Dear Friends;

Diabetes control has come a long way. a variety of pills, insulins, and pumps have all overwhelmed the market trying to improve both glycemic control and lifestyle. But have our numbers become better? the answer to this question doesn’t lie in the availability of better products. Control solely depends on the individual person. It is often easy to blame old habits, inconvenience or cravings, for neglect-ing to carb count and adhere to a proper diet. However, that’s something an endocrinologist can never do for you.

the key to good health lies within each of us. one must evaluate the eating habits that have so long been ingrained in us by society, family and culture. We must carb count as well as un-derstand the difference between cravings and true hunger, noting that when we are truly hungry we are not desiring only particular foods, but are in actual need of nourishment. We must look within to find the strength and the belief that we are capable of taking charge of our lifestyle; that we have the self esteem we need to take pride in our health and well being, so that our children, too, can learn from our example.

the idea of portion control is not new. Rambam, a 12th century commentator and physician,

tried to encourage and urge the Jews to be a holy nation, to understand that it is possible to be a “navol b’reshus haTorah,” to present in-correct behavior even within the guidelines of torah. this means that even while reciting the appropriate Brachos, eating within the strict-est Kashrus guidelines, one can consume so much excess that the goal of the meal being holy and spiritual is no longer present. our sages constantly direct us to appropriately safeguard our health, as the body is a holy vessel that houses the soul.

Friends With Diabetes International, through its many programs and services, and especially through this publication “Going ForWarD,” is there to motivate, inspire, and encourage via education and support. Let us all use this inspi-ration to become active in our own health.

I have confidence in your abilities. Every single one of you has the potential to control your diabetes. So don’t forget to watch your portion size and count your carbs!

the following letter was addressed to M.L. and R.G. from England, in response to the poem that was printed in the summer edition of Going ForWarD.

Dear girls,You expressed so much emunah in your poem, yet your suffering can be clearly felt. the challenge of

diabetes is consuming your life. the secret you carry with you all the time, along with your constant struggle to keep your levels steady, is wearing you down.

Perhaps there is another way?

Summer 5767 | 5 | Going ForWarD

Dear Editor,

Please allow me to share my thoughts in response to the poem submitted by M. L. and R. G. from England (summer issue):

a teacher who was very beloved by her students announced to her class: “I’m looking for two very capable, strong girls to carry this heavy box which is very valuable to me.” the students were all eager to be chosen, because this would indicate that the teacher thinks highly of them. after scrutinizing all her students, she chose two responsible, bright girls whom she knew she could trust with carrying the valuable package. after dismissal, these girls were put at the front of the line, very close to their teacher, because they carried such a responsibility and the teacher wanted to be there to help them if necessary. the bond between the teacher and these two students was strengthened, and the girls were nicely rewarded for their job well done.

Dear M.L. and R.G.,those of us who have diabetes have been chosen for something special; and although we weren’t exactly eager to receive this package, since we did get it, we need to appreciate the advantages. our

beloved Father has chosen us since He knows that you are more capable than others around us to carry and handle this particular situation correctly. We have a responsibility to use this condition as a tool to become better people. this challenge has the potential to help us grow spiritually, since we are placed closer to the Ribono Shel olam. How lucky we are to have a closer relationship to the Eibershter Who is watching us carefully and helping us all the time!

the Gemara teaches that the Shechina rests above the head of a person who is not completely healthy because he is removed from the usual realm of nature and Hashem maintains a special guard over him. By thinking “it’s too hard for me to deal with this” or “why did I deserve this,” the person loses out on a tremendous opportunity. We need to look deep within ourselves and we’ll find the strength and capability to deal with it. and if we look around we’ll see the Hashgacha in every step of our lives.

Let us carry this valuable package with care, by staying in control of our diabetes and emotions. May Hashem help us deal with this test and use this opportunity to reach greater heights.R.L.G.

there are two parts to dealing with diabetes: the physical (or medical) part, and the psychological aspect. Both are crucial for good health and proper adjustment to life with diabetes.

If your diabetes is taking its toll on you, it might be time to determine which of these aspects needs a boost. First, review your medical situation. are you receiving the best care? Does your doctor understand your individual needs? Is he or she completely up-to-date with the latest developments in diabetes health? (as bizarre as it sounds, too many doctors are simply unaware of many advancements in diabetes management. Educate yourself and know which questions to ask your doctor!) Do you have a good diabetes educator? Have you tried using an insulin pump?

after you’ve checked off your list of medical needs, it’s time to examine your psychological needs – which has a rather small list: a positive attitude and proper support.

a positive attitude is all it takes to transform a monstrous ordeal into the surmountable challenge it is. the world has seen depression among the wealthiest and healthiest, and true inner happiness among the ill and needy. It all depends on one’s frame of mind! there is a famous parable about a king who wanted to find the happiest person in the world. He searched among the rich and famous, but couldn’t find a truly happy person. He lowered his standards and searched among the middle-class people, but

still couldn’t find the person he was looking for. after searching high and low, he finally found a homeless, beggarly individual who was shining with inner joy, because he mastered the art of counting his blessings and being sincerely grateful for them.

