    Why is LOTO important?

    The Lock Out/TagOut procedureprevents theunexpected start upor release of storedenergy that couldcause injury toemployees by placing

    a lock and/or warningtag on an energyisolation device.

    LOTO Program Enforcement

    A violation of a University employee'sresponsibility must be reported to theemployee's immediate supervisor for

    appropriate action.

    A violation of a contractors responsibility

    must be reported to the project manager

    (or the person who hired the contractor)for appropriate action.

    LO devices must be durable & withstand theenvironment to which they are exposed. TO devicesmust be constructed & printed so they do not deteriorateor become illegible;

    LO/TO devices must be standardized by color, shape orsize;

    LO/TO devices must be substantial enoughto preventearly or accidental removal and removal without

    excessive force.

    Notify ALL affected employees that aLOTO procedure is being used and why.

    Shut the equipment down using operatingcontrols.

    Follow proper procedure for the equipmentto avoid endangering anyone during theshutdown.

    LO & TO energy isolating sources with assignedlocks and tags. Usually requires locking outelectrical disconnect.

    Be sure to isolate ALL energy sources(secondary as well as primary).

    directly on the energycontrol.

    When LO is used,every employee in the

    A TO system alone is not acceptable at anytime unless the equipment is not capable of

    being locked out.

    The tag must be placed at the same locationthat the lockout device would have beenattached, and additional measures must be

    taken to ensure the level of safety isequivalent to that obtained by using a lock.

    This includes: blocking of a controlling switch,removal of an isolating circuit element,

    opening of an extra disconnect device, orremoval of a valve handle ONLY.

    Inspect the system tomake sure all parts havestopped moving.

    Disperse stored energy

    (springs, hydraulicsystems, air, gas, waterpressure, or steam).

    If stored energy can build

    up again, monitor it tomake certain it staysbelow hazardous levels.

    Make certain alldangerous areas areclear of personnel.

    Verify that the equipment

    cannot be turned to theon position. Press allstart buttons andactivating controls on theequipment.

    Return all controls to theoff position after certainthat LO is effective.

    BGSUs LOTO ProgramRemoving LOTO Devices/Restoring

    Equipment Safeguard all employees by making

    certain everyone is clear of the equipment.

    Also notify affected employees ofimpending re-energization.

    Verify that all tools have been removedand all guards reinstalled.

    Remove LOTO devices. Each device mustbe removed by the person who put it on.

    Energize equipment.

    Remove all tools & personnel from thearea.

    Notify affected employees of impending

    re-energization. Authorized employees remove the LO

    device but MUST leave tag in place.

    Energize equipment. When finished, de-energize & reapply LO

    device.

    BGSUs LOTO ProgramShift Changes

    If servicing lasts more than one shift, LOTOprotection CANNOT be interrupted.

    The original authorized employee will removetheir lock & the incoming employee will placetheirs on the isolating device. If the firstemployee leaves prior to the replacementsarrival, a supervisor may place their lock on toensure safety.

    The incoming authorized employee MUSTVERIFY that ALL energy sources are locked outprior to beginning work.

    BGSUs LOTO ProgramAbandoned Lock Procedure

    Verify authorized employee has left the worksite& notify them that their lock is being removed.

    LOTO devices may ONLY be removed if

    supervisor is present & authorizes removal. After all communication attempts are complete &

    the equipment has been inspected, energy canbe restored.

    An abandoned lock procedure form must befilled out & filed in the Lock Shop.

    Work on cord & plugequipment IF it isunplugged, the plugremains within arms reachduring servicing and

    authorized employee hasexclusive control of theplug.

    If guards arent removed orbypassed & are effective in

    preventing worker exposureto hazards created by theunexpected energization orstart up of machines orequipment, or the release of

    energy.

    Equipment Specific LOTO forms must becompleted by the supervisor and/or theauthorized employee.

    All equipment specific procedures must beeasily accessible or posted directly on theequipment.

    Change in authorized employees job assignmentsor job area that contains sources of hazardousenergy;

    Change in machines, equipment or process that

    presents a new hazard; Change in energy control procedures;

    Need for refresher training found during periodicinspections; and/or

    Employer believes there are deviations from, orinadequacies in, the employees knowledge or useof LOTO procedures.

    An inspection will be conducted at leastannually by an authorized employee notinvolved in the procedure being inspected.

    The inspection will identify any deficiencies inor deviations from the program.

    The supervisor must certify that the

    inspections have been performed.

    Where LO is used, the inspector must revieweach authorized employee's responsibilitiesunder the procedure with that employee (group

    meetings are acceptable). Where TO is used, the inspector must review

    BOTH the authorized and affected employee'sresponsibilities with those employees for the

    energy control procedure being inspected.
