6. That 1948, PAULO MUWANGA and I, married and I took his name, to become known as KASALINA ZAWEDDE MUWANGA.

7. That since my marriage, I have been known as KASALINA (or its anglicised form: CATHERINE) ZAWEDDE MUWANGA. That is the order in which my names are generally stated in official documents and the order in which I sign my name.

8. That without my knowledge or authority, the order of names was changed, inverted on my passport to read: KASALINA MUWANGA ZAWEDDE instead of KASALINA ZAWEDDE MUWANGA (Muwanga; surname).

9. My Ugandan Passport No. B0617497 (issued 20/06/2006, expiry 20.06.2017) wrongly STATES MUWANGA as middle name and, ZAWEDDE, as last name.

10. That my passport is my main form of identification and it is important it aligns with the names appearing on other key documents dealing with my legal, financial, medical and personal affairs.

11. That I make this declaration in verification of my names and in support of application for a new passport under my correct names of “KASALINA ZAWEDDE MUWANGA”.

12. That whatever is stated herein is true to the best of my knowledge and I make this solemn declaration conscientiously believing it to be true, in accordance with the Statutory Declarations Act, Cap. 22, Laws of Uganda.
