\ Layton Construction Company, LLC SAFETY ESSENTIALS Execution and compliance must be valued over PERCEIVED URGENCY PROJECT MANAGEMENT PLANNING (EXECUTION) The Project Management Plan (PMP) is a comprehensive execution plan with the intent of capturing the entire project from start to finish. The Layton Project Team develops the plan and is responsible and accountable for implementation and monitoring the objectives. The PMP will review the schedule of activities and associated risks, as well as evaluate the need for appropriate Layton and Subcontractor safety staffing. SUBCONTRACTOR PREQUALIFICATION Each subcontractor that does not pass Layton’s prequalification process will have additional oversight in the form of a Corrective Action Plan (CAP) that will need to be completed prior to award of contract. The CAP includes additional weekly reporting procedures in BIM360 Field. SUBCONTRACTOR PRE-MOBILIZATION MEETING The Pre-Mobilization meeting between the Layton Project Team and the senior superintendent will review all required items of the subcontractor prior to mobilization. Included will be: schedule, project requirements such as: Orientation, daily production huddle, daily Pre-Task Plan requirements, safety initiatives, and the requirement of an OSHA 30 for subcontractor’s supervision. All training documentation for aerial lifts, working at heights, ladder, scaffold, and forklift training will be submitted at the pre-mobilization meeting. PROJECT SPECIFIC ORIENTATION The Project Orientation communicate to each worker Layton’s expectations and safety culture. It allows for training and involvement of the Layton Project Team to explain LaPSZ, L.I.F.E. and other requirements of the worker while on the project site. WEEKLY SUBCONTRACTOR COORDINATION MEETING The weekly subcontractor coordination meeting will be run by the Layton Project Team, each subcontractor will meet and review the 3-week look ahead schedule, bringing up concerns, and coordinating safety and work activities between trades. The communication in this meeting should bring unity among all subcontractors on site, and create a more efficient and cost effective project. DAILY ALL HANDS COORDINATION, PRODUCTION, AND SAFETY HUDDLE The All Hands Coordination Production and Safety Huddle is a companywide daily meeting held prior to every shift regardless of the number of workers on site. This huddle occurs in a dedicated location and lasts no more than 15 minutes, it includes a coordinated warm up and stretching routine, followed by coordination of work from each subcontractor. This allows for every worker on site to know all activity and possible hazards. “Safety Excellence” is measured through the fundamental implementation of these essentials.

PROJECT MANAGEMENT PLANNING (EXECUTION)

develops the plan and is responsible and accountable for implementation and

DAILY PRODUCTION AND SAFETY PRE-TASK PLANNING

Each subcontractor crew will discuss and document the day’s production tasks and

the associated hazards with the tasks and methods to complete each task safety.

Each crew member will sign the Pre-Task plan showing that they understand their

tasks and responsibilities for the day’s production.

HAZARD IDENTIFICATION AND SAFETY AUDITS Daily audits are a proactive way for each Layton Project Team to improve safety

processes. BIM 360 Field is the tool utilized to complete this process. Each Prime

subcontractor will utilize an iPad to complete required QA/QC checklists for their

scope of work, as well as complete (1) one weekly documented safety inspection of

their scope of work. Each Layton Project team will complete a weekly Safety Focus

Walk highlighting issues in both quality and safety and assign tasks to correct

issues as well as a monthly Leading Indicator checklist audit.

PERSONAL PROTECTIVE EQUIPMENT (PPE) USE PPE is the last line of defense when it comes to protecting workers. At Layton we

require at minimum: clear eye protection meeting ANZI-Z87 for interior low light,

and hard hats meeting ANZI-Z89 standards, high visibility clothing and substantial

above the ankle work boot, as well as work gloves appropriate to the task. Strict

enforcement of wearing PPE will be imperative on every Layton Project.

PROJECT APPEARANCE, HOUSEKEEPING “NOTHING HITS THE FLOOR”

“Nothing Hits the Floor” is the Layton standard of project appearance. The plan is

intended to improve productivity, reduce waste and construction debris, improve

housekeeping, and increase worker safety through increasing on-site efficiencies,

cleanliness, and material organization. All subcontractors on-site will agree to

participate in this program.

SAFE WORK PLATFORMS “LADDERS LAST”

The Layton “Ladders Last” program will save lives. The concept is based on

prevention rather than protection, this is accomplished by identifying other means

of access and or elevated work platforms that better protect workers and support

safe production rather than defaulting to the choice of a ladder.