We have so many blessings to count! Living with diabetes in our day and age is hardly the terrible calamity it was just a few years ago. our (type 1) diets are not restricted; our activities aren’t limited. We should be positive about it!

another crucial factor in our psychological wellbeing is proper support from family and friends. You girls are indeed lucky to at least have each other, yet you mention the terrible load of your secret. You are certainly entitled to your own decisions about disclosing or concealing your diabetes. However, you must realize that a major drawback to keeping it secret is that you lose out on so much positive support! the people around us are usually extremely accepting, especially of a medical condition that doesn’t change you in any noticeable way. Conquer your fear, be confident in your own heart, and much of the burden will simply fall away.

I wish you lots of good health, hatzlocha, and strength! You are Hashem’s precious children, with unique, tailor-made nisyonos that are the very best for you!

Sincerely, MH

Whether you spell them latkas or latkes, they’re absolutely

luscious! although potato latkes are a traditional Chanukah treat, why not experiment by combining them with other vegetables such as sweet potatoes, carrots, spinach or zucchini to lower the glycemic load? You can also make delicious low-carb latkes using cauliflower or broccoli.

For potato latkes, i like to use idaho (russet) potatoes because they are higher in starch and have a lower

water content. You can also use red potatoes or Yukon Golds suc-cessfully. the processor grates po-tatoes in a flash! immediately add grated onions - the onion juice forms a barrier against the oxygen, preventing the potatoes from turn-ing black. some cooks like to rinse the grated potatoes in a colander, squeezing them dry. You can also crush half of a Vitamin C tablet and add it to the grated potatoes to keep the mixture white.

Lighten up! instead of frying, bake latkes for a lower-fat version. alter-natively, use a non-stick skillet and a minimum of oil. i find olive oil gives a wonderful flavor.

Latkes can be made in advance and frozen. after cooking, arrange them in a single layer on a baking sheet. When frozen, wrap them well. to save space when freezing or reheating latkes, stand them upright in a loaf pan, like soldiers! reheat them uncovered at 375 de-grees F for about 10 minutes, until crispy and piping hot.

You can serve latkes with a dollop of applesauce, low-fat sour cream or yogurt. For an elegant appetizer, make small latkes and top them with tzadziki, thinly sliced smoked salmon and a sprig of dill. to serve with a meat meal, top with a dollop of salsa.

two recipes are from my new cook-book norene’s healthy Kitchen, which contains 600 quick, easy, heart-healthy recipes that are ex-cellent for those concerned with their carbs, calories and fat intake. there are lots of chef’s secrets, nutrition notes and excellent in-formation about the glycemic in-dex. the recipe for no-Fry Potato and spinach Latkes comes from healthy helpings (originally pub-lished as MealLeaniYumm!) and are a tried and true favorite. both

These luscious, low-carb latkes are a delicious alternative to traditional potato latkes. They’re excellent for Chanukah or any time of year.

½ medium cauliflower, cut into florets (about 4 cups)

1 medium onion, chopped

1 clove garlic (about 1 tsp minced)

1 large egg

¼ cup matzo meal (prefer-ably whole wheat)

2 tbsp minced fresh dillweed

¾ tsp salt (or to taste)

¼ tsp freshly ground black pepper

2 tbsp vegetable oil, for frying

Chef’s SecretsDon’t Flip out! If the latkes aren’t thoroughly

•cooked, they’ll fall apart when you try to flip

them over.Hot Stuff: to reheat, arrange the latkes in a

•single layer on a foil-lined baking sheet. Bake

uncovered in a preheated 375°F oven for 8 to 10 minutes, until hot and crispy.

Steam the cauliflower for 10 minutes or until 1. tender. (or microwave covered on high for 6 to 7 minutes.) You should have about 3 cups once it’s cooked.

In a food processor fitted with the steel blade, 2. process the cauliflower until mashed, about 10 to 12 seconds. add the remaining ingredi-ents except the oil; process with quick on/off pulses to combine. If the mixture seems too loose, add a little extra matzo meal.

Heat the oil in a large nonstick skillet over 3. medium heat. Drop the mixture by rounded spoonfuls into the hot oil to form pancakes. Flatten each one slightly with the back of the

spoon. Brown 4 to 5 minutes on each side or until golden brown and cooked through. Spray the skillet with cooking spray between batches and add more oil as needed. Drain the cooked latkes well on paper towels to absorb extra oil.

