1. This statement made by me accurately sets out the evidence that I would be prepared, if

necessary, to give in court as a witness. The statement is true to the best of my knowledge

and belief and I make it knowing that, if it is tendered in evidence, I will be liable to

prosecution if I have wilfully stated in it anything that l know to be false, or do not believe

to be true.

2. I am a P1 Ambulance Paramedic for NSW Ambulance (NSWA). I have held this position

for 3 years. I am currently stationed at Paddington Ambulance Station. I have a Bachelor

of Paramedicine. I hold a current national paramedic registration as conferred by the

Paramedicine Board and the Australian Health Practitioners Regulation Authority

(AHPRA).

3. I have been asked by Detective Chief Inspector, Jason Dickinson APM of NSW Police

Force, to provide a statement regarding my attendance at the Ruby Princess cruise ship

on 19 March 2020.

4. I have reviewed the transcript of the NSW Ambulance audio recordings from 18 March

2020 - 19 March 2020. A copy of the transcript is annexed to this statement.

5. I appear in those recordings at table 19.

6. On 18 March 2020 I was stationed at Paddington Station. I was rostered to work an

overnight shift. I started my shift at 6:45pm and finished at 7:35am.

7. I was rostered on that night with another crew member, Rebecca Orr.

8. At approximately 2.34 am I received a call from a Dispatcher at the Control Centre who

advised that we had been assigned to attend the Overseas Passenger Terminal (OPT) at

Circular Quay to transfer a patient to Royal Prince Alfred Hospital (RPA). We also received

a notification through the Ambulance mobile data terminal (MDT) for the job at around the

time of that call.

9. I now know that patient to be Lesley Bacon.

10. I have reviewed the Incident Detail Report. A copy is annex.d to this statement. I did not

create this report however to the best of my knowledge it is an accurate record of our

attendance at this job.

11 . I have reviewed the Electronic Medical Record (EMR) for this job. A copy is annexed to

this statement. I did create this report and to the best of my knowledge it is an accurate

record of our attendance at this job.

12. When I received the notification, my crew mate, Rebecca Orr, drove us to the OPT in

Circular Quay. At the time all the infonnation I had was that the patient had an upper

respiratory tract infection, left leg pain, was a suspected or confirmed coronavirus patient

and that she required a stretcher.

13. Upon arrival at the OPT the Ambulance Duty Operations Manager, Christopher Townsend,

was in attendance and he advised us that both the patients being medically disembarked

were potentially COVIO-19 positive as they had been swabbed but were awaiting results

and that full personal protective equipment (PPE) should be worn .

14. Prior to entering the ship, we put on the required PPE. This consisted of a gown, glasses.

gloves and a P2 face mask.

15. We then went onto the ship and into the onboard medical centre. I recall that there were

two rooms in the medical centre, with one patient in each, and that there was at least one

nurse, one doctor and the husband of the patient we were transporting. The nurse and

the doctor were female.

16. I recall that the doctor was wearing a mask and that the nurse did not wear a mask until

we began transferring the patient. The patient's husband was not wearing a mask.

17. The nurse initially approached us and said that the doctor would be with us shortly. The

doctor then approached us and told us that the patient was complaining of left leg pain

with a suspected femoral compression, that she was presenting with a respiratory tract

infection, had been swabbed and tested negative for Influenza A but that they could not

rule out COVID-19. She then told us the patient's comorbidities which included,

osteoarthritis, pulmonary embolus, hypercholesterolaemia, Diabetes Type 2 (insulin

dependent), Emphysema and Hypothyroidism. I wrote these details on the back of my

glove. I then confirmed these again with Lesley once we were in the Ambulance.

18. The only other conversation I recall is that the doctor or nurse mentioned that other

passengers had already disembarked from the ship and had made their own way to RPA

to get tested for COVID-19.

19. I then told the patient that we would run her outside and proceeded to transport the patient

from the onboard medical clinic and load her onto the Ambulance.

20. Once in the Ambulance , I sat in the back with the patient, Whilst my crewmate,

Rebecca Orr, drove to RPA. I entered the case description into the electronic medical

record (EMR), including the details I had written on the back of my glove.

21 . Whilst in the back of the Ambulance I confirmed with the patient the details I had entered

in the case description in the EMR. I then engaged in some casual conversation with her.

The patient told me that she and her husband were from Wollongong and that they had

already completed the cruise three times and that they had had a great time. She stated

that she was now particularly concerned about her husband and she

had arranged for someone to pick him up from the ship and that she worried about the

pain in her leg. The patient also told me that she did not think that she had Coronavirus

and I agreed, saying I thought it was more a case of her leg pain and shortness of breath

was an exacerbation of her emphysema and the upper respiratory tract infection. I

apologised for having to be in mask.

22. We then arrived at RPA at approximately 3.57 am. My crewmate, Rebecca Orr. went into

RPA and spoke to the staff in triage, who advised to take the patient directly to the

designated COVID area. We then unloaded the patient, bypassed the hospital triage and

took her to the designated COVID area at RPA as instructed. I then provided a handover

to the medical officer as per the information I had recorded in the eMR.

23. Upon departure from RPA we cleaned and decontaminated the Ambulance prior to

returning to active duty.

24. The following day we reported the incident to the Ambulance Duty Operational Manager

so that the job could be added to the Ambulance COVID-19 job register.

Peter Dilonardo It's just got - let me have a look - we got a call from Carnival Cruises. Boat is docking at 2.30. We can board at 3 o'clock. We're to go to the gatehouse and be escorted by security.

Peter Dilonardo You see, we've got at Circular Quay West - that's the normal one where the ships all go to dock.

