UnitedHealthcare® Senior Supplement Insurance Plans

UnitedHealthcare® Group Medicare Advantage (PPO) Plan

Your cost for medical plan coverage and services

• Senior Supplement plans pay secondary to original Medicare.

• There are two Nationwide-offered options with different monthly contributions and out-of-pocket costs.

• Plan F – Higher monthly contribution with lower out-of-pocket costs for services.

• Plan K – Lower monthly contribution with higher out-of-pocket costs for services.

• A Medicare Advantage plan is a type of health care plan that contracts with Medicare to provide all of your Part A (hospital) and Part B (doctor) benefits.

• Your monthly contribution will be lowest under this plan, and your out-of-pocket costs for services will be based on the plan's copayments and coinsurance.

Your providers • There is no network of providers. You can go to any provider or hospital for care, as long as they accept original Medicare.

• There is a network of providers, but you have the flexibility to go to providers who are outside the network. You pay the same copayment whether the provider is in the network or not.

• In most cases, the provider will file the claim for you directly with UnitedHealthcare. UnitedHealthcare will then pay the provider directly.

• Note: Non-network providers have the choice to accept the plan or not (except for medical emergencies). Please check with your providers before enrolling in the PPO.

Your medical card at point of service

• You present both your Medicare Health Insurance Card and your UnitedHealthcare Senior Supplement Card when you access care.

• You present only your UnitedHealthcare Group Medicare Advantage PPO card when you access care.

Your medical claims and Explanation of Benefits (EOBs)

• Your provider files claims with Medicare, which forwards any balance to UnitedHealthcare for processing.

• You'll receive an Explanation of Benefits (EOB) from both Medicare and UnitedHealthcare.

• You are billed for your share after the claim has been processed.

• In-network, your provider sends the claim to UnitedHealthcare. You pay your share of the cost.

• Out-of-network, you are responsible for filing the claim with UnitedHealthcare, although many providers will do this for you. You are responsible for paying your share of the cost.

• Out-of-network providers are allowed to "balance bill" the health plan (not you) for excess charges above the Medicare fee assignment. UnitedHealthcare will pay these charges. You are responsible only to pay your copayment.

• You'll receive an Explanation of Benefits (EOB) from UnitedHealthcare.

Your prescription drugs (your contributions for drug coverage are separate from your contributions for medical coverage)

• You should enroll in a Medicare Part D plan for your prescription drug coverage.

• You can choose to enroll in any prescription drug plan. You are not limited to the plans available through Nationwide.

• If you don't enroll in a prescription drug plan when you first become eligible, you may be subject to the Medicare Late Enrollment Penalty (LEP).

• Medicare does not allow an individual Part D plan to be combined with a group Medicare Advantage PPO Plan.

• If you enroll in the UnitedHealthcare Group Medicare Advantage PPO Plan for your medical coverage, your only options for your prescription drug coverage are the two UnitedHealthcare Medicare Rx for Group plans.

• You can choose between a high plan option and a low plan option to help meet your individual prescription drug needs.

Full details on all the plans, including the cost of each plan, were included in the open enrollment kit you received in the mail.

Comparing Nationwide’s plans for Medicare-eligible retirees

Nationwide offers two types of medical plans to its Medicare eligible retirees and dependents who are eligible to enroll in company-sponsored retiree health care coverage:

• Senior Supplement plans — which pay secondary to your original Medicare coverage using a deductible/co-insurance cost structure

• Medicare Advantage plan — which combines coverage for both hospital and doctor visits under a single plan using a co-pay cost structure

Both types of plans generally cover everything that Medicare covers, but there are differences in cost (both in monthly contributions and out-of-pocket expenses when service is received), access to providers, card use at point-of-service, how claims are handled and prescription drug plans options. More details about each plan, including covered services, out-of-pocket costs and monthly contributions are provided in the retiree open enrollment kit.

If you enroll a dependent in one of the Nationwide-sponsored benefit plans (the “Plans”), you’ll be asked to periodically provide documentation that proves your dependent(s) are eligible to participate. The Plans verified include: medical and life.

Dependent verification is considered a best practice, because it helps identify dependents whose status may have changed over time, like a spouse or a step-child.

Dependent verification helps ensure that the Plans comply with legal requirements and helps manage costs for both participants and the company.