Broccoli Latkes: Substitute 4 cups of broccoli florets for cauliflower.

Yield: 14 latkes. Recipe easily doubles and triples. Reheats well (see Chef’s Secrets below). Freezing isn’t recom-mended.

Everyone loves latkes! This luscious recipe from Beverley Corber, of Vancouver, B.C., makes a big batch so it’s perfect for a crowd. While making them, it was tricky to keep track of how many latkes the recipe actually made because we kept tasting them to make sure they were perfect—and they were.

Winter 5768 | 11 | Going ForWarD

2 medium sweet potatoes, peeled

2 large carrots or 12 baby carrots

2 large parsnips

2 large Idaho potatoes

½ Vidalia or sweet onion

4 to 5 green onions

¼ cup fresh dillweed

6 large eggs

1 cup whole wheat flour or matzo meal

2 tsp salt

½ tsp freshly ground black pepper

¼ cup grapeseed or canola oil (plus more as needed)

Chef’s SecretsEggs-actly! Instead of 6 eggs, substitute with 4 eggs and 4 egg whites or 1¼ •cups liquid egg substitute.

Grate tips: Beverley rinses the grated potatoes and then squeezes out the •excess moisture—I don’t bother. She likes to use grapeseed oil as it has a high smoking point, so you can cook at higher temperatures without burning the latkes.

Warm It Up: to keep cooked latkes warm, arrange in a single layer on a large •baking sheet and place in 250°F oven. Do not cover.

Frozen assets: to save space when freezing or reheating latkes, stand them •upright in loaf pans. Reheat, uncovered, in a preheated 400°F oven for about 10 minutes or until crispy. there’s no need to defrost them first.

Peel the vegetables and cut into chunks. In 1. a food processor fitted with the grater, grate the sweet potatoes, carrots, and parsnips in batches through the feed tube, using medium pressure on the pusher. transfer to a large bowl and set aside. Next, grate the potatoes in batches through the feed tube, using me-dium pressure on the pusher. transfer to the bowl of grated vegetables.

Remove the grater and insert the steel blade 2. in the food processor. Process the Vidalia onion, green onions, and dillweed until finely minced, about 8 to 10 seconds. add the eggs to the processor bowl and process for 5 seconds longer. transfer the mixture to the

grated vegetables along with the flour, salt, and pepper; mix well.

Spray a large nonstick skillet with cooking 3. spray. add 2 tbsp oil and heat over medium high heat. Using a tablespoon, drop the mix-ture by rounded spoonfuls into the hot oil and brown well on both sides, about 2 to 3 minutes per side. Remove the browned latkes from the pan and drain well on paper tow-els. add additional oil to the pan as needed and stir the batter before cooking each new batch.

Yield: 48 latkes. Keeps for up to 2 to 3 days in the refrigerator; reheats well. Freezes well for up to

2 months.

Traditional latkes are usually fried in at least ¼ cup of oil, so one latke contains about 3 grams of fat. i don’t know anyone who can stop at just one latke, so these “no-guilt” latkes are a terrific alternative. Each one contains just over a gram of fat – that’s a miracle!

“I wish there was someone who would be able to figure out which foods work best for me.”

“I wish there was someone who would be able to answer all my questions on dieting

The Balanced Diet Program is truly a wish come true. For over eight years, people from many communities have turned to the Balanced Diet Program to have customized diets formulated especially for them, with their specific needs and preferences taken into consideration. The program has helped hundreds of men, women and children lose weight and control their health conditions without feeling deprived.

The Balanced Diet Program began in Brooklyn, and after many years of success, it has expand-ed to Monsey and other localities. In addition to the office locations, the program offers a fax-membership whereby clients can com-municate with the program counselor with-out having to travel out of town for personal consultations. Clients also have the option to call or fax their ques-tions at any time of day, and to

consult with their counselor whenever the need arises.

The secret of the program’s success is that it never forces clients into standard diet plans; instead, it focuses on working around each in-dividual’s needs and lifestyle. That’s why the Balanced Diet Program is the ideal solution for people with health issues, such as diabetes.

Measuring food and designing menus isn’t all there is to the Balanced Diet Program. The pro-gram concentrates equally on the psychological aspect of weight loss. The strong connection be-tween counselor and client helps dieters achieve success faster and easier than they ever thought possible, because a positive outlook and proper perspective on health maintenance is crucial for successful weight loss. The program has suc-cessfully encouraged many people to take con-trol of their diets, especially teenagers.

