1. Tenders are to be addressed to the Chief Medical Officer of Health, Dakshin Dinajpur, Nelson MandelaSarani, Balurghat, Dakshin Dinajpur in the envelope which should reach the office of the undersignedwithin 15.03.2017(12.00 noon).

2. Vehicle owner will be allowed to submit application for one/all unit.3. Supportive documents: each tender should be accompanied by the following documents, duly self attested

by the bidder.(a) Photocopy of valid registration certificate (bluebook) with valid fitness certificate.(b) Photocopy of up-to-date road tax clearance(c) Photocopy of up-to-date certificate from Pollution control Board(d) Photocopy of up-to-date Insurance certificate(e) Photocopy of PAN card of vehicle owners(f) Photocopy of receipt ofIT return for financial year 2015-16(g) Photocopy of current payment schedule and service tax registration.(h) Photocopy of VAT Registration and valid clearance condition.(i) Photocopy of Professional Tax Registration certificate and valid clearance condition.G) Photocopy of credentials for supplying of vehicle at Govt. offices.

4. The owner of vehicles shall have to furnish all the original of the aforesaid document for verification.5. The bidders have to submit Application fees (non refundable) amounting to Rs. 2000-00(Rupees two

thousands) only and EMD Rs. 5000 -00 (Rupees five thousands) only, both inform of Demand Draft!Banker's cheque in favour ofCMOH, DlDinajpur.

6. P.Oil and mobile will be issued as per Govt. norms.7. Tender Committee reserves the right to cancel tender or any BID without assigning any reason.8. The quoted rate should not exceed the existing rate of hiring of vehicle of the Transport Deptt., Govt.

West Bengal.9. Preference will be given to the new vehicles.
