    HAZOP RECORD FORMFacility/Project

    Drawing No./Procedure

    Subject

Condensate lines from the condensers to the

    holding tank

    DEVIATION CAUSE EFFECT SAFEGUARDS ACTION

    No flow

    High Flow

    Pump malfunction

    Closure of V 22 18

    Closure of V 22 19

    Closure of V 22 17

    Overheating in E112

P125 failure would result in no coolingwater running through the heat

    exchanger.

Closure of this valve would increase the

pressure of the condensate line exitingE113. This would also cause an

increased pressure in V147. This would

change the pressure gradient and force

the liquid onto the vacuum lines causingmixing in the system. There is potential

for vessel E113, V147 and V150 todamage and rupture.

This would also cause an increasedpressure in V148. This would change

the pressure gradient and force theliquid onto the vacuum lines causing

mixing in the system. V148 and V150

    could damage and rupture.

The closure would cut off cooling watersupply to E113. This would result in no

condensate being formed in the shelland tube heat exchanger.

Due to overheating in E112 more than

the required/optimal amount of vapouris fed into E113 uncondensed. This

increases the flow rate of condensate in

    Less flow

    CONSIDER REVERSE FLOW

    High temperature

    More cooling water supplied toE113.

    Temperature of cooling water

    less than estimated

    Less cooling water to E113.

    Temperature of cooling water

    less than estimated

    Leaks in pipeline

Ambient temperature rises.Either environmental such as a

heat wave or human error such

as using a fire torch or weldingmachine near the pipelines and

    heat exchangers. Cooling

    waters efficiency reduces.

the stream and resulting in a increase in

pressure and filing up V150 at a fatserrate then estimated. This would also put

    increase pressure on V147.

More than optimal amount of vapourcondenses. This increases the flow rate

of condensate in the stream andresulting in a increase in pressure and

filing up V150 at a fatser rate then

estimated. This would also put increasepressure on V147

This would result in less condensate

    N-heptane hazardous. It is flammable

and can cause skin irritation. This

causes a lot of potential risk if released.

N-heptane is flammable. And increasein the temperature can build up pressure

or act as a ignition source.

Pressure transmitter placed on the inlet tothe pump. Drain on V150.

    Temperature indicator

    Temperature indicators

High pressure alarms. Alsoensure good maintenance.

Flow meter with a highlevel alarm on V150

    Temperature alarms

    connected to the indicator.

    Flow meter with a high

    level alarm on V150

    Educate people on the

    hazards of n-heptane.

Ensure it is a no smokingzone and people wear

    protective clothing. Installflow indicators between

    very long pipelines.

    Higher temperature alarms

attached with indicator.Educate staff on the

    hazards of n-heptane.

Ensure it is a no smokingzone and people wear

    protective clothing.
