Introduction

The earthquake and tsunami at the Fukushima Nuclear Plant (March 2011) resulted in significant damage to the facility.

Introduction

Earthquake epicenter was off the coast. • The earthquake resulted in a large tsunami that is estimated to have

exceeded 45 feet. • Units 1, 2, and 3 were in

operation • 5, and 6, were shut down for

routine refueling • Unit 4 reactor fuel had been

offloaded into the Unit 4 spent fuel pool

The Accident

The plant staff were challenged with: • Loss of all A/C power. • Control and instrument power was limited and soon failed. • No means for cooling the Spent Fuel Pool (SFP). • Lost long term core cooling capability.

Following the earthquake: • The three operating units automatically shut down

• Offsite power was lost to the entire facility • Emergency diesel generators started at all six units

The facility response to the earthquake was normal.

The Accident

Approximately 40 minutes following the earthquake and shutdown of the operating units, the first large tsunami wave inundated the site. The tsunami caused extensive damage to site facilities and resulted in a complete loss of all ac electrical power on Units 1 through 5.

The Accident

Unit 6 retained the function of one air-cooled emergency diesel generator and shared this power with unit 5. Operators lost the ability to cool the fuel in the Units1, 2 and 3. Damage to the nuclear fuel occurred shortly after the loss of cooling capabilities on these units.

Near-Term Task Force March 23, 2011 the NRC Chairman directed the staff to establish the Near-Term Task Force (NTTF) The NTTF recommended revising the NRC’s Station Blackout (SBO) rule (Title 10 of the Code of Federal Regulations (10 CFR) section 50.63). Advanced Notice of Proposed Rulemaking has been issued in Federal Register. August 2011 INPO issued IER 11-4 in response to NRC NTTF SBO recommendations. March 2012 NRC issued Order EA-12-049 to Modify Licenses with Regard to Requirements for Mitigation Strategies for Beyond-Design-Basis Events. March 2012 NRC issued Order EA-12-051, to Modify Licenses with Regard to Reliable Spent Fuel Pool Instrumentation.

Industry Response August 2012 Nuclear Energy Institute (NEI) issued Rev 0 of NEI 12-06 “Diverse and Flexible Coping Strategies (FLEX) Implementation Guide”. NEI 12-06 was endorsed by NRC in August 2012 by JLD-ISG-2012-01 “Compliance with Order EA-12-049, Order modifying Licenses with Regard to Requirements for Mitigation Strategies for Beyond-Design-Basis External Events”. August 2012 NEI issued Rev 1 of NEI 12-02 “Industry Guidance for Compliance with NRC Order EA-12-051”. NEI 12-02 was endorsed by NRC in August 2012 by JLD-ISG-2012-03 “Compliance with Order EA-12-051, Reliable Spent Fuel Pool Instrumentation”.

SBO Rule 1. Establish a minimum coping time of 8 hours for a loss of all

AC Power.

2. Establish the equipment, procedures, and training necessary to implement an ‘‘extended loss of all AC Power (ESBO)’’ coping time of 72 hours for certain Critical Safety Functions:

A. Core cooling B. Spent Fuel Pool (SFP) cooling C. Reactor Coolant System (RCS) and primary containment integrity.

3. Preplan and pre-stage offsite resources to Support the Critical Safety Functions

Include the ability to deliver the equipment to the site in the time period allowed for extended coping, under conditions involving significant degradation of offsite transportation infrastructure associated with significant natural disasters.

Boundary Conditions • Beyond-design-basis external event occurs impacting all units on site. • All reactors on-site initially operating at power, unless site has procedural

direction to shut down due to the impending event. • Each reactor is successfully shut down when required (i.e., all rods inserted, no

ATWS). • On-site staff are at site administrative minimum shift staffing levels. • No independent, concurrent events, e.g., no active security threat. • All personnel on-site are available to support site response. • Spent fuel in dry storage is outside the scope of FLEX.

…the FLEX strategies are also “diverse and flexible” such that they can be implemented in many different conditions as it is not possible to predict the exact site conditions following a beyond-design-basis external event. As such, the strategies can be implemented in all modes by maintaining the portable FLEX equipment available to be deployed during all modes.

Flex - Mechanical The Triton 5000 Pumping System is a single, integrated pumping module capable of pumping 5,000 gpm at 350 ft total dynamic head(150 psig).

It uses a satellite pumping system consisting of two floating submersible source pumps that supply water to a Dominator pump at distances up to 150 ft.

The floating submersible pumps can provide up to 50 ft of vertical lift.
