AMENDMENT TO PREVIOUSLY SUBMITTED INFORMATION

In keeping with the requirements set forth under the US Nuclear Regulatory Commission, 10 C.F.R., the attached reportis submitted for the above subject period for material transferred under our general license.

The general license noted above pertains to an Electron Capture Detector (ECD) which is utilized in a PerkinElmer GasChromatograph. The radioactive material Ni63, has a maximum activity of (15) millicuries.

We are resubmitting the report for Minnesota as we inadvertently did not include a transfer (totaling one ECD

device) on our previous 1It QTR 2015 submission. It was thought to be transferred to a user in a different state.
