Subject: Interview for admission to AFIH Course 2016–17 -reg.

(Provisional)

Dear Doctors,

Please refer to your application on the above subject submitted to this institute. In this connection you are required to appear before the selection committee at this institute on Friday, 25th November, 2016 at 1000 hrs. You must bring the following documents along with you, failing which you may not be interviewed.

a. Original Certificate (showing the date of birth) and mark-sheet of High School or equivalent.

b. Original MBBS degree & other PG degree or diplomas or other examinations passed mentioned in the application. Admission will not be granted based upon provisional certificates issued by the college/university.

c. MCI/State Medical Council registration certificate.

d. Internship completion certificate.

e. Original valid experience certificates for Minimum of 1 year experience in industry or relevant field of occupational health or 2 years experience otherwise, after completion of compulsory internship. The cut - off date for counting two years of experience after internship is 1st December, 2016.

f. Original sponsorship /permission letter by the present employer on letter head (if not already sent) “No objection certificate” will not be treated as a permission letter.

g. Original valid cast certificate if applicable (Latest in case of OBC)

The selected candidates will be required to deposit the fees for admission as under on 1st December, 2016 along with five colour passport size photographs and self attested photocopies of all documents.

The course will start from 1st December, 2016 and the attendance for all the students will reckoned from this date.

Institution Fees: Rs 2400/-(non-refundable) as Institution Fee, for the entire course. This amount will have to be paid by Cash/ Demand Draft drawn in favor of the Regional Labour Institute payable at Faridabad.

Note:

1. The course is non residential and staying in RLI hostel is not compulsory. The twin sharing Non Family AC hostel accommodation would be allotted on first cum first served basis on

payment of Rs 75/- (Food charges extra) per day per person.

2. Mere fulfillment of minimum requirements and call for this interview is not an assurance for admission to the course.

3. This institute will not pay any T.A. /D.A. for attending the interviews.
