SPECIAL INFORMATION relating to the HMA BOARD AWARD
THE HMA BOARD AWARD IS A DISCRETIONARY AWARD.
THE CRITERIA FOR THE HMA BOARD AWARD ARE BASED ON THE COLLECTIVE DATA FROM ALL OTHER SUBMISSIONS A COMPANY HAS ENTERED IN, EXCLUDING THE MANUFACTURER OF THE YEAR WHICH IS A SEPARATE NOMINATION SECTION.
ELIGIBILITY
➢ COMPANIES NEED TO HAVE BEEN OPERATING FOR MORE THAN 18 MONTHS
➢ COMPANIES NEED TO SHOW THAT THEY ARE HEADING IN A DIRECTION THAT IS BOTH POSITIVE AND VISIONARY
➢ IF THE JUDGE IS SATISFIED THAT A SUBMISSION IN ANY OF THE GENERAL CATEGORIES (SAFETY, PEOPLE SKILLS AND DEVELOPMENT, ENVIRONMENTAL, PRODUCT DESIGN, INNOVATION, MANUFACTURING PROCESS, TECHNOLOGY, MARKETING, EXPORT), POSSESSES THE RELEVANT EVIDENCE TO ADDRESS THE NECESSARY CRITERIA FOR THE HMA BOARD AWARD, THE SUBMISSION WILL BE FURTHER CONSIDERED FOR THE HMA BOARD AWARD
➢ THE WINNER OF THE HMA BOARD AWARD IS ELIGIBLE TO WIN AN AWARD IN ANOTHER CATEGORY, BUT CANNOT WIN BOTH THE HMA BOARD AWARD AND THE MANUFACTURER OF THE YEAR AWARD.

Terms and Conditions regarding the2017 HMA Board Award
The winner of this Award will be rewarded with an overseas trip for ONE PERSON to a Trade Expo or event, pertinent to the winner's/ company's line of business.
The trip is to take place between October 2017 and October 2018 and will be arranged through the HMA's "In Kind" Travel Sponsors, helloworld, HTG, Newcastle.
The prize covers the cost of 1 economy class return airfare inclusive of taxes and insurance with Qantas Airways (Sydney-Destination-Sydney), plus $2,000 towards accommodation and incidental costs.
As a follow up to the trip, a report is to be submitted (within one month of returning from the trip) to the HMA Board of Directors. The report should demonstrate the benefits and impact of the trip on the person and/or company. The information will be used by HMA to profile and promote future Awards and will be posted on the HMA website to highlight the successful outcomes achieved by winning such a prize.
The Winner may also be requested to speak at one of the HMA's future events to share the experience of such a trip with other manufacturers.
Nominations are open to any manufacturing company that has made an application for an award in at least one or a number of the 2017 listed award general categories - SAFETY, PEOPLE SKILLS AND DEVELOPMENT, ENVIRONMENTAL, PRODUCT DESIGN, INNOVATION, MANUFACTURING PROCESS, TECHNOLOGY, MARKETING, EXPORT.

Excellence in SAFETY
PLEASE READ THE FOLLOWING DEFINITION BEFORE COMPLETING YOUR APPLICATION FORM TO ENSURE YOU ARE SUBMITTING IN THE CORRECT CATEGORY.
HMA IS KEEN TO SEE INITIATIVES THAT HAVE BEEN IDENTIFIED, DRIVEN AND RESOLVED WITHIN OUR REGION (Hunter, Central Coast, Mid North Coast).Nominations are open to any manufacturing company that has introduced an innovative safety program that has contributed to an improvement in the health, safety and well-being of employees, customers or the community (please provide evidence of this where requested, e.g. lost Time Injury / Medical Treatment Injury figures, number of injuries and near misses, number of day’s injury free, impact on workers compensation policy, sick days, etc.).
The judges will look for examples of innovation in safety and safety systems demonstrated by, but not limited to, 'out of the ordinary' safety systems approach, positive impact on employee safety, health and well-being, improved employee motivation and morale, evidence of the development of a 'safety culture', hazard and / or risk / accident reduction and increased awareness of WH&S procedures/policies.
Note - Applicants may find that their application into the Excellence in Safety category may also be a 'fit' for another category, such as People Skills and Development, Manufacturing Process, etc. By adapting the information to fit the criteria of other categories you give yourself the opportunity for your product, process, etc. to be profiled in more than one sector.
EACH OF THE LISTED CRITERIA MUST BE ADDRESSED AND WHERE POSSIBLE, EVIDENCE PROVIDED.Photographs, drawings, etc. may be included as an ATTACHMENT to this Application Form and labelled clearly and then copied onto the USB with your Application Form, e.g. Question 3 – evidence provided.
Please address the following criteria and restrict the content of each response to 200 words or less. i.e. - Each question should only have a maximum of 200 words.
In saying this, don’t undersell your program either.
We suggest that the minimum amount of words used to convey the benefits of your program should be 150.

Any accompanying documentation provided 'as evidence', may either be inserted as part of this form, or, as an attachment to this application.
