DECISION MEMO: Governing Board Managed Care Organization Seat Memo prepared by: Christina Hulet, on behalf of the Executive Committee Date prepared: December 20, 2019 Date of proposed action: January 9, 2020

Issue The region’s five Managed Care Organizations (MCOs) are responsible for nominating one representative to serve on HealthierHere’s Governing Board. Consistent with HealthierHere’s bylaws, the MCOs have elected to rotate their seat appointment on an annual basis. Marissa Ingalls of Coordinated Care served in 2019. It is now time to appoint a new Governing Board member for 2020.

Background HealthierHere’s 26-member Governing Board was established with designated seats representing specific sectors/perspectives (see chart below). In allocating seats, the team sought to balance representation between public, private, and non-profit organizations, as well as between institutional health care partners and community-based organizations/members. In addition, some seats were designated as ‘open’ (i.e., open to applicants from the general public), while others were ‘reserved’ for lead entities to nominate a candidate for the board’s approval.

The Managed Care Organization (MCO) seat is ‘reserved’ to the MCOs to nominate their preferred candidate. As such, the Executive Committee received and reviewed their recent request to appoint Caitlin Safford to the Governing Board for 2020. Caitlin Safford serves as the Director of External Affairs and Community Development for Amerigroup. Prior to this, she was the Strategic Policy and Communications Lead for Health Systems Transformation and Innovation at the Washington State Department of Health. Attached is her full board application and a letter of support from the Managed Care Organizations.

Recommendation The Executive Committee recommends that the Governing Board approve the MCO’s nomination to appoint Caitlin Safford to the MCO seat. Caitlin brings over ten years in healthcare and social service experience, including a focus on policy and systems change transformation. She has also been actively involved in ACH boards and workgroups statewide for the past four years.

1. Which sector seat you are interested in filling? Why are you interested in serving on this board? MCO sector. It happens to be Amerigroup’s turn at the helm for MCOs but I also am now a resident of King County, and I wasn’t when the ACH first was formed, so I have a vested County resident interest in seeing a better system built within the whole region.

2. What experiences have you had with healthcare, social service systems? I have 10 years in healthcare and social services, mostly on the policy and programmatic side. Most recently, I have spent the last 5 years working on health care transformation implementation, related specifically to policy making and systems change implementation.

3. In what cross-sector and community partnerships are you involved and what strategies will you use to keep them engaged in this work?

I’ve been serving on ACH Boards and workgroups statewide for 4 years, which has included working closely with all sectors to make decisions on the right path forward for the ACH. Additionally, I’ve served in coalition with a variety of organizations to create and advocate for more effective policy changes. The skills I view important in this are having the courage to reach out to partners you wouldn’t normally work with and listen to what you have in common and how they believe you can work together so it’s not one-sided.

4. What talents, skills, or qualifications do you have that would help you be successful in this role? I listen well, communicate back to my sector with ease, am a systems-thinker and strategizer by nature, and have a knack for explaining complex topics in simpler terms for those who aren’t embedded in the day-to-day work.

Strategic Planning Susan McLaughlin presented the results of HealthierHere’s strategic planning/sustainability survey which was distributed to board members and partners in November. See Powerpoint presentation on HealthierHere’s website. Governing Board reflections on survey results:

• Interest in prioritizing policy work; HealthierHere has a unique position/opportunity as a neutral convener; some were surprised that this scored low on the survey

• Interest in HealthierHere serving/expanding its role as an equity advocate • Surprise that shared care planning, paramedicine and opioid work scored low; could be a

function of being early in implementation • Opportunity to better communicate and make HealthierHere’s work more visible • Theme around HealthierHere spreading its limited dollars too thin; do we need to focus our

work for greater impact?

Governing board and community members broke up into 3 small groups to discuss HealthierHere’s potential role in three areas prioritized by survey respondents. Key themes included: HealthierHere as Service Organization:

• Clear majority for prioritizing Community Information Exchange (CIE) and data sharing; o Window of opportunity to advance CIE as a public utility o Consider local partners such as Microsoft, Amazon, EPIC

• Provide training and technical assistance (e.g., on practice transformation, bidirectional care, behavioral health)

• Consider targeted innovations (e.g., piloting services for most vulnerable populations in partnership with MCOs and clinical providers)

• Policy/advocacy support • Health literacy • Complex care coordination • Development/dissemination of evidence-based practices

HealthierHere as Regional Convener:

• Themes related to the tone and tenor of how HealthierHere should be a regional convener (e.g., cross-sector table, outreach, highlighting successes)

• Bring community voice, community engagement and equity to all tables (e.g., to clinical/system players)

• Legislative policy/advocacy work; HealthierHere is particularly well positioned to create/advance policy across stakeholders

Community Voice/Public Comment Betsy Lieberman opened the floor for public comment. There was no public comment.