There must be a reason why people from all over trust the Balanced Diet Program. It is personal,

effective and non-restrictive. Enjoy the

it must’ve happened to you al-ready. it’s six-thirty, your mind is

awhirl with the day’s activities, and you’re just about to sit down to a tempting dinner. it’s probably only by the sheer force of habit that you absently grab your blood sugar meter and go through the mo-tions of a routine bG check. insert strip, prick finger, squeeze, wipe… and you’re just about to zip the meter case closed. Your mind is already tasting those scrumptious meatballs and spaghetti, when the numbers patiently staring at you finally grab your attention. My blood sugar is what?!

it could be a reading of 346 mg/dl, straight out of the blue. Or maybe that meter is informing you that your current blood sugar is 48 mg/dl, when you’re feeling just fine. but the thought that follows is usually

the same: is this meter really accu-rate? For people who rely on the little machine for a glimpse into the mysterious goings-on in their bodies, the thought is treacherous. We trust those numbers implicitly, using them to make delicate deci-sions in insulin or medication dos-ing, faithfully recording them (okay, some of the time), and even allow-ing them to dictate our moods and feelings of self-worth! is it possible that our blood sugar readings are not entirely accurate?

the Lawaccording to the Food and drug administration (Fda), your blood glucose monitor is considered ac-curate if its results usually (96% of the time) fall within 20 percent, in either direction, of actual blood

sugars. the widely accepted isO (international standards Organiza-tion) guidelines differ somewhat, stating that glucose values below 75 mg/dl should fall within 15 mg/dl of actual sugars.

in plain language, that means that if your meter reads 100 mg/dl, your blood sugar may really be any-where between 80 and 120mg/dl. a reading of 60 mg/dl means your blood glucose is anywhere be-tween 45 and 75 mg/dl, and things get really scary at the high end of the spectrum, where a 300 mg/dl, can mean anything from 240 to 360 mg/dl! (in fact, rabbi Meisels ponders, meter inaccuracy may be part of the reason why our efforts at treating high blood sugars are so often inadequate. Perhaps the actual blood sugar was quite a bit higher than our meter thought it was, and so the correction dose we delivered really was not suf-ficient.)

seeing these chilling numbers in bold print, it’s not surprising that many people feel the Fda should enforce more stringent standards of meter accuracy. david Mendosa, a popular web writer on diabetes

topics, goes so far as to brand this “woeful lack of meter accuracy” a scandal. so much effort is invested into careful carb counting, precise insulin dosing, and accurate treat-ment of high and low blood sug-

ars; it is indeed scandalous to think that the all-important numbers at the basis of these decisions are only weak approximates.

Practically Speaking…it’s true: the numbers are fright-ening, but they may not be quite as problematic as they appear. a study gauging the accuracy of Lifescan’s One touch Ultra meters concluded that the test results were within 10% of true blood glucose values about 80% of the time, and they were rarely less than 15% accurate. We may want our meters to do even better than that, but, practically speaking, this level of accuracy doesn’t cause much harm.

blood glucose monitor is considered accurate if its results usually fall within 20% of actual blood sugars.

Going ForWarD | 16 | Winter 5768

When blood sugars are high, most people are somewhat resistant to insulin, so it probably doesn’t hurt if an overcorrection is administered based on a blood sugar reading that is higher than it actually should be. and when treating low blood sugars, we tend to go overboard, often consuming many more carbohydrates than we really need to. (raiding the fridge, anyone?) so, once again, our blood glucose monitor can’t really be blamed even if the number it displayed was not all that exact.

Furthermore, meter inaccuracies are probably less significant than errors caused by faulty carb count-ing or when drawing up an imper-fect amount of insulin by syringe. and let’s not forget that even those much-touted lab results, against which meter results are routinely compared, also contain a certain margin of error.

sometimes, an important medi-cal decision will be based on one specific blood glucose value. For example, a doctor may insist that a woman who has gestational dia-betes start insulin therapy if her morning blood sugar rises even just a drop over 100 mg/dl. in such a

case, remember that we can never view a single blood glucose result as being absolutely accurate. how-ever, if results remain consistent

over several days, then a trend has been established and we can be pretty certain

that those numbers are reflecting reality.

What You Can DoYou can’t make your meter more accurate

than it was designed to be, but there’s still a lot

you can do (or shouldn’t be doing) to ensure optimum results.

