SUMMARY5F Apparels Limited was surveyed for fire safety on 2/6/2015 by Jensen Hughes. Thissummary identifies the major deficiencies that were identified during the inspection.

The purpose of this survey was to identify significant fire safety issues and to providerecommendations for remediation based on applicable standards specified by theAccord. The scope of this initial fire safety inspection was limited to the review andidentification of major fire safety issues. The inspection did not include identification ofminor deficiencies, which will be further addressed as part of follow-up inspections.

Table 1 summarizes the major fire safety issues identified during the inspection.Recommendations have been provided to address each issue.

An implementation schedule shall be developed by the factory to remediate each of thefindings. The specific timing of improvements, including any requested extensions dueto design / installation constraints, shall be submitted to the Accord for approval.

The exit stairs are notseparated from workareas and other spaceson each floor by fire-rated construction.

Provide minimum 1-hr rateddoors for buildings with 3 orfewer stories and 1.5-hr firerated doors for buildings thatare 4 or more stories. Seal allunprotected openings toseparate the exit stairs fromwork areas and other buildingspaces on all floor levels.Ensure that the fire doors areself-closing and positivelatching and that they areprovided with fire exit (panic)hardware where servingproduction floors.

If fire doors are required to beheld open for functionalreasons, provide automatic-closing devices tied to the firealarm system.

Once fire doors are installed,every door in a stairenclosure serving more than5 stories shall be providedwith re-entry unless it meetsACCORD standard 6.8.3.1.