Board Business Meeting Minutes The board reviewed and approved the November meeting minutes with no revisions. Abstentions: Roi-Martin Brown (Washington Community Action Network), Kristin Conn (Kaiser Permanente Medical Group), Shelley Cooper-Ashford (Center for Multicultural Health), Cathy Knight (Seattle/King County Aging & Disability Services), Sherry Williams (Swedish Medical Center)

Executive Director’s Report Susan McLaughlin referred to the Executive Director’s Report. She also noted that the State Department of Corrections (DOC) has received funding to open 200 new work-release beds, including some in King County. DOC is convening a workgroup; if members are interested in participating, please let Susan know. Several reports are in process. The state’s mid-point assessment report has been delayed; the draft report will be available January 6th, followed by public comment, and a final report in early February. The ACH self-sufficiency legislative report will be released mid-December. The independent evaluator report is expected in March 2020.

Finance Committee Business 2020 Administrative Budget Thuy Hua-Ly presented the 2020 administrative budget decision memo. This $4.8 million budget was previewed at the November Governing Board meeting and is being recommended for approval by the Finance Committee. There is an 18% total increase in salaries and benefits, which includes 2 additional FTEs and a 4% COLA/merit raise. The board reviewed and unanimously approved the 2020 administrative budget. There were no abstentions.

IGT Payment Approval Thuy Hua-Ly presented the Intergovernmental Payment (IGT) decision memo. The Finance Committee recommended approval. The HCA developed this financing mechanism to address a funding shortfall with CMS. This mechanism serves as a passthrough to IGT contributors. The board reviewed and approved the IGT payment.

2020/2021 Investment Strategy Susan McLaughlin gave a preview of the 2020/2021 investment strategy for CBO, clinical and Tribal partners. See PowerPoint presentation on HealthierHere’s website. HealthierHere will have a total of $34.2 million for 2020/2021 investments, of which $15.9 million will be allocated to Medicaid providers, $14 million to CBOs, and $4.3 million to Tribes. HealthierHere joint committees are finalizing the investment strategy and will bring a recommendation to the board for approval in February. There will also be quarterly reporting to assess how HealthierHere is doing, as well as opportunities to reassess the investment strategy midway. Meeting adjourned.

Dear Governing Board Members:

Happy New Year! I hope you all had a joyful and relaxing holiday season. We are bringing in the new year and new decade with a sense of excitement and urgency as we prepare to make our largest investments yet to improve the health and well being of the people in our region, while at the same time navigating uncertainty at the Federal, State and local levels. We must act now to continue to support those individuals experiencing homelessness, poor health, suffering from mental illness and substance use disorders and those unable to access or navigate the complex delivery systems today.

As the 2020 Washington state legislative session gets ready to kick off, we will be relying on our Board members, committee members and partners to help us track key pieces of legislation that will impact our work. We have a great training scheduled for today remind us of the do’s and don’ts of advocacy to help us achieve our goals safely and effectively. We will also spend a bit of time deepening our discussions around equity, our role as Board members in helping to advance equity and act as allies to support all voices and perspectives.

As we transition to the new year, this quote stood out to me to mark our journey: “Coming together is a beginning. Keeping together is progress. Working together is success.” – Henry Ford

We’ve been coming together for a while now and have also managed to keep together for several years. Now let’s roll up those sleeves and get to work!

Monthly Spotlight: December Co-Design Collaborative Digs into CIE HealthierHere hosted its third Co-Design Collaborative on December 6, bringing together community, clinical and tribal partners to connect and collaborate. Following our October collaborative’s discussion about community-based care coordination, the December gathering focused on Community Information Exchange (CIE) as a tool that can support care coordination efforts. partners learned about CIE, discussed potential impacts in our region, and took a closer look at how a CIE platform works. Special thanks to Molly Blumgart (National Director, KP) from Unite Us for joining us to provide a demonstration of the Unite Us platform. Huge thanks also to our partners for taking time out of their day to examine, discuss and offer their thoughts and considerations about CIE for King County. We were encouraged to hear that 18 organizations expressed interest in exploring moving forward with CIE planning on Unite Us.