A mass dependent re-verification is expected to be conducted during the first quarter of 2019.

Alight, a third-party vendor, coordinates this verification process on behalf of the Plans. If you have an individual enrolled who needs verified, you will receive a letter at your home from the Alight Dependent Verification Center. Please respond promptly when the documentation is requested to avoid termination of coverage.

The Nationwide Foundation gifted $1 million to the American Red Cross in September to help southeastern U.S. residents recover from Florence.

“The ability to respond effectively to a major disaster like the damage caused by Florence is a testament to the experience and preparedness of the American Red Cross,” says Steve Rasmussen, CEO of Nationwide and chairman of the Nationwide Foundation Board of Trustees. “We’ve seen first-hand the Red Cross’ ability and efficiency in providing initial relief and support, and know that our financial gift is the optimal way to help them help others.”

In disaster response situations like Florence where multiple states are affected, the Red

Cross conducts rapid needs assessments and mobilizes emergency supplies like blankets, food, water and blood to prepare communities before the disaster hits, then helps families recover after with shelter and basic needs.

“Support from partners such as the Nationwide Foundation enables the Red Cross to quickly mobilize volunteers, equipment and supplies to provide help and hope to people impacted by disasters and countless other crises,” said Gail McGovern, president and CEO of the American Red Cross. “We deeply appreciate the Nationwide Foundation’s generous donation and longstanding partnership as we work together to support people in need.”

AROUND THE COMPANY

We’re delivering value to our commercial auto members through a new partnership with Nexar, a mobility startup focused on promoting safer driving.

The company’s video recording technology provides vehicle-to-vehicle communication that connects directly to a dashcam app and monitors specific driving behaviors like hard braking, fast accelerations and miles driven. The technology can also show footage of accidents and unsafe driving habits.

During a recent pilot in New York City, Nexar dashcams were installed in the fleet vehicles of Nationwide commercial lines client, HASC Center, which is the kind of client that reflects the heart and caring culture of Nationwide. HASC Center is a non-profit agency that provides services – including transportation services – for children and adults with special needs.

Over the course of 16 weeks, the dashcams monitored 20 vehicles collectively operating over 3,000 hours and more than 20,000 miles of mostly city travel.

The pilot results showed that HASC Center drivers demonstrated significantly safer driving habits between the time the program started and when it ended, showing a meaningful decrease in the key driving behaviors identified as most likely to lead to accidents.

“As Nationwide nears its centennial in 2026, we’re doubling down on our innovation efforts to prepare for the next 100 years,” said Scott Sanchez, Nationwide chief innovation officer. “Investing in Nexar is one example of how Nationwide is helping solve consumers’ protection needs in new and digital ways.”

In addition to being the No. 1 insurer of small commercial business, Nationwide is No. 1 in customer satisfaction with small commercial insurance.

Each year, J.D. Power conducts a survey to measure satisfaction among small commercial insurance customers with 50 or fewer employees. The study ranks overall satisfaction for five factors (in order of importance): interaction; policy offerings; price; billing and payment; and claims.

Nationwide topped the industry with our score of 844 on a 1,000-point scale.

In addition:

• Nationwide is the only carrier among the 11 in the study whose Customer Satisfaction Index ranked among the best for both claims and policy offerings.

• Nationwide ranked first on three of the five rating factors, including policy offerings, price and claims.

Earlier this year, Nationwide also ranked No. 1 in customer satisfaction with Group Retirement Mixed Plan Providers.

Ziva and Zeus, a pair of German Shepherds from Medical Lake, Washington, are the 2018 recipients of the 10th annual Hambone Award®, honoring the most unusual pet insurance claim of the year.

Ziva and Zeus were found inside an abandoned missile silo after they went missing for 21 days.

Ziva and Zeus were treated by the veterinary team at Pet Emergency Clinic and Referral Center in Spokane, Washington, where the dogs received life-saving care. The Pet Emergency Clinic and Referral Center will be granted a $10,000 Nationwide-funded award through the Veterinary Care Foundation (VCF) to help

treat pets in their community whose owners could not otherwise afford veterinary care.

Ziva and Zeus will receive the coveted bronze Hambone Award Trophy, as well as a Nationwide gift bag filled with toys, treats and various pet supplies.
