MARCH 2016 From Our Medical Director In March of 2006 Pastor Mary and I were still getting to know each other and firming up our vision for a free clinic to serve people who had common health conditions like diabetes and high blood pressure but nowhere to go for regular med- ical care. Still to come: applying for non-profit status, raising our first dollar, hiring our first employee, con- verting the church annex from a bakery into a space where we could see patients, securing volunteers, obtaining medical supplies and equipment, developing partnerships with local health systems, settling on a name for our organization, building a website, devel- oping promotional materials, and – most importantly – building the trust of the community. It was overwhelm- ing at times. I often tossed and turned during the night, worrying about how it all was going to come together. But then Pastor Mary would sit me down next to her in the church and tell me, “You don’t have to see all the way to the end. Just focus on the next thing and we’ll get there.” She was right. We did get there. And we’ve come so far! This year has been our busiest ever. We treated more patients inside our walls and reached more people through community outreach. We added a fourth bar- bershop to our successful Cuts and Checks barbershop blood pressure program. Early this year we brought on a third part-time medical provider, long-time volunteer Dr. Michele Babaie. Last month, after over a year of planning, we made the transition from paper charts to Epic, an electronic health record used by all the major health providers in our area. Read more inside about this big “LEAP” forward. We also spent much of the past year looking for a larger space to accommodate our growing staff and expand- ing patient population. You’ll find details inside about our great news related to a new home. Through all of the growth and changes, our top prior- ity has been to provide the highest quality care to our patients. Many have had negative experiences in the medical world, and it takes a while to rebuild their trust. This means we shower our patients with affection as well as gentle reminders about keeping their appoint- ments, taking their medicine, getting regular exer- cise, staying off the soft drinks, and quitting smoking. One patient was overheard in the waiting room telling his friend, “I used to be sick before I started coming to North by Northeast. Now I’m not sick anymore.” Although he still has the same serious health issues as before, he feels loved and supported. He feels well. We’re celebrating our tenth anniversary all year long and hope you will join us. Maybe you’d like to join our Chronic Givers Club with a gift of $10/month, or mark our anniversary with an especially generous one-time donation. We’ll be throwing a party next fall and you’re invited. Stay tuned for the details! RelocationWe’re moving...and we’re thrilled to share our great news! While our 2,000 square foot space at Cascadia’s Garlington Center has served us well since 2009, we now need something bigger for our expanding patient population and staff. Over the past year we pounded the pavement looking at possible new homes for the clinic. We also talked a lot with patients and commu-nity members about what they wanted in a new space. What we heard over and over again was, “Please stay in the neighborhood! This is where you are most needed.” We’re delighted to announce that we will be staying. Later this year (August/September) we’ll be moving into a newly renovated building at 714 NE Alberta Street. The 3,200 square foot building has more space for patient care, room for trainings and gatherings, plenty of office space for our staff, and off-street park-ing. A $300,000 Community Livability Grant from the Portland Development Commission will allow us to make all the necessary improvements to turn this build-ing into a first-class health center. Stay tuned for more details about the move and how we’ll be celebrating our new space later this year!

An Epic LEAPOn February 8, we reached a milestone! We made the change from paper charts to Epic, an electronic health record (EHR). EHRs make all of a patient’s health information available in a single database. This helps our staff members easily share information between each other about our patients’ care as well as see infor-mation about the care our patients receive outside North by Northeast. We can now order labs and medications electronically so our patients don’t have to come to our office to pick up paperwork, saving them time and phone calls. And tasks we used to do manually – like checking which of our patients had a high blood pres-sure reading at their last visit – are now automated, so our Community Health Worker can easily see which patients need her attention. This huge step was made possible by Legacy Health and their LEAP program (Legacy Epic for Affiliated Providers). LEAP helps with the cost of implementation and provides training and support for all our staff. Thank you, Legacy Health and all the LEAP program staff! We also appreciate our patients’ patience as we worked out the bumps during this transition. Everyone seen during the first two weeks was given a tee shirt as a small thank-you from us.

Thank you all!We want to say a huge “thank you” to all of you – donors, supporters, volunteers, board members, patients and friends. Thanks to you we ended 2015 with a record amount in donations and beat our year-end fundraising goal of $75,000 by 40%. We raised over $100,000 in December and couldn’t have done it without you. Your gifts mean we head into 2016 – a year with lots in store – knowing you have our back.

In addition to generous year-end gifts from individu-als and families, we received several grants toward the end of 2015 that are enabling us to expand capacity and launch new programs in 2016.

An award of $45,000 from the Collins Foundation is already helping us transition from paper charts to an Electronic Health Record (EHR) and will enable us to make sure we’re using the EHR to provide even better care to our patients.

Kaiser Permanente and Providence Health & Services together awarded North by Northeast a $40,000 grant over the next two years. The grant will also help with the transition to electronic charting and billing, and will strengthen our financial reporting and fundraising activities.

We’re seeing more patients every month, and thanks to a $30,000 grant from PacificSource Foundation for Health Improvement we have hired another part-time provider and will continue to provide high quality ser-vices to all our new and established patients.

Finally, we kicked off 2016 with a grant of $100,000 from CareOregon that will help us engage our current patients and make sure community members know about the services we provide through outreach, tabling and events.