Code with Care:Most meters on the market require coding or calibration when a new batch of strips is started. this de-tail is very important! research has shown that miscoding can greatly hinder the accuracy of your meter readings, causing potentially seri-ous insulin dosing errors.

some meters are automatically calibrated without user input and do not require coding at all. “the autocoded meters… give bG val-ues that result in the lowest risk of insulin dose error”, states a recent

study (March 2007) published in the Journal of diabetes science and technology. but if you stick to your trusty meter that does re-quire manual coding, remember to do so conscientiously.

test Strip Smarts:before starting on a new vial of test strips, check its expiration date. You may be surprised to see that the lot expired a while ago, potentially altering the quality of the strips. For this reason, it is wise to use all strips from a previous shipment before starting on those from a newer shipment.

Manufacturers also routinely warn, “Do not store Test Strips outside the vial”, and with good reason. When test strips are exposed to air, the humidity effects a slow decline in their function. When you do need to open the vial, be sure to close it as soon as possible. in fact, some people have reported a noticeable deterioration in meter accuracy when using a strip that was left out of the vial for even a few minutes. it pays to follow directions!

remember that test strips should not be exposed to extreme heat, cold, or humidity. so a hot window sill or a steamy bathroom would not be ideal places to store your

strips. We inquired if test strips could be safely stored in luggage placed in an airplane’s cargo hold, where temperatures are very low. Kim Kelly, director of diabetes Programs at Lifescan, responded, “the strips perform best at room temperature, but transporting them in cold cargo holds is not as bad as you might think, as airlines often carry animals in the cargo hold and would be severely criti-cized if they were too cold.”

When obtaining a Blood Sample: before you do anything, make sure your hands are clean. One member related, “Whenever i get an unex-pected bG reading, i always wash and wipe my hands before retest-ing. i am [often] surprised to get a completely different result the second time around.“ in fact, this is a very common scenario, espe-cially if your hands were in recent contact with food. Washing them, or even just giving your hands a quick look-over before checking blood sugars, is as easy as it’s ef-fective.

next, be sure to fill the test strip completely. if too little blood is applied, some meters will sound an alarm, but others will issue a

reading as usual… except that the result will be very inaccurate. don’t milk your finger excessively, as this can dislodge interstitial fluid along with the blood, causing distorted blood sugar results. and, finally, use a fresh puncture site and blood sample each time you perform a bG check, even if you will be test-ing several times consecutively. (some people check their sugars repeatedly on the same meter to see if the machine will produce the same results for each consecutive test. if you do this, remember that your meter is okay even if the read-ings differ slightly from each other, but if the results vary substantially, contact the manufacturer.)

agaMatrix inc. has produced a new meter called the Wavesense Keynote Tm, which is being touted as the most accu-rate meter on the market currently available for home use. so the wave of the future may be upon us, it seems, bringing me-ters with greater accuracy (although this assumption is tentative, as this new meter has not yet undergone cer-tain studies). and while we’re talking about accuracy, let’s remember to pay attention to the details in blood sugar management that will really keep those num-bers precisely in range! n

“mommy, What’sdiabetes?”

By Menucha Jacobs

When Gittel’s* oldest daugh-ter was just three years

old, she watched intently as her Mommy packed syringes, insulin,

and testing supplies for a day trip. As if to confirm an obvious fact, the little girl clarified, “One needle for you, and one for Aunt Chany, right?”

Gittel was mystified. “For Aunt Chany? No, darling, these are just for Mommy.”

Confusion cast shadows over her daughter’s charming face.

“But don’t all Mommies have diabe-tes?”

as parents, our job is never done. throughout the day (and some-times the night!), and in every area of our lives… we’re always explaining, teaching, and trans-mitting ideas and information to the next generation. When a par-ent has diabetes, how does s/he explain the condition to children at varying age levels? What role of involvement do offspring play in

managing the parent’s diabetes? Going Forward explores these and so many other issues that face parents with diabetes on a daily basis.

An Answer For every Question

When Gittel recalls the above in-cident, she laughs. “i explained to my daughter, who is ten years old today, that baruch hashem, most Mommies do not have diabetes. it is just something that i happen to have.”

Joe solowiejczyk, rn MsW Cde, approves of this approach: “Open and honest dialogue provides both the children and the adults [involved] an opportunity to work through the realities of difficult situations. the child receives the message that it’s okay to ask ques-tions and to wrestle with difficult thoughts and concepts.”

and in the words of rabbi Meisels: “it’s very important to be open with our children and to provide hon-

est and uncomplicated answers to their questions. exactly what and how much to say will depend on the age of the child you are talking to. but the idea is consistent: every question needs an answer.”

besides for the straight-talk, older children can also learn more about diabetes by reading books. “My children love to read,” says rabbi Meisels, “and they’ve read through all the age-appropriate diabetes books in the house.”

as for the little folk? When younger children witness a parent checking blood sugars or administering in-sulin, it is simple to explain, “this is what Mommy’s doctor told her to do.” Or, “now totty is taking medicine.” these answers are easy for young minds to grasp, but still show that you took the questions seriously.

and remember that children prob-ably won’t question that which they see all the time. When i asked Gittel how her children react to her pump supplies, which have been fixtures in her home for the last couple of years, she shook her head. “it’s no big deal. they just take it for granted.”

