Learning Outcome:
This program will discuss a number
of topics related to diabetes care.
As diabetes educators, it is our re-
sponsibility to “make noise” for dia-
betes related care. All of our dy-
namic speakers have made noise in
his/her respective area and will em-
power other diabetes educators to
do the same. Target Audience:
This is a knowledge-based activity designed for
individuals’ active in diabetes education, including
RNs, RDNs, Pharmacists, Nurse Practitioners, Clini-
cal Nurse Specialists, Physician Assistants, MDs,
and other health care providers interested in stay-
ing up to date on current practices of care for pa-
tients with diabetes and other related conditions.

Learning Objectives:
At the end of these sessions the learner should be able to: 1) Describe appropriate lifestyle interventions for obesity.
2) Discuss how Intensive Behavioral Therapy can be implemented in a health care
clinic.
3) Discuss evidence-based medicinal approaches for weight loss in pre-diabetes and
diabetes.
4) Select and recommend appropriate medication therapy for obesity.
5) Describe the process of obtaining program sustainability through managed
care and health insurance reimbursement.
6) Explain which evidence-based model for are being reimbursed by Medicare and
managed care organizations.
7) Define the Role of the members of a multi-disciplinary team in delivering evidence-
based programs in a manner to obtain health insurance reimbursement.
8) Explain why accountable care organizations want to expand access to evidence-
based education programs.
9) Discuss the Evaluation Measures used to measure program effectiveness and the
requirement for a continuous quality improvement process.
10) Explain the Centers for Medicare & Medicaid Services (CMS) requirements for
diabetes self-management training reimbursement.
11) Explain the Centers for Medicare & Medicaid Services (CMS) requirements for
Chronic Care Management (CCM) Services.
12) Describe recent updates in the pharmacotherapy management
recommendations for type 2 diabetes.
13) Discuss the rationale regarding therapeutic decision making when adding to, or
replacing metformin in type 2 diabetes.
14) Assess appropriate use of SGLT-2 inhibitors, DPP-4 inhibitors, GLP-1 agonists,
and newer concentrated insulins based on specific patient characteristics.
15) Describe the sequence of steps undertaken to develop the 2018–2020 Illinois
Diabetes Action Plan.
16) Distinguish key characteristics that differentiate prior state diabetes plans
from the 2018 – 2020 state plan.
17) List three priority areas outlined in the 2018–2020 Illinois Diabetes Action Plan.
18) Choose 1 – 2 specific objectives or strategies that could be applied within
your organization directly or indirectly (through projects with existing
partners).
19) Evaluate key benefits of using data management tools to assist organizations in
the delivery of diabetes or chronic disease prevention programs to patients
with diabetes.
Requirements for successful completion: Successful completion of this course includes attending the course from 8:00am until completion at 4:30pm. Evaluations and Certificates of continuing education will be online with password emailed to all participants who are present the full day & complete the evaluation using the online link that will be provided at the end of the symposium.
