Background:
The Port Klang Authority (PKA) is a statutory corporation
established on 1st July 1963, to take over the administration of Port
Klang from the Malayan Railway Administration.
PKA was the port operator before privatization beginning – 1986
1986 – first privatization - KCT
1992 – remaining operational facilities & sercives - KPM
- merged in 2000 as Northport (M) Bhd
1994 – newly developed Westports privatized to KMT (Westports
Malaysia Sdn. Bhd)

Acceptability of DG (123) 1: No vessel shall carry and no person shall bring or cause to be brought into port any dangerous goods unless the
approval of the authority has been obtained

Cause of accident : human negligence during use of portable pump & hose
Fatality : 13 workers and crewmen were killed

DG Proper Shipping Name: POLYMERIC
BEADS, EXPANDABLE evolving
flammable vapour/ IMO Class 9
/UN2211
Properties - A moulding material in
bead form consisting predominantly of
polystyrene, poly(methyl methacrylate)
or other polymeric material containing
5% to 8% of a volatile hydrocarbon
which is predominantly pentane. During
storage a small proportion of this
pentane is released to the air; this
proportion increases at elevated
temperature.
Inside hatch No.2 which
resulted in the hatch cover
blowing off the vessel.
No fatality.
IMDG Code requires DG to be
stowed On deck or if under deck
in a hold that is mechanically
ventilated.
As the containers stuffed with IMO
Class 9/UN2211 were not declared,
the containers were stowed in an
unventilated hold. As a result the
flammable gases evolving from the
container accumulated in the space
and eventually an explosive
atmosphere formed and it was this
that was subsequently ignited
Result of explosion
1 lashing gang member while
unlashing container in the adjacent
cargo hold was burnt (critical but
survived)
5 containers on deck were blown
into sea
1 container was blown onto
the wharf
3 containers were dislodged
between the vessel and the
gantry crane on shore
10 containers lie damaged on
the deck

Sank After Explosion
Vessel from Penang to Bintulu –
transit cargo
A cargo ship loaded with chemicals and
steel products exploded in a huge
fireball and sank at Southpoint.
No casualties but the sheer force of the
blast shattered windows and shock
waves were felt up to 3km away.
4 undeclared DG on board
(UN1495/5.1; UN3018/6.1;
UN3345/6.1; UN3082/9)