“Can Mom have a Piece of My birth-day Cake?” (Word association Pub-lishers, 2002) is a story and activity book aimed at elucidating what it means to a child, aged four to nine, when someone in the family has diabetes. We are proud to present an interview with rochelle L. stern, the author of this unique book.

FwD Your book addresses a very specific segment of the diabetes population and fills an original niche. Why did you decide to write the book?

r. s. My oldest daughter abby (abigail ruby is her birth name and she’s the main character in the book!) was 4 years old when she first asked me what my insulin shots were. at 4 years, as we know, chil-dren are naturally inquisitive and

and women of all ages are coming home from the doctor with a diag-nosis of type 1 diabetes. Many of these people are already parents of several children, and their new condition is very much lacking that blessed familiarity that allows Gittel’s offspring to “just take it for granted”.

Libby*, diagnosed originally with gestational diabetes, was told af-ter the birth of her second child that she was a full fledged type 1. “When reality finally penetrated, and the shock of the di-agnosis slowly passed, i began talking a lot to my children. My older daughter, then about 3 years old, became totally involved in my diabetes care. she accompanied me to numerous doctor’s ap-pointments, has watched me check my blood sug-ars probably hundreds of times, and sometimes even plays the role of the infamous diabetes police!”

Like Gittel’s little girl, Libby’s daughter, too, needed reassurance that not all Mommies automati-cally “graduate” to a life with diabetes. but perhaps because she was witness to the fear and the freshness of her mother’s just-di-agnosed condition, Libby’s 8-year-old is still very much aware of and attentive to her mother’s diabetes. Like the little grown-up that she is,

she sometimes attempts to “help” her mother manage her health. “When i check my blood sugars,” Libby laughs, “my oldest might tell me, ‘Mommy, are you high? Your doctor will be mad!’ i occa-sionally have to remind her that i will handle things on my own and make my own decisions.”

Mr. solowiejczyk points out the benefits of providing children with the opportunity to “help” the adults in their lives. “by al-lowing the child to participate in an appropriate way, the adult

is conveying a message about dealing with difficult personal situations and how to turn to others for help.”

winkies: treAt or treAtMent?

everyone i spoke to smiled ap-preciatively when the “Winkies dilemma” came up. Many people with diabetes com-monly store Winkies not just in kitchen cabinets, but in the bedroom, in the car, and in just about every drawer and pocket they own. nobody wants to

be caught without the life-giving sugar boost when they

need it most.

but does this sound familiar? You’re standing at the shoe store counter, fumbling for the right amount of change. as you fish around for a dollar bill, a roll of Winkies peeks out of your pocket. immediately, a

Page 23: GoinGF rWar - Friends With Diabetesso you can manage in any situation. First, Animas has the only insulin pump ... Dr. Don Zwickler Endocrine Associates of Rockland Pomona, NY

Going ForWarD | 22 | Winter 5768

tired voice whines, “Mommy, can i have a candy? but i was so good!” the last thing you want right then is a tantrum, but how can you af-ford to be without Winkies?

“My children always used to ask for Winkies,” Gittel acknowledges, “so i taught them that Winkies are very high in sugar and they are very unhealthy! Winkies are only for Mommy when her blood sugars are low! by now, this attitude is so ingrained, that my kids won’t even accept the stuff when my father brings it as a treat!”

rabbi Meisels echoes these words almost exactly. “i used to let my children take Winkies every time they encountered a bag of them in the car. Until… one day, i had a low blood sugar. i was in the car, and there were no more Winkies in the bag!”

since then, his policy is strictly, “Winkies are totty’s medicine. and think about it. if Winkies earned their status [as the ideal agent for treating hypoglycemia] because of the glucose they contain, do we want our children snacking on them?”

Low BLooD sugArsdr. richard bernstein famously tells the story of a young man who was experiencing a low blood sugar. When a family member attempted to hand him a glass of orange juice, the patient threw the life-giving liquid right back at his face. the glass came next.

dr. bernstein explains, “the cogni-tive difficulties that accompany severe hypoglycemia can make the slightest frustra-tion or irritation overwhelming.” Many people with diabetes indeed find themselves increas-ingly nervous, short-circuited, or inappropri-ately emotional in the throes of a low blood sugar. and who are the first ones to suffer from these mood swings, if not for our innocent and usually-present chil-dren…?

so how can we explain ourselves to them? rabbi Meisels says, “i tell my children that i don’t feel well. in general, my policy is that if i don’t feel well, i don’t act. When i know i’ll say something hurtful if i open my mouth, i’d rath-er not open my mouth at all.”

