un thema sy’n gyson ym mhob un ohonynt yw ‘gobaith’. Ystyr gwellhad mewn gofal iechyd meddwl yw bod pob unigolyn yn gwneud ymdrech barhaus i fyw bywyd ar eu telerau eu hunain, gan adlewyrchu eu gobeithion a’u huchelgais.

Introduction to the units of study and the key themes:
‘Recovery, cultural capability, complexity in care and treatment planning’
These resources are intended to support continuous improvement in mental health care and are designed to support the key elements of Parts 2 and 3 of the Mental Health (Wales) Measure 2010. In this way, they incorporate a number of key themes:
• Recovery for all.
• Cultural Capability.
• Complexity.
The overall aim of these resources is to support high quality and consistent delivery of care and treatment planning in Wales for all persons in secondary mental health services, and which support delivery of the legislative requirements arising from Part 2 of the Mental Health (Wales) Measure 2010.
These resources will:
• Provide an opportunity for participants to understand the requirements of the Mental Health (Wales) Measure 2010.
• Offer education and training on care and treatment planning to a broad audience including service users, carers, clinicians, 3rd sector partners, managers of mental services and other interested parties.
• Reflect the unique Welsh context of care and treatment planning.
• Support the development of recovery and culturally capable orientated care and treatment planning in secondary mental health services for persons of all ages.
A) Recovery for all
The following units of study are based on a strong belief that a recovery orientated approach offers service users, mental health workers and services with a helpful and integrated approach to effective mental health care, treatment and support. The concept of ‘recovery’ has many interpretations, but consistent within all of them is the theme of ‘hope’. Recovery in mental health care is about each person making a continued effort to lead a self-determined life that reflects their hopes and ambitions.

Gwellhad i blant a phobl ifanc a phobl hyn sy’n dioddef o ddementia
Gellir defnyddio egwyddorion gwellhad mewn ffordd wahanol yn dibynnu ar y gwasanaeth, er enghraifft, pobl hyn, plant a phobl ifanc, pobl ag anabledd dysgu ac ati. Fel hwylusydd, bydd angen i chi annog trafodaeth a defnyddio egwyddorion gwellhad yn ystyrlon.
Er mwyn helpu’r broses hon rydym wedi cynnig rhai syniadau cychwynnol yma.
• Ymrwymiad i wneud cynnydd
Mae hwn yn ymrwymiad hirdymor ar y cyd rhwng defnyddwyr gwasanaeth a gweithwyr gofal iechyd meddwl. Maen nhw yn ymrwymo i wneud cynnydd (pa mor gyflym bynnag y mae hynny’n digwydd). Mae’r pwyslais ar ymateb i anghenion cymdeithasol, ysbrydol, corfforol a seicolegol yr unigolyn ac ystyried bod rhain yn ffactorau pwysig ym mywyd pobl. Yn achos pobl hyn, mae hynny yn bwysig er mwyn sicrhau urddas y person ym mhob agwedd o’u bywyd. O ran pobl ifanc, gall olygu rhoi sylw i’w hanghenion unigol yng nghyd-destun y rhwydwaith teuluol.
• Grymuso a Hunan Reolaeth
Mae grymuso’n cynnwys elfennau o ddewis, pwrpas, cynhwysiant a hunaniaeth bersonol. Mae’r rhain yn agweddau pwysig wrth ddarparu gofal o’r radd flaenaf i bobl hyn sy’n dioddef o ddementia. Mae’n ymrwymiad i’r unigolyn yn gyntaf ac i’r salwch yn ail. Gyda phlant a phobl ifanc, credir ei bod yn bwysig cefnogi’r person i ddatblygu trwy wneud penderfyniadau a byw’n iach. Gyda phobl sy’n dioddef o ddementia, gellir gwneud dewisiadau ar sail gosodiadau cynt sy’n help iddyn nhw gynllunio ar gyfer y dyfodol.