Highlights

Welcoming Christopher Green and Christine Jones to the HH Team We are thrilled to introduce our newest staff members, Chris Green and Christine Jones. Chris joins our team in the role of Practice Transformation Manager as part of the Clinical Practice Transformation Team, led by Michael McKee. Chris brings with him 20+ years of healthcare experience, most recently as a Population Health Analyst at UW Medicine, and he has been involved with projects regarding high-risk care management, risk assessment, and Bree Collaborative care transformation. Christine joins the team in the role of Administrative Specialist and reports to our Chief Operating Officer, Gena Morgan. Christine has a plethora of administrative experience, including support for Sound Community Bank and the American Cancer Society, and has achieved the Professional Administrative Certificate of Excellence (P.A.C.E.) in addition to other continuing education accomplishments.

Shared Care Plan Pilot Kick Off Congratulations Harborview Medical Center, Evergreen Treatment Services, and Sound Mental Health who kicked off their Shared Care Plan Pilot collaboration with a lively and productive first meeting in mid-November. They are developing a process to identify patients for the pilot and will be joined by a Federally Qualified Health Center partner in early 2020. Thanks to these participants and to the Shared Care Plan Workgroup, which will reconvene in the new year to share progress and gather input about the pilot. The Shared Care Plan pilot work will be coordinated with our Community Information Exchange efforts as we continue to identify ways to better connect our clinical and community partners.

Tribal Health Pay-for-Performance Reporting With advisement from our Tribal Partners, HealthierHere is working to adapt reporting tools to be more culturally appropriate. As part of this work, we recently launched a new Tribal Health reporting tool through which our Tribal Partners are participating in HealthierHere’s semiannual Pay-for-Progress reporting. Pay-for-Progress incentivizes and provides visibility to each Clinical Practice Partner’s improvement over self in areas such as whole person care screenings, population health (registries, risk stratification), and equity. Statewide Collective Ambulatory Workgroup Kick Off In partnership with the Washington State Hospital Association (WSHA), we held a kickoff webinar on December 13th for Washington State’s new Collective Platform Standards Workgroup. This workgroup aims to help support implementation and optimization of the Collective Ambulatory technology for Medicaid providers (including hospital/health systems, Community Health Centers and Behavioral Health Agencies) across the state. The workgroup brings together Accountable Communities of Health, providers, Managed Care Organizations, and Collective Medical Technologies (CMT) to develop standardized protocols and processes for the statewide use of Collective Ambulatory.

New Data Collected by Community Grants Partners

Kudos to HealthierHere’s second round of Community Grants Partners for collecting a new set of community data! Like the first round of participating community organizations, this second cohort of grantees set out to connect with members of their communities to learn about their experiences with health care – what’s working, what’s not, and how it can improve. Information was gathered via surveys and focus groups, and the data was submitted to HealthierHere in December. HealthierHere and Public Health – Seattle & King County (under contract with HealthierHere) will analyze the data and share summaries and insights in the coming months.

Joint Committee Meetings on 2020/2021 Investment Strategy January 14 & 23 HealthierHere’s Committees of the Board came together on November 20 for our first 2020/2021 Investment Strategy meeting. These meetings bring our committee members together to review the strategy proposed by HealthierHere, with the goal of jointly recommending an investment strategy proposal that will be brought to the Governing Board for approval. Thanks to everyone who participated in our first meeting, and to those who provided feedback on how we can best move the process forward. Keeping that feedback in mind, we will have two additional meetings in January to

Clinical Pay-for-Progress Reporting due January 3 HealthierHere’s second Pay-for-Progress reporting period ended on January 3rd. Clinical Practice Partners submitted their data, including self-evaluations related to bi-directional integration of care, population health, and provision of Opioid Use Disorder treatment. Earlier this year, the first reporting period provided a baseline for these measures and, moving forward, these reports will help us view progress over time and identify areas that need additional or ongoing support.

Whole Person Care Innovation Letters of Interest due January 17

In November HealthierHere invited Clinical Practice Partners to respond to a Request for Letters of Interest for a new innovation, ‘Testing Models for Integrated Care.’ Letters of interest are due by January 17 and interested partners will participate in a readiness assessment and site visit with HealthierHere and UW AIMS Center staff in February to assess the feasibility and potential to achieve the proposed innovation. This test of innovation aims to address the lack of communication and shared protocols for people who visit multiple systems to meet their physical and behavioral health needs. The proposed interventions will be scalable to other organizations and will address the following question: ‘What does it take to provide better, more integrated care for the defined focus population?’