Gittel trained her children to offer her a glass of or-ange juice if she seems unduly nervous or emotional. she relates, “i was upset about something one morn-ing, and really ‘gave it’ to my kids. as i was yelling, my daughter asked me cautiously if i needed a cup of or-ange juice. i drank the

juice, then took insulin to cover it, all the while secretly grateful that my children would attribute this foul mood to a low blood sugar, and they would never know that i was just plain… an angry tyrant.” We shared a conspiratorial laugh. diabetes does have its perks!

severe hypoglycemia is b”h entire-ly avoidable, especially on the in-sulin pump, when insulin doses are properly set. but some parents, es-pecially the newly diagnosed and those who are particularly vulner-able to low blood sugars in certain stages, do find it reassuring when their children are trained to re-spond to an emergency situation. Libby remembers a particularly rough period when she was prone to debilitating lows. “i taught my children to contact my husband on his cell phone if they noticed me acting funny or talking irrationally. but i must say that when i did need them to do this for me, my children panicked completely and were un-able to be of help.”

Libby reassured her children by explaining that this difficult period was only a passing phase, and, in-deed, her numbers did smooth out considerably within a short while.

AttituDe is everything!

rabbi Meisels describes, “i often ask my children for one or another of my diabetes supplies. they can identify and bring test strips, car-tridges, infusion sets… just about

also tend to be afraid of needles! i perused all the children’s diabe-tes books in the library and in the bookstore in order to answer her question. to my surprise, i couldn’t find one book that was written for children who don’t have diabetes. i settled on a workbook that ex-plained diabetes in a simple and concise way to children who were diagnosed with the condition, but i felt that it was much too detailed for abby. i found myself reading the book and improvising the ma-terial so that it would make simple sense to her. that’s when i realized i should write a book by myself to target this market.

FwD We would imagine that many people were eager to read what seems to us the only book of its kind! What was the feed-back you received on your book?

. i’ve gotten a lot of great feedback on the book. i’ve read it to a few second and third grade classes, and inevitably, when i get to the part showing a glucose me-ter and test strips, several children jump up and say, “Ooooooh, my grandpa has one of those,” or “My mom has one of those.” You could almost hear the gears clicking in a child’s head when that happens. they start to put the information together and realize that their be-loved family member has diabetes. it always strikes me that even though we are in an era of open information, many adults don’t

anything. and my oldest daugh-ter, who is twelve years old, knows how to dole out my portion of sup-per using measuring cups, to aid in the carb counting. the little kids like to play with empty test strip containers, and they are absolutely fascinated when i take my symlin injections.”

but whether you decide to make your diabetes a “family affair” or not, one thing is certain: attitude is everything. it is up to us to pres-ent diabetes in a non-threatening and manageable light.

“Children learn by what they see, hear and feel,” asserts Mr. solow-iejczyk, “and when parents reflect a positive and relaxed attitude, children learn to feel okay about the situation and to feel good about themselves.”

Only our own cheerful outlook can help our children paint their world in happy, wondrous and vibrant colors! n

want to “burden” their children with information on diabetes out of the concern that it will make their child fearful. however, i think arming them with information actually eases the fear in children. they want to know about things around them, and they are not afraid to know about it. in fact, if they are properly informed, i think children could be very supportive of an adult with diabetes. My son, who is 9 years old, was recently told by a classmate that a relative got diabetes from eating too much sugar. he was able to correct his classmate’s error, and i even came down afterwards to explain more about diabetes.

FwD Your book was published in 2002. are there any changes or additions you would like to incorporate in future printings?

r. s. i would love to make an edition in spanish or illustratively geared to other ethnic groups in the U.s. also, as i am now working as a medical librarian for novo nor-disk, i am looking into the possibil-ity of incorporating my book into their programs and endeavors.

What i always like to fantasize about the book’s evolution is that it goes out of print because of the development of a cure for diabetes. then i would write the book called, “My Mom Used to have diabetes”!

his tread was heavy and ev-ery footstep required effort.

rabbi simcha Goldbrenner*, inter-nationally renowned and beloved educator, was suddenly besieged by doubt that he had acted wisely by agreeing to this strenuous hike. spring had just begun hint-ing at its arrival and Monsey was alive with the promise of glorious days to come. still, what rabbi Goldbrenner really needed that shabbos was decidedly not an extended nature walk, but rather a very long nap…