The Welsh Government have supported the key components of a recovery approach, being
1. A commitment to progress
2. A commitment to the whole person and
3. Empowerment and self management
Recovery for children and young people and older people with a dementia
The principles of recovery may be applied differently across services, for example older people, children and young people, people with a learning disability etc. As a facilitator, you will need to promote discussion and thoughtful application of recovery principles within these settings.
In order to assist this process we have generated some initial thoughts here.
• A commitment to progress
This represents a shared and long term commitment on both the service users and mental health care workers to maintain a commitment to progress (at whatever pace that happens). However progress can also refer to ‘maintenance’ and in older person’s care, this can relate to making efforts to maintain the person and their quality of life for a long as possible during the progression of the disease.
• A commitment to the whole person;
This means making a concerted shift to practising more inclusively, seeing and responding to the person, not a diagnostic label. The emphasis is on responding to the social, spiritual, physical and psychological areas of the individual and seeing these as important in people’s lives. For older people this is an important in maintaining dignity of the person in all aspects of their daily life. In young people it may mean ensuring their individual needs are considered and met within the context of a family network.
• Empowerment and Self Management
Empowerment has elements of choice, purpose, inclusion and personal identity. These are important aspects of high quality care provision for older people with a dementia. It is a commitment to the person first and their illness second. In children and young people it is a belief in supporting the person to develop through decision making and maintaining healthy lifestyles. With people with dementia choice might be exercised through advance statements that can help them to plan for their future.
Self management is inextricably linked to empowerment. It involves finding meaningful ways to live life with mental health problems. This changes the nature of helping relationships towards coaching and partnership.

Mae’r pwyslais hwn ar wellhad yn berthnasol i’r dull o ddarparu gwasanaeth i bobl hyn, wrth i ni feddwl sut y gall ymarferwyr gefnogi’r unigolyn i fyw gyda’i gyflwr a’i symptomau o ddydd i ddydd. Mae’n ymrwymiad i ofalu am les yr unigolyn, gan weithio ar sail ‘yr hyn sydd’, ‘yr hyn a allai fod’ a’r ‘hyn a ddylai fod’. Ystyr gwellhad yw byw gyda’ch sefyllfa a gwneud y gorau o’ch bywyd.
Defnyddiwch eich amser i helpu cyfranogwyr i feddwl sut mae’r egwyddorion hyn yn berthnasol yn eu gwaith.
B) Dealltwriaeth ddiwylliannol
Mae’r deunyddiau hyn yn seiliedig ar yr ymrwymiad ei bod yn rhaid i’r gweithlu iechyd meddwl weithio tuag at lefel uwch o ddealltwriaeth ddiwylliannol wrth drin a chefnogi’r boblogaeth ddwyieithog (Cymraeg/Saesneg) yng Nghymru.
Diffinio Dealltwriaeth Ddiwylliannol
Mae dealltwriaeth ddiwylliannol yn cyfeirio at werthoedd, ymddygiad, agweddau ac ymarfer mewn sefydliadau, gan dimau ac unigolion. Caiff ei nodweddu gan barch tuag at gredoau, ieithoedd ac ymddygiad diwylliannol pobl sy’n defnyddio gwasanaethau a chydweithwyr. Mae’r ffocws hwn yn cydnabod mai proses ddynamig yw hon sy’n golygu bod angen i bobl fyfyrio’n barhaus ar eu hymarfer a’i newid os oes angen.
Un elfen hanfodol o ddealltwriaeth ddiwylliannol yw’r cysylltiad rhwng agweddau ac ymddygiad. Wrth ysgrifennu’r deunyddiau hyn y bwriad oedd bod ymarferwyr a sefydliadau yn ymateb yn briodol i ddisgwyliadau a hawliau diwylliannol. Yng Nghymru, mae cael mynediad at siaradwyr yn Gymraeg neu yn Saesneg, yn ôl angen yr unigolyn, yn ystyriaeth bwysig wrth ddarparu gwasanaethau iechyd meddwl. Mae’r mater hwn yn bwysig i rai sy’n siarad Cymraeg, er nad yw wedi ei gyfyngu’n unig iddyn nhw.
Mae’r ddogfen ddefnyddiol ‘Dwy Iaith: Dau Ddewis’ (Cyngor Gofal Cymru, Elaine Davies1) wedi helpu i ganolbwyntio ar y materion pwysig y dylid eu hystyried wrth ddarparu gofal yng Nghymru:
• Mae gwahaniaeth rhwng gallu iaith a defnyddio iaith – gall pobl siarad dwy iaith ond efallai mai un iaith yn unig y maent yn ei defnyddio go iawn, a’u bod yn fwy cyfforddus yn defnyddio un o’r ddwy iaith (neu fwy).
• Gall hyfedredd ieithyddol pobl amrywio yn y pedwar sgil iaith (siarad, darllen, gwrando ac ysgrifennu). Gall pobl newid rhwng ieithoedd ar gyfer pob un o’r sgiliau hyn a rhaid i ymarferwyr fod yn effro i hynny.
• Ychydig o bobl sy’n rhugl a’r un mor fedrus yn y ddwy iaith. Mae un iaith yn tueddu i fod yn gryfach na’r llall.
• Mae gallu ieithyddol pobl yn tueddu i amrywio dros amser. Wrth fynd yn hyn nid yw’n anarferol i bobl ddewis defnyddio eu mamiaith unwaith eto i gyfathrebu.
Gofynnwn i hwyluswyr a dysgwyr roi sylw i’r materion pwysig hyn wrth iddynt weithio trwy’r defnyddiau hyn:
“Rwy’n teimlo’n fwy cartrefol wrth siarad fy mamiaith. Mae fel bod gartref gyda’r holl bethau cyfarwydd a chyfforddus o’ch cwmpas. Mae siarad ail iaith fel teimlo eich bod yn nhy rhywun arall.” (Ellen: siarad Cymraeg/Saesneg)2.

