The client defined health "Makikita ko nman pag malusog ang katawan ko" as verbalized by the patient. For his opinion he defined healthy; because he is happy and he live his life to the fullest,

While he is in the hospital he cant defined his health as healthy.

Because he is in pain.

NUTRITIONAL & METABOLIC

PATTERN

The client eats healthy foods everyday, he doesn’t takes any food 1upplement. He always drink 7-8 glasses of water everyday, he said water is more vital to life than foods, in his breakfast he ate only light foods. He usually eats alone and when at home he only prepared foods only for his self.

When his in hospital he only drinks is water and he only eat any foods but low in salt, before he admitted at the hospital his still smoking.

To maintained his good condition.

ELIMINATION PATTERN

The client verbalized "Bago ako na pasok dito sa hospital regular naman ang pag tae ko sa 1 araw lagi naman ako na tae" he also said sometimes her bowel has a odor. He urinate at regular intervals everyday

While his in the hospital, his urine color is yellowish in color but sometimes it is colorless. He doesn't have any problem about his perspiration, and he doesn't have body odor.

To identify if there are any infection or difficulty in elimination.

ACTIVITY – EXERCISE PATTERN

My client knows that activity or exercise increase fatigue, before he admitted at the hospital he always play basketball 4 times a week, surfing

While he is in the hospital he always play PSP and watch TV sometimes he takes a little walk around the 5th floor of the hospital and he

To feel relax and forget the pain he encounter.

SLEEP - REST PATTERN

He usually sleep every 9pm to 7 am, he go to his work everyday he feel refresh every time he is awake, he wakes up on his own he always dream at night, but he doesn’t snore. He prefers to sleep in quiet and dark room.

The client is restless “Hindi ako masyado makapag pahinga kasi masakit yung kanan na lungs ko” as vervalized by the patient. His sleep rest pattern while he is in the hospital he sleep at 11pm and he wake up at 6-7 am.

He can’t tolerate the pain he feel.

COGNITIVE – PERCEPTUAL

PATTERN

He doesn’t have any difficulty in reading and writing, he also doesn’t have any hearing problem, he has a 15/20 eye vision.

He can learn easily and understand all the words you tell to him, his memory is normal and he speaks clearly.

SELF PERCEPTION – SELF CONTROL

He describe his self as a healthy in his mind and body, he take care of his body properly he always try to look good, he said sometimes he is moody specially when he is tires and sleepy.

He can’t take care of his self because of pain he feel, he is always moody because he is restless.

ROLE RELATIONSHIP

PATTERN

He lives at concepcion road, with his family if there are a problem with their family he always do his best to resolve it sometimes problem of their family is financial.

He tries to be strong to make his self feel better and his family not to worry about him.

PATTERN

He defined sexuality as a degree to which a person exhibits and experiences maleness or femaleness

He can enjoy his relationship with other because his in the hospital, he feel alone.

physically, emotionally and mentally. He said he is comfortable with his sexual identity.

COPING AND STRESS

TOLERANCE

He always sleeps regular and always 8 hours, he doesn’t feel any stress.

He feels stress because of incomplete rest because he feels pain every time.

VALUE BELIEF PATTERN

He believes in god and before he sleeps he is always praying.

He always prays to get well soon and thanks to god for his second chance.
