History of The HarborIn July of 1998, Alexian Brothers The Harbor opened its doors to its first residents: individuals who were homeless and had HIV or AIDS, many of whom werealso overcoming drug or alcohol addiction. The Harbor has since been recognized with awards by the Taskforce on Waukegan Neighborhoods (TOWN) as well as Waukegan Township for its outstanding commitment and dedication to the homeless population in Lake County. Since opening its doors in 1998, The Harbor has served over 150 homeless men and women with HIV, the majority of whom have graduated from the Transitional Living Program and been a positive influence in their communities. The Harbor provides a structured and supportive living environment with 24/7 staffing, eight single furnished private rooms, a family room, physical fitness equipment, a learning center, and more. The Harbor and Bonaventure House, in Chicago, are the only state-licensed recovery homes for men and women living with HIV/AIDS in Illinois.

Assessing the NeedOur residents face multiple barriers to health care and stable housing. These include homelessness, poverty, substance use and diagnosed mental illnesses.» In Lake County, it is estimated that there are

approximately 260 people living with HIV/AIDS who are homeless.

» 96% of our clients have incomes below the Federal poverty line.

» 88% have a history of a substance use.

» 66% have a diagnosed mental health condition.

Addressing the NeedThe Harbor embodies the Alexian Brothers values of compassion, dignity of the person, care of the poor, holism, and partnership. We strive to address the needs of our residents and community.

AFTER CARE After Care provides continuing support to graduates of the Transitional Living Program to achieve stable, independent living.

The Harbor has four major goals for our transitional living program:» Increase access to safe, decent, sanitary, and

affordable housing for those who are living with HIV and are homeless.

» Increase health outcomes of those living with HIV/AIDS and are homeless by providing case management, care coordination, occupational therapy, recovery support, and health education.

» Break the cycle of homelessness and poverty by increasing life skills, accessing food, and fostering independence through employment, volunteerism or education.

» Provide individualized recovery support and counseling through licensed CADC’s that help residents maintain their self-identified goal of a drug and alcohol free lifestyle.

Requirements for ResidencyApplicants for The Harbor’s Transitional Living Program must:

1. Have a diagnosis of HIV or AIDS;2. Be homeless or at-risk of homelessness;3. Be at least 18 years old; and4. If in recovery, have 90 days of substantiated sober

time, and be committed to remaining sober.

The Harbor fully adheres to the federal fair housing law

and does not discriminate on the basis of race, creed, religion, sex, national origin, age, familial status, marital status, ancestry,

military disclosure or disability.

» The Harbor provides housing and supportive services for people living with HIV to work on stabilizing their health and recovery.

» Providing housing is providing healthcare. Homeless people are three to six more times likely than housed people to become ill due to factors such as poor hygiene, malnutrition, and exposure to cold and rainy weather.

» The Harbor provides opportunities to seek continued education and employment, providing a strong foundation for independence and long-term stability.

» By providing After Care services, The Harbor ensures that graduates from the Transitional Living Program have a successful integration into the community.

Programs and ServicesThrough our Transitional Living Program, The Harbor offers our residents a wide array of supportive services, including: CASE MANAGEMENT Each resident develops a comprehensive individualized service plan with an on-site Case Manager/Recovery Coordinator (CM/RC). Residents meet with their case managers each week to monitor and evaluate progress. Case management and recovery services are vital in preparing residents to become self-sufficient and return to independent living in the community. RECOVERY The Harbor is a licensed recovery home through the state of Illinois. Its residents maintain a safe, sobriety-based living facility with the help of one-on-one and group services, peer support and recovery support provided by licensed CADC’s. OCCUPATIONAL THERAPY Group and individual sessions help residents to identify strategies needed to overcome barriers and work towards healthy, independent lives. SPIRITUAL CARE Our program is holistic, which includes promoting spiritual well-being alongside physical, mental, and emotional well-being. All spiritual services are optional and ecumenical.