Yossi*, his devoted son, couldn’t help phrasing the obvious ques-tion. “ta, really, couldn’t you have just refused this invitation? You knew you would be return-ing from the chinuch conference right before shabbos, completely exhausted and looking forward to a relaxing day of rest. did you really have to agree to walk forty-five minutes to address a bunch of boys at some kind of, what is it, some diabetes convention?”

rabbi Goldbrenner looked confused himself. “You’re right,” he shrugged helplessly, “i can’t re-ally say why, but when rabbi Meisels called to hear if i would address the shabbaton he was planning, i just couldn’t refuse him. he sounded so noble, so dedicated to his goal of uplifting children with diabetes. i found that i simply couldn’t consid-er my own petty needs for sleep just then…”

but the indefatigable rabbi Goldbrenner was soon back to himself. smiling at his son, he finished heartily, “and we’ll probably never understand why i asked you to come along with me to this speaking engage-ment. but i certainly don’t regret

it!” thus ensued a spirited conver-sation between father and son, both of whom had a lot to catch up on since the latter’s previous visit home from Yeshivah.

“We’ll probably never under-stand..,“ rabbi Goldbrenner had said, but this time, the venerable educator was wrong. sometimes we trod surely and confidently down the winding path of life. at other times, we falter uncertainly, frightened and unsure. and then there are times when we think we’re heading one way, while we’re really approaching an entirely dif-ferent di-r e c t i o n … a l w a y s , the Master d i r e c t o r is guiding those foot-steps with clarity and with com-passion. and every once in a while, he lets us glimpse his intricate roadmap, and we are privileged to understand.

they arrived sooner than they ex-pected, and their first impressions of the shabbaton came as some-what of a surprise. they had envi-sioned, perhaps, a pitiful collection of children drawing strength and moral support in the face of a for-midable life challenge. What they actually found was an atmosphere that would later be described by one of the shabbaton’s speakers

as “entirely upbeat “. in fact, rabbi Goldbrenner’s first exclamation upon observing the festivities was, “but they’re normal kids!” as they had arrived a bit early, rabbi Gold-brenner and Yossi were treated to a detailed explanation by rabbi Meisels about the realities of living with diabetes in the twenty-first century. they learned about the tremendous medical and techno-logical advances that make dia-betes an extremely manageable condition. and, of course, seeing the lively and boisterous “pa-tients” in action, quickly relieved the Goldbrenners of any previous

misconcep-tions they may have h a r b o r e d . these were a group of t e e n a g e r s sharing a unique com-

mon denominator, but they were by no means a sorry lot.

his overwhelming fatigue not-withstanding, rabbi Goldbrenner gave a rousing oration. he made the boys laugh, he made them cry, and before they could realize what was happening, he made them learn. he taught them about the ribono shel Olam’s love for each and every one of us, and about the tremendous power we all possess to lead our lives as we should. he spoke, and the boys listened. it is no exaggeration to say that many of them were impacted for life.

and then, the emotional thank yous were over, and rabbi Gold-brenner headed home. Father and son had a lot to digest after this uplifting experience, and they slowly meandered down the path, their footsteps sounding softly in the night.

sundaythe phone rang in rabbi Gold-brenner’s study. the caller’s first “hello” revealed that he was un-easy with what he was planning to say, and when he identified himself as binyamin berkowitz*, some of that unease trickled over to rabbi Goldbrenner as well. attempting to lighten the atmosphere, rabbi Goldbrenner ex-claimed warmly, “Mr. berkowitz, what an honor! Yossi has the most wonderful things to say about your daughter! is there any-thing i can help with?” this par-ticular shidduch, he knew, looked extremely promising. in fact, it was one of the reasons why his diligent Yossi had been home for shabbos altogether that week.

“thank you!” Mr. berkowitz ac-knowledged, but he was still clearly tense. “the thing is… there is something you should know about our daughter. You see, she has type 1 diabetes, a condition you may not have heard too much

about. she is baruch hashem well-controlled and in otherwise perfect health.” now that he had begun talking, the information was emerging in a rush. “there is a devoted Jew in Monsey who heads an organization for people with diabetes. his name is rabbi Meisels, and he can surely clarify for you exactly what the condition entails. Perhaps you would like to speak to him?”

there was a heartbeat of silence before rabbi Goldbrenner cried, “rabbi Meisels? but i already spoke to him!”

ah, footsteps. there they were, several months later, bedecked in

finery as befit the occasion. and then the kallah arrived, looking splen-

did in her pristine white. slowly, she circled around the chassan, Yossi, seven times. these were somber footsteps, dancing foot-steps, laden with so much emo-tion, so many prayers and thanks. because we never know where the path of life will bring us, but always, the Master director guides each and every footstep with clarity and with compassion. and every once in a while, he lets us glimpse his intricate roadmap, and we are privileged to understand. n
