Office of Developmental Program (ODP)Office of Developmental Program (ODP)
PACC is Now Offering Occupational Therapy Services!Occupational Therapy PACC is now offering Occupational Therapy Services through ODP and the Office of Long Term Living (OLTL) for individuals of all ages. Occupational Therapy encourages rehabilitation through the performance of activities required in daily life. Contact Heather Tortorea at (412) 621-6151 ext. 2012 or [email protected].
1) Job Coaching and Support: Some functions of job support include training of individuals in job assignments, periodic follow-up or ongoing support. Other activities include support to relearn a job, training to assist individuals to use transportation for work, maintenance of appropriate work behavior while on the job.
2) Job Finding and Development: Development of skills; outreach to potential employers; training of job tasks; and other such tasks to assist individual in acquiring a job. Some functions of job finding include interview assistance, employer outreach and orientation, resume preparation, and job skill development.
3) Career Assessment: A service that consists of gathering and reviewing individuals interests, skills, and work/volunteer history. We also conduct assessments to assess individual’s interest and aptitude in a specific job while identifying types of jobs in the community that match interests, strengths, and skills. We also assist with developing a career assessment report with recommendations that address needs, interests, strengths, and characteristics of specific work environments.
4) Supportive Employment Services W9794 H2023 W7235: If an individual is deemed non-competitively employable through OVR, they may be eligible to receive Support Employment Services through waivers. Contact your Support Coordinator.
Home and Community Habilitation Services W7060 1:1 W7059 W7058: Services to help people gain, maintain, and improve skills that allow them to live and participate in their local community. This service may be made available to individuals in their own home or in other residential or community settings not subject to licensing regulations. Participants that utilize the group code can work on friendship building and socializing/outings with a buddy, peer, and friend in a group of 2. PA Connecting Communities can provide services for people living in private homes, or supportive living arrangements, including transportation to and from participant home or another agreed upon location. We provide our participants with an opportunity to live ‘Everyday Lives’ by encouraging social activities, friendships, and community inclusion and is a great alternative to traditional day programs and workshops. Each consumer will have an individualized Community Participation Plan based on their goals, wants and needs. Support individuals based upon assessment, those with behaviors that prevent an individual from living or working without crisis. The provision includes the training of individuals, staff, relatives and caregivers. TRANSITION SERVICES
PA Connecting Communities Transition services can be thought as “bridging the gap” between school programs and the opportunities of adult life, including higher education or training, employment, independent living and community participation. PACC custom designs age
appropriate activities and programs for high school students transitioning beyond high school. We provide learning experiences for individuals who require more preparation to assume their role as an engaged, active member in their community. The program has been designed to
immerse students in the community while focusing on an individual’s needs, taking into account each their strengths, preferences and interests.
Education or training beyond the high school experience is available to all individuals. PA Connecting Communities goal is to teach skills needed in the appropriate environment to
prepare learners for careers. Activities include monthly transition classes that address issues such as filling out forms, budgeting, applying for work and interviewing skills.
PA Connecting Communities is an Office of Vocational Rehabilitation (OVR) vendor. PACC is a state qualified non-profit. We collaborate with agencies, school districts, and other non-profits to
provide transitional services.

What are transition services?Transition services are activities that prepare students with disabilities to move from school to post-school life. The activities are based on the student’s needs, preferences, and interests,
and may include activities in the following areas:
PACC is here to help with the process of transitioning from high school to adulthood. PA Connecting Communities (PACC) offers high school transition students programs to integrate
into the community. PACC provides school contracts with school districts and work based learning experiences (WBLE) and job shadowing through our PETS/OVR contract. We offer two options, as a contracted OVR provider and/ or through our school contracts we conduct
evaluations and assessments to prepare your child for a successful transition into the workforce. These assessments not only evaluate strengths and needs on the jobsite but also help to
identify possible career paths. In addition, we have many programs that will benefit your child in school and out in the community. PACC offers life skills, social skills, recreation, transitional
development, and vocational planning. We provide programs for youth on the autism spectrum. We partner with school districts and provide students daytime hours following guidelines in the
students IEP. PACC offers behavioral and occupational services.

Office of Vocational Rehabilitation (OVR)What is new? PACC has entered a contract with OVR to provide pre-employment transition services for high school students 14 – 21 years of age. This is a new funding source to prepare students for employment and career opportunities. Additional OVR Services for all ages:Vocational Evaluations to assess the individuals’ skills and interests. This can be completed in our Vocational Assessment Center.Job Coaching persons are provided 1:1 staff to assist with job development for obtaining and retaining employment.CBWA (Community Based Work Assessment) allows the individuals to work under supervision with a group of individuals at a community site before being placed on a job.PETS (Pre-Employment Transition Service) – PACC had a PETS agreement to provide the work based learning experience (WBLE).Work Based Learning Experiences (WBLE) – A WBLE is an experience that includes job supports at a temporary, seasonal or summer job for a youth that may or may not be consistent with the youth’s long term employment goal. A WBLE is intended to document student strengths and limitations, teach students soft skills, and provide opportunities to explore careers of interest.Job Shadowing - Experience in an occupation of interest within a community integrated setting. A Job Coach works with employers of interest and arranges for student to observe employees on a jobsite to obtain an overview of the work environment, tasks, and abilities required for the field of interest.”Extended Services beyond the typical 90 day training period.
PA Connecting Communities turns questions into answers. PACC provides OVR Employment Services & Supportive Employment Services through ODP for career devel-opment opportunities. Within the last year PACC has partnered with many local area busi-nesses that are willing to assist you in the path to competitive employment.There are several components to Supportive Employment:
• Job Preparation: Skill development crucial to finding, building and maintaining a career, such as: Interview assistance, resume preparation, employer outreach and orientation, job searches and public transportation training.
• Job Preservation: On the job support to assist with changes in duties or behav-iors to help maintain employment.

Welcome to PACC’s Home and Community Day Activity Program . We are offering a variety of activities during day hours that emphasize community integration, skill development, socialization
and independence while working towards personal goals . PACC’s Home & Community Day Activities are open to all individuals residing in group homes with the Community Participation CP Code, self-
paying individuals, and other agencies . Your individual HAB staff will provide the transportation to any of the events and Access may also be available . You may participate in the full start to finish time of any
activity, but have the option to arrive later or leave early depending on your personal schedules .
All outdoor activities are subject to change due to weather. For any questionsabout activities, events and programs, contact our Director of Events, Programs and Activities,
