Facility Description:
The Sequoyah Middle School campus consists of one main school building located at 3456 Aztec Road Doraville, Georgia. The original campus was constructed in 1965 and additions to the main school building were constructed in 1966 and 1967. In addition to the main school building, the campus contains storage buildings, dugouts, baseball field, football field, track, and tennis courts. This report contains condition and adequacy data collected during the 2010/2011 Facility Condition Assessment (FCA). The detailed condition and deficiency statements are contained in this report for each building and other facilities on the campus.

Facility Description:
The Sequoyah Middle School main building is a one-story building located at 3456 Aztec Road Doraville, Georgia. Originally built in 1965, there have been two additions in 1966 and 1967. Renovations were done in 2009 and 2010, and additional renovations for new science labs, gang restrooms, interior doors, vinyl flooring fittings, fixed furnishings, and exterior windows are scheduled for 2011. This report contains condition and adequacy data collected during the 2010/2011 Facility Condition Assessment (FCA). The detailed condition and deficiency statements are contained in this report.
Energy conservation opportunities for this building include: 1) Replacing the HVAC equipment with more efficient units on the next renewal; 2) Replacing the plumbing fixtures and flush controls with low flow units on the next renewal; and 3) Replacing the building control and automation system with a system that controls both HVAC and lighting.

Suitability - Middle_Jr. High
Sequoyah Middle School is a six through eighth grade facility. The building was originally a high school that has
been converted to a middle school. Recent renovations to the school have included a new heating ventilation and
air-conditioning system as well as ceiling tiles. Some of the spaces were still vacant due to the renovation and
should be occupied during the month of November.
Suitability - Middle_Jr. High->Site-->Traffic
The school is equipped with separate parent drop off and bus lanes. The bus lane is short in length and requires the
buses to parked beside each other. Students then must walk between buses to access their bus.
Suitability - Middle_Jr. High->Site-->Play Courts & Fields
The school has a baseball diamond and tennis courts that are not currently used. The school has a practice facility
that is used for both football and soccer. It is surrounded with an asphalt track. The practice field is in extremely
poor condition and the asphalt track has numerous cracks and holes. These facilities are also used for physical
education.

The school is built around two open courtyards with classrooms on all four sides of each courtyard. The hallways
are long in length and supervision of the building is difficult.

The doors, other than the main entrance, remain locked during the day. None of the entryways are directly
monitored by cameras, but the hallway cameras do provide surveillance once an individual has entered the building.

the units is significant and at times has blown student papers off of the desks.

accommodating up to 33 students which makes them crowded.

facility that accommodates a computer lab as well as two instructional areas.

The gym is a full-sized gymnasium with two full-sized basketball cross courts. On occasion, there are up to 120
students in the gymnasium area for physical education classes. When this occurs, the facility is very crowded. The
locker rooms are comprised of numerous small rooms that are used as either dressing rooms or storage. This
configuration makes the locker room areas extremely difficult to supervise.

The school has a large instrumental music room and choral room. Currently, the school does not have a choral
program. In addition, a general classroom space has been converted to an orchestra room.

configuration is a lecture lab combination with the lab stations around the perimeter of the classroom. The lecture
rooms with a demonstration table are small in size.

The school does not have a conference room associated with the administrative area. Conferences are held either
in the media center or classrooms. The principal's office is small and does not have a second exit.

sit at a desk in the main foyer to monitor individuals entering the school.

Technology Readiness->MDF\IT Equipment Environment
The MDF is located in a closet in the teachers lounge/workroom. It is a small space that is on a separate
air-conditioned system.
Technology Readiness->Electrical Power
The instructional spaces meet this standard, however faculty utilize numerous extension cords and various wiring
configurations to accommodate new technology such as smartboards and LCD projectors. Many of the outlets are
not in the right locations and with out extension cords teachers would not be able to utilize the equipment effectively.
Technology Readiness->Cooling
The school is just completing the installation of a new HVAC system. The system is extremely noisy in the
classrooms.

The school is not equipped with a wireless system.
Technology Readiness->Video Distribution
Approximately 40% of the general classroom spaces utilize smartboards. The other classrooms are equipped with
TV monitors and cable. During the visit many of the classrooms did not have access to cable due to the disruptions
associated with the installation of a new HVAC system.

The school relies on an intercom system for the primary means of communication. Classrooms are equipped with a
phone that does have voicemail capability.
