Process Hazard AssessmentThe FSES certified independent expert facilitators will perform the necessary preparation activities then chair and deliver the appropriate reporting for the qualitative PHA activities including HAZID,

HAZAN, HAZOP, CHAZOP and FMEA.

Safety Requirements Specifications

FSES compile conceptual and detailed Safety Requirements Specifications to meet

the requirements of IEC 61511.

SIL VerificationWe also verify the capability of Safety Instrumented Functions

in accordance with IEC 61508 and IEC 61511 by quantifying the effects of random hardware failures such as PFD or Frequency of Dangerous Failures (PFH), calculating the

hardware safety integrity architectural constraints including Safe Failure Fraction (SFF), Hardware Fault Tolerance (HFT) and Common Cause Failure (CCF). Once quantified FSES

will verify the target SIL by using various modelling techniques including Reliability Block Diagram (RBD), FTA and Failure Mode

and Effect Analysis (FMEA).

Operations and Maintenance

We conduct review studies and provide consultancy services to ensure the functional

safety requirements are maintained during operations and maintenance, including proof testing, management of overrides, recording

failures and demands as well as change management. In addition, our expert team of consultants provide Reliability, Availability and

Maintenance studies (RAMs).

Functional Safety Assessments

Independent functional safety assessments are conducted by FSES to advise if functional safety and

safety integrity have been achieved by the Safety Instrumented System in accordance with IEC 61508.

Safety Integrity Level (SIL) Determination

We provide SIL determination studies, where the tolerable risk criteria is set and the necessary risk reduction is assigned. Then

safety integrity requirements for the SIFs can be allocated in terms of the Probability of Failure on Demand (PFD). FSES utilise

different methods to achieve SIL targets qualitatively by Risk Graphs, semi-quantitatively by Layers of Protection Analysis

(LOPA) or fully quantitatively by Fault Tree Analysis (FTA).

Safety Instrumented System (SIS)

Conceptual DesignSelecting the correct equipment and

architecture are key to achieving functional safety and reliability targets. FSES carry

out independent SIS conceptual designs, selecting the right equipment based on

integrity and functional requirements.

Functional Safety Lifecycle/ManagementThis relates to how functional safety requirements and procedures are

implemented during a project. FSES provide assistance or create and maintain in its

entirety the Functional Safety Lifecycle and Management requirements for clients in

accordance with IEC 61511.

Quantitative Risk Assessment

FSES have delivered numerous QRA projects using the FSES QRA Life Cycle
