INCIDENT REPORT NARRATIVE CONTINUATION REPORT # Q@!4 /m= 003A 19703
12-1 0-2000, 1840 hrs Aircraft #1 was traveling from Key West International to Ft. Lauderdale
Executive Airport. The pilot instructor encounter rough weather conditions and landed on Indian Key. The pilot reported departing the island during a clear in the weather and headed north northwest towards Indian Key Fill (U.S.Hwy 1)- The Aircraft once again encounter rough weather conditions, and the pilot decided to return to Indian Key. The aircraft lost control during the turn and crash landed on the ocean flats between Indian Key and Indian Key Fil€ near Indian Key Channel, lower Matecumbe in Islamorada, The pilot also mentioned poor visibility and too many obstacles, (radio antennas, power poles and power lines) to attempt a landing on the fill. The operator was knocked unconscious by the impact. Passenger #l ,a trainee pilot, rescued the operator out of the aircraft. The operator regained consciousness and both walked and swim to the fill using a seat cushion as a type IV flotation device.. 9 1 1 dispatcher was notified by witness # 1. Once on land aircraft operator and passenger located witness #I and requested his assistance. See witness statement form taken by the Monroe County Sheriff. Operator and passenger were not clear as to the main cause of the accident, however witness #1 indicated he observed the green starboard light of the aircraft approaching the fill, as he was traveling north approaching the bridge. He also observed when the heficopter spotlight came on, then the aircraft changed course and crashed. The victims were transported, treated and released from Mariners Hospital in Tavernier. The aircraft was found by the U.S. Coast Guard on 12-1 1-2000, Latitude N 24 52.9’ Longitude W 080 40.6’ . The aircraft was located 1/4 o f a mile west northwest off h d i m Key. US Tow-Boat South Dade was contract by the owners to salvage the aircraft.
The Helicopter was in used by a flight instructor from Volar Helicopter Inc.