This focus on recovery, has application for an approach to older people in thinking about how practitioners, can support the person to live with their condition on a day by day basis. It is a commitment to well-being, to work to ‘what is’, ‘what might be’ and ‘what should be’. Recovery is living with your situation and making the most out of life.
Use your time to support participants to think about how these principles apply within their working practice.
B) Cultural Capability
These materials are based on a commitment that the mental health workforce must work towards greater cultural capability in treatment and support of the bilingual Welsh/English speaking population in Wales.
Defining Cultural Capability
Cultural capability refers to the values, behaviours, attitudes and practice in organisations, teams and people. It is characterised by respect for beliefs, languages and cultural displays of behaviour of people who use services and work colleagues. A belief in capability is based on an understanding that this is a dynamic process which requires people to continually reflect upon their practice and make refinements where necessary.
A critical element of cultural capability is the link between attitudes and behaviour. These materials are written with the intention that aspects of cultural rights and expectations are met by practitioners and organisations. In Wales, ensuring access to Welsh or English speakers, depending on the needs of the service user, is an important consideration in the delivery of mental health services. This issue is significant for, but not exclusive to, speakers of the Welsh language.
The helpful publication ‘Different Words: Different Worlds’ (Cyngor Gofal Cymru/Care Council for Wales1) has provided a focus on the important issues for care delivery in Wales:
• There is a distinction between ability and use of a language – A person may speak two languages but may only use one in practice, they may be more comfortable using one of their two (or more) languages.
• A person’s proficiency in language may vary across the four language skills (speaking, reading, listening and writing). A person may switch between languages for each of these skills and practitioners must be alive to this issue.
• Few bilingual people are fluent and equally competent in both languages. One language tends to have a dominant position.
• A persons’ competence in language may vary over time. It is not uncommon for people as they get older to return to their native language as a preference in communication.
We request facilitators and learners to attend to these important issues as they work through these materials:
“I feel more at ease speaking in my mother tongue. It’s like being at home with all the usual familiar worn and comfortable clutter around you. Speaking a second language is like being you, but in someone else’s house” (Ellen: Welsh/English speaker)2.

B) Complexity
Another important principle is that of complexity in the presentation and experience of mental health difficulties. The learning materials are intended to support reflection on actual practice, but we have no doubt that mental health practice is even more complex and ‘messier’ than that presented here.
Therefore, your engagement as either facilitator or participant represents a partnership to deepen understanding about complexity in practice. Learning is about transformation, by which we mean the ability of the participant and facilitator to expand their range of possible actions. We encourage you to be flexible in your approach to learning and facilitation, but always with your eye on supporting rVecovery and acting in a culturally capable way.