Peter Dilonardo There's no ship name. But there's two patients coming off that one. And then we've got another ship coming into White Bay Cruise Terminal, the Sun Princess.

Peter Dilonardo Yeah. They've got respiratory issues which is one of the signs of Corona. I mean, it's obviously not confirmed and they need further testing. But I know there's been restrictions on ships being able to dock.

We do ask them, we do have a bio-security, I guess, email that we send them - get them to confirm, you know, last ports of call; has anyone been in contact with a known case of Corona, Convid-19, Covid-19. Now, but I just need to be - we just need to be more specific with the ship

We've contacted RPA Hospital, they're unaware of any patients coming in. So we've tried ringing back the contact person and details that were left for us. But we couldn't get through. We left a voicemail message for them to give us a call back. I know yesterday, the day before, we had two patients off the Voyager of the Seas which wasn't allowed to port. So I thought it was -

Cameron They didn't have Corona though, did they? Butchart

Peter Dilonardo No, no - well, I believe they've got Corona patients on board, but the two patients we took off were not Corona, or suspected Corona, they were medical issues.

Cameron Right. Because we've actually got to put a Pilot on board this ship, the Pilot Butchart actually brings the ship in. So, the there are issues for our staff. Are you able

Peter Dilonardo Generally if it is staff it will stay in the notes that it is a staff member, so I'm assuming it's two passengers.

Cameron So what information are you seeking from us? Butchart

Peter Dilonardo Look, we just wanted to know if the ship was actually coming into port, and if it is they were allowed to disembark. Because I know that there's restrictions going on everywhere with people, especially cruise ships, being allowed to come into port and offload patients. Because the two we had yesterday, the day before had medical issues, they weren't Corona, but we went out with the Water Police and a pilot, I guess, and they were brought back to land. The ship wasn't allowed to come in.

Cameron Oh OK. So that was a different story. You mentioned the Voyager of the Seas. Butchart That was a heart attack, wasn't it? A heart attack or a stroke?

Peter Dilonardo Yeah. I think one was a heart attack and one was a stroke.

Cameron Yeah. OK. So that was different. The vessel didn't actually have a booking to Butchart come into the harbour.

Peter Dilonardo Ah. OK. I thought they just weren't allowed - obviously, that rumour got around and the stories were mixed up.

Cameron That's a medical evacuation . Butchart

Peter Dilonardo Ah. OK.

Cameron We brought them in to get them off as quick as we can then get back to sea Butchart again. We've actually got a booking this evening. They have booked the

Overseas Passenger Terminal and they will be docking alongside the OP Terminal.

Peter Dilonardo Ah. OK. So that's - they are coming in?

Peter Dilonardo Oh. OK. Alright. No. It's because I spoke to Steve, one of the other blokes, and he couldn't find any, any um -

Cameron Well, if it is the Carnival ship, but as I said, I haven't heard anything about it so Butchart I might make some contacts with through Carnival, and find out why we

The reason I'm making further enquiries is because I called RPA Hospital and they're unaware of any patients coming in, and I know with all the Corona Virus things going on, they've refused entry to a couple of ships, which is why I've made further enquiries, especially as the hospital doesn't know about these patients coming in. Sometimes they don't always contact the hospital, but I know with all the whole Corona Virus going on, they don't like surprises that just rock up, especially when they've been assessed by medical doctors and they're supposed to go through the Ministry of Health. There's a certain protocol and procedure they're supposed to follow before we get involved and I don't even know if that's being followed, or bypassed, or - but the referring doctor, who I believe is on the ship, is a Dr Von

sc1111 I'm doing a bit of running around because the Harbour Master can't get on to NSW Health. Is there a line or, that you have, that we can -

Peter Dilonardo Look, there's a Corona Hotline number, but I don't know if they're 24 hours. I know RPA have got a Corona clinic, but that's only like 9 o'clock they close, and I just rang and spoke to the Nurse Unit Manager and she wasn't aware there was any patients coming in, and we're going there though, but what sometimes happens with the cruise liners is the doctors on board will book it with us to take them to the various EDs. It's pretty common knowledge that RPA is one of the Corona clinic hospitals. They don't always ring the hospital and say 'hey, we've got a patient coming in, will you accept them?' type thing, they just ring and book it with us and we just turn up. So, it's now always, like I said, it's common for them not to ring the hospital to let 'em know what patients are coming in, and we just turn up, which is probably what the case is for now.

sc1111 Yep. Alright. Well, I've, they just need some direction. The Harbour Master needs some sort of direction about, from Health, because they're running a point about this obviously.

Peter Dilonardo Yeah, well they knew of the ship coming in.

sc1111 Who? Did Health get or -

Peter Dilonardo No, no, the Port - the second bloke, Cameron, that rang me back. He said they had a booking on it but they were unaware of any suspected Corona patients. So, I think they were a bit freaking out of that. Just let me - I've got a - I've just gotta find that email.

The message they were given was that these two patients were tested for Corona Virus, but the results haven't come back which is why they've gone down - it's automatic notation that if they've been tested for it, they're suspected until they're cleared.

Peter Dilonardo Yeah. I think that - the testing they do takes 72 hours before they come back.

Patient has been presenting with some upper respiratory tract infectious symptoms. And slightly elevated inaudible at point zero six. Obs as follows temp 36.9 blood pressure 137/73 heart rate 97 stats 96 pulse rate 16 GCS 15. We've got an ETA of approximately 15 minutes.

She is presenting with respiratory symptoms of upper respiratory tract infection with a productive cough. She's also got left-sided nerve pain, possible femoral nerve compressions as per doctor on the ship. Otherwise, she has a blood pressure of 135/82; a heart rate of 98; a temperature of 37.8; a resp.rate of 20, sats of 94% on two litres and a GCS of 15.
