Aim

• To increase the percentage of pts of color who complete colorectal cancer screening by at least 10 % in 2011.

• Improve our percentage at least from 45.7 % to 55.7 %

Measurement

• We are quantifying the percentages of B.C pts of color who have completed the CRC screening.

Implementation

• Second fit testing clarification. • Pt education (showing laminating

education piece to pt while waiting for providers).

• FIT screening education to nurses and providers.

• Sage program referral for CRC screening.• Incentives.

• These are the percentages of pts of color who completed CRC screening by quarter.

• 4th quarter 2010 : 45.7 %.• 1st quarter 2011: 58.6 %.• 2nd quarter 2011: 57.2%.• Reached initial goal for pt of color to increase 10

%, but our trend for last 5 months has been a plateau.

• Sharing of our Eban experience and results with other care team members.

• Stressed importance of identifying target population by pre visit planning and encouraging CRC screening.

• Review results with all care teams on a quarterly basis