Information for facilitators: how to use these learning resources
Who is this manual for?
This manual is a freely available learning resource for updating, training and supporting health and social care workers, service users and carers about care and treatment planning. It is grounded in the requirements of the Mental Health (Wales) Measure 2010 and the policy and guidance associated with it.
How to use this manual and video resourcesEach unit of learning is structured as follows:

Ideally, the training team should consist of at least two facilitators’, including a service user. Co-facilitation with service users and carers can offer an important alternative perspective on aspects of care and treatment planning. Bilingual training reflects the spirit and cultural values within the Mental Health (Wales) Measure 2010 and the Welsh Language Measure (2011) and these materials lend themselves easily to such an approach.
The video resourcesThe video resources consists of a library of video and audio narrative clips of service users, carers and practitioners talking about their experiences and observations of mental health and services in relation to care and treatment planning. Sometimes the speakers will talk about CPA and this should be viewed as, care and treatment planning by another name. These narratives add real value to the learning resource. The clips may be used as triggers for problem based learning activities or to affirm or negate points of view. The trainer’s manual indicates where these short narratives may be used to prompt discussion and learning that is relevant to the theme of each unit of learning. You may use as many or few of these as fits local need and circumstance.

Location of training
Having dedicated training time is an excellent opportunity for practitioners to spend time away from usual clinical/practice situations in order to examine successful interventions and identify areas for further improvement. We strongly urge training facilitators, training managers and participants to review where they conduct training programmes. Using alternative resources, such as 3rd sector charities, community mental health centres etc., as training venues may foster multi-agency dialogue and result in added value from a training programme.

Facilitator preparation
• Familiarise yourselves with the learning resource: the manual; the DVD; the handouts and which units and activities you will include in the programme.
• Recruitment and information: liaise with relevant others about attendance and how participants will be given information about the programme.
• Meet with your co-trainer(s) sufficiently in advance to decide respective roles and responsibilities and plan the units. Plan in advance how you will use the resource, which activities, PowerPoint presentations, DVD narratives etc. Some of the suggested exercises will require careful planning and allocation of specific tasks.
• Check the venue is suitable and please note our suggestion earlier on the range of possible learning venues. Think about size and resources for breaking out into groups. Find out domestic information so that you can pass on to participants: fire regulations, refreshments, toilets, quiet spaces for time out, telephones etc.
• Equipment: The DVD can be played on PCs or on TV with a DVD player. If you are using a PC, you will need speakers (the in-built speakers will NOT be loud enough). The clips will run on Windows Media Player, Real Player, Nero or QuickTime (free download at: http://quicktime.mediaplayer-stop.com/). Other equipment: data projector, the DVD; flip chart, stand and markers; removable adhesive for posters. Check what is provided at the venue.
• Resources: sufficient handouts and briefings for the group.
• Time management: allow sufficient time to set up the room and prepare equipment prior to start times.

Introducing reflective learning
Much of the training in these units requires you to reflect on your experience. Prior to commencing the learning it is worth briefly considering what reflection is.
Reflection is more than just thinking about an experience and involves an element of change. It involves the reflector in reorganising their thoughts, knowledge, feelings and actions. In other words “making sense” of experiences and potentially responding to them in different ways. Often this can involve a more critical approach in which we challenge our taken for granted views of experience and subsequently amend our thoughts, feelings and behaviour.
A useful way to reflect is to use a simple framework such as that proposed by Gibbs6. This framework is a cyclical model and includes the following stages:
Description – What is happening?
Feelings – What am I thinking and feeling about what’s happening?
Evaluation – What’s good/bad about this experience?
Analysis – What sense can I make of what’s happening?
Conclusion – how might I want to respond – what are my options?
Action Planning – What will I do?/How will I respond?
The icons used in these resources
To provide some aid to you as a facilitator, we have included a small number of icons to help you understand what you may have to include in your training. You are not bound by these instructions, but they may prove helpful as you begin to become familiar with the resources. The icons are:
Alert icon: This indicates that the subject matter is potentially contentious and so be mindful of being able to manage difference of opinions in the training group.
DVD icon: This indicates that you may wish to consider using a DVD or audio clip (from the menu list) at their point of the training. The DVD clip is designed to reinforce a particular learning outcome or to prime participants for a learning outcome.
Exercise icon: This indicates there is a group exercise at this point in the training (usually involving group discussion, negotiation and agreement).
Handout icon: This indicates there is a handout available for participants.
Slide/PPT icon: This identifies where power-point slides may be used to support learning.
