This “Test Witness Report” form is intended to be used as template to assist applicants in applying for DOA and therefore demonstrating the required design capability and provide a basis for the DOA applicant to produce the own Template for a Classification for Design Changes.
The template form and text already entered are examples and shall be checked if they are appropriate and be changed accordingly by the organisation. All text boxes should be deleted after the information is entered to produce the final DOA form.
The required information can be presented entirely in this document, or in additional documents appropriately identified and referred to.
