Jeans & Tee Shirts are strictly prohibited at the club Collared Shirts for the men (TUCKED IN AT ALL TIMES) No Tank Tops of any kind for men & women Hats in the forward position at all times Spike less shoes (REQUIRED)

Payment Schedule :

Deposits : $_________ deposit is to reserve the space shall be paid by the client upon the signing of this agreement. All deposits are non refundable.

Additional Payments as follows: A) If function is booked one or more years prior to the event, client is required to submit an

additional deposit of $1000.00 per year B) 50% deposit (3) months prior C) Balance of estimated bill to be paid by Cash or certified check ONLY 14 days prior to the

affair. d) Any remaining balances payable at the conclusion of function in CASH ONLY. Absolutely NO

personal checks or credit cards will be accepted for the final payment unless payment is made at least 2 months prior (Credit Card) or one month prior for checks.

Contract Authorization: In the event that this agreement is signed in the name of a corporation, partnership, club or society, the person signing represents to bvcc that he or she has full authority to sign such contract, and in the event they are not so authorized they will be personally liable for the faithful performance of this contract.

Conduct: We will not accept behavior that is not in the best interest of both the club or the function at hand. We reserve the right to ask an individual or individuals for behavior to leave the premise for behavior that is not conducive to club policies : Client agrees to begin its function promptly on the scheduled time and agrees to have its guests, invitees, and other persons vacate the designated function location at the closing hour indicated. the client furthermore agrees to the following overtime charges.( Clubhouse Events ) A $500.00 overtime charge will begin after 25 minutes of scheduled conclusion and every hour thereafter.. ( Pavilion ) $750.00 overtime charges will begin after 30 minutes of the scheduled conclusion and every hour thereafter. Client undertakes to conduct the event in an orderly manner in full compliance with the applicable laws, regulations & policies. Client assumes full responsibility for the conduct of all persons in attendance and for any damage done to any part of bvcc premise during any time such premise are under the control of the client, clients guests, invitees or clients outside vendors. Outside vendors are prohibited from consuming any alcoholic beverages, will be permitted to enjoy a meal after the clients and their guest have completed meal time. bvcc reserves the right to use legal measures to ensure the safety of both the employees, clients & guests. All independent contracts hired by the client must provide bvcc active insurance certificate coverage to perform any and all services at bvcc (TWO) weeks prior to the event.

Golf Carts / Damage: ________Intials A golf cart will be provided for two golfers only Additional golf carts provided for larger groups (14 ) days prior to the scheduled event) Any & all damages to the golf carts or golf course will be the responsibility of the host or group. All costs related

thereof will be the responsibility of the same. Note.... All damages must and will be relayed to either the management or golf shop immediately.

NON GOLFERS ATTENDING Are STRICKLY prohibited from using golf carts All golfers using golf carts must be over the age of 16 & have a valid driver's license. • The conduct & behavior of the guests is solely the responsibility of the host. The host will be responsible for any and

all damages & personal injury caused by his or her guests. • All guest are prohibited from bringing coolers, beverages of any kind, food or controlled substance on to the premise.

Guests will be asked to leave the facility.

SINAGE & FINAL COUNT: _________Initials: Bella Vista Country Club will place any and all required signage & sponsor signs & must be delivered (3 ) days prior to

the event. and be picked up at the conclusion of the event . The host of the event is responsible for providing Bella Vista Country Club (1) weeks prior to the event a golfer count

and foursome list.

CANCELLATION __________Initials

Clients shall provide evidence of the intent to cancel the agreement by registered or certified mail return receipt requested. In the event of such cancellation by the client, or in the event of a breach of any of the terms of this agreement by the client or in the event the agreement is terminated by bvcc because of a breach by the patron, bvcc may at its option, be entitled to receive, collect or hold a sum equal to 60% (60%) of the maximum contract price as and for liquidated damages to bvcc. This amount shall hold only to and including three (3) months prior to function. any cancellation within three (3) months prior to function entitles bvcc to receive up to 100% of the maximum contract price, offset only by replacement booking and then, only to the extent the replacement booking equals portion of said booking. ALL DEPOSITS are NON REFUNDABLE.

Inclement Weather :_________Initials In the event of inclement weather, the outing will continue as scheduled unless the course is deemed unplayable by the

Golf course management, Golf professional or superintendant. Each golfer will be issued a voucher to return with his foursome to enjoy a round of golf with the scheduled year.
