Our Mission:
The mission of Family Support & Resource Center is to
make it possible for Dane County children and youth
with disabilities to live at home with their families as
valued and participating members of the community.
Disability affects families from all walks of life. The families we serve vary greatly in their circumstances, in their resources, and in their ability to cope with the extra responsibilities placed on them.
What families do have in common is the need for flexible and individualized assistance that will help keep them connected to their community, hopeful about the future, and supported in managing the extra stresses and expenses. Because disabilities are not typically cured and their impact often increases over time, most families we serve need long-term supports.

FSRC is guided by these principles
All children, including children with disabilities, benefit by
growing up as a secure member of a family and sharing in
positive, long-term relationships with other family members.
Families have great capacity to care for children who have
disabilities when they receive support from friends, neighbors,
human service professionals, and other community members.
Parents are generally in the best position to define their own
needs, to find solutions, and to choose the kinds of services that
will help them to care for their child with a disability.
A sense of trust and confidence in the continuity of support is
important to establish and preserve with each family.
Communities are enriched when all people, including those with
disabilities, are valued and included as full contributing
members.

Consumer Satisfaction Survey
In April we distributed consumer satisfaction surveys
to 100 (20%) of our families based on a random
sample. 52% of the surveys were returned with
overwhelmingly positive responses.
Over 95% of families feel staff are:
respectful and supportive
take the time to listen, advocate, and
problem-solve
As a result of their relationship with FSRC, 100% of
families believe they have experienced significant
positive changes, including:
overall improved quality of life for their
child
lowered stress in their family
feeling better connected and supported
increased confidence to meet their
child’s needs
2016 Agency Highlights:
FSRC works closely with families and community providers whenever a child is being referred for out-of-home placement. If placement is necessary, we strive for options that are non-institutional and/or short-term. At the end of 2016, ten of 12 children experiencing significant crisis remain living with their families with additional supports.

FSRC Celebrates 30 Years!
On August 27, FSRC celebrated 30 years of support to children and their families at our annual picnic at McKee Farms Park in Fitchburg.
Over 265 people enjoyed good food, company, activities and more, in honor of the agency’s many years of fine service.
Since 1986, 86 staff, over 100 members of our Board of Directors, and 1000’s of families and donors have worked together to make FSRC a great place to support children who have disabilities.

Support for
Waiting List Families
Families waiting for services are able to receive
support through information on the FSRC website,
quarterly newsletter, invitation to all agency special
events and mailings, holiday giving, consultation
from staff, and 1 X grants. In 2016, we distributed
28 grants totaling $38,000 for adaptive equipment,
home modifications, respite care, daily living skill
training, recreation and parent education.

Family Support staff work
with families to:
Understand and gain access to available resources
Coordinate services, advocate for the child and facilitate teamwork and problem-solving
Purchase needed goods and services not available through other funding sources
Help families to build and strengthen networks of supports, to encourage friendships for their children, and to plan for the future
Keeping All Children at Home with Their Families
Our Family Support Program provided ongoing services to 542 children in 2016, including 46
children who first entered the program. 54 children were closed from the program during the
year. Reasons for program closures: 25 young adults transitioned to adult services, 13
voluntarily closed, nine individuals did not continue to meet eligibility, three died, two moved
to residential treatment, one moved from Dane County, and one young adult moved to own
apartment. As of December 31, 2016, there are 400 children on the waiting list for CLTS
Waiver.
FSRC Programs Offer Choices, Flexibility & High Impact Assistance
Families in our Family Support Program have access to and choices among
goods and services that are flexible and individually tailored to family needs.
Families receive varying levels of case management time and financial
assistance, according to needs they identify.
Our Family Support Program receives funds through various sources,
including Dane County Department of Human Services, United Way, donors,
and other funders. The majority of our funding is connected to the State
Children’s Community Options Program and the Children’s Long Term
Support (CLTS) Waiver Program. In 2016, our funds were distributed in the
form of grants of varying sizes to families, as shown on the following charts:

Family Subsidies
The chart reflects CLTS Waiver and Children’s COP funds that were paid
directly through the Family Support and Resource Center. During 2016,
funds for respite care, respite camp, and home modifications slightly
increased, while there were decreases in therapy, respite camp, and training.
During the year, changes were made to the CLTS Waiver disallowing
payment for many therapy services. Funds for recreation, day care,
supportive home care, and transportation remained about the same as
previous year.

OVERTURE CENTER FOR THE ARTS
COMMUNITY TICKET PROGRAM
This program helps families who are low income enjoy
performances at Overture for a greatly reduced rate.
BOOK PROJECT
FSRC receives free children’s books from the Barnes and
Noble Holiday Book Drive sponsored through the
Children’s Service Society.
THANK YOU HOLIDAY DONORS!
We are amazed each year at the generosity and
dedication of our holiday donors who give of their
resources and time to help make the holidays brighter
for many of the families we serve! This year, 175
children received individually selected gifts. Our donors
included the members of Unity Church of Madison,
employees of CapSpecialty, Capitol Lakes, Wisconsin
Bankers Association, Alliant Energy, and many
individuals and families.
GREAT PUMPKIN HALLOWEEN PARTY
– A TREAT FOR EVERYONE!
FSRC had fun partnering with the Country Inn and
Suites again this year to provide a free, safe, and
accessible environment to celebrate Halloween and trick
or treating. This year FSRC transformed a hotel room
into a Peter Pan themed experience. The event was
attended by over 200 children and parents.
HUHOT MONGOLIAN GRILL
Partial proceeds from their April 21, 2016 dinner service
was donated to FSRC as part of their Helping Hands
fundraising program.
AT&T PIONEERS
SUPPORT
STUDENTS!

who are involved with the backpack program and other
education-based support that make children's needs a
priority. We have enjoyed participating in this annual
activity since 2004.
STEVE JONES MEMORIAL AWARD
FSRC was the proud recipient of the 2016 Steve Jones
Memorial Award sponsored by the Capital Times Kids
Fund. The award is given each year to recognize an
“outstanding program in Dane County that exemplifies
working with youth.”
