ABOUT the Foundation

For two decades, the Harris County Hospital District Foundation has been part of connecting the people of our community with the medical care they need, regardless of their ability to pay. Our focus has been on funding programs to improve the lives of patients and educating the community about the phenomenal work the Harris County Hospital District (HCHD) does every day, such as providing facility enhancement and expansion, piloting innovative and unique healthcare programs and funding capital improvements.

A LETTERfrom Our Chairman

The fi rst 20 years of the Harris County Hospital District Foundation have been characterized by growth and excitement. Foundation donors and supporters helped launch our fi rst mobile mammography van, renovate the neonatal nurseries at Ben Taub General Hospital, and build playgrounds at Lyndon Baines Johnson and Ben Taub General Hospitals. We have seen the creation of the Children’s Health Fund, the Henry J.N. Taub Trauma Endowment, and a dynamic new group of Foundation supporters in their 20s called Scrubs.

In the spring of 2012, the Foundation culminated its fi rst capital campaign succeeding in raising more than $24 million for the new Smith Clinic. On July 26, 2012 we were proud to make our fi rst transfer of capital campaign funds, ten million eighty-nine thousand dollars, to the Hospital District. Made possible in part by a gift from the Lester and Sue Smith Foundation, the facility will house the Smith Clinics and Centers, the J. Evans and Mary P. Attwell Radiation Therapy Center, and the Cullen Healthcare Clinics.

Gifts of all sizes make it possible for our public-private partnership to succeed. Your gift in support of that partnership helps us to have the vital community care and wellness programs described in this report.

Please plan on joining us at one of our Foundation events. These include the Jubilee of Caring Gala, at which the Ben Taub Humanitarian Award is presented; the Silver Tea, which supports the Nursing Leadership Fund; the Texas Med 5K Fun Run; Cinco de Bingo presented by Scrubs; and our fall polo event, Mallets & Medicine, in support of the Center of Excellence in Cancer Care.

CHILDREN’S Health Fund

Leadership, Education and a Healthy Lifestyle

The mission of the Children’s Health Fund (CHF) is to provide support for long-range health programs for children who are not eligible for other similar programs and who are at the highest risk in our society. Last year, almost 2,000 children received direct behavioral health services from the following CHF grantees: Covenant House-Houston, Houston-Galveston Institute, Mental Health Association-Houston, and Santa Maria Hostel. Thanks to CHF funding provided to the Harris County Systems of Hope, 1,293 parents and community leaders were trained on how to access services for children with mental health issues.

Through these CHF-funded community-based initiatives, children and their parents received school-based counseling, social-emotional skills education, healthy lifestyle programming, family leadership services for parents raising children with mental health problems, and adolescents with mental health conditions received appropriate healthcare services.

CAPITALCampaign

The Smith Clinic

With our community growing at a rapid pace and the need for medical care facilities at an all-time high, our goal with the capital campaign was to expand health care in Harris County. Through the fi ve-year campaign, under the leadership of former Foundation Board President Ginni Mithoff, the Foundation raised more than $24 million to help fi nance the Smith Clinic, a fi ve-story outpatient facility that will offer patients a number of specialty services.

Named in honor of civic leaders Lester and Sue Smith, the Smith Clinic will improve patient access to healthcare services, reduce wait times, focus on preventive health care and serve more people in our community. The clinic holds more than 100 exam rooms and will offer general medicine, obstetrics, diabetes and oncology services. It will also include the Lester and Sue Smith Breast Center of Excellence, diagnostic imaging units, facilities for outpatient treatments, and the J. Evans and Mary P. Attwell Radiation Therapy Center.

The Smith Clinic

With our community growing at a rapid pace and the need for medical care facilities at an all-time high, our goal with the capital campaign was to expand health care in Harris County. Through the fi ve-year campaign, under the leadership of former Foundation Board President Ginni Mithoff, the Foundation raised more than $24 million to help fi nance the Smith Clinic, a fi ve-story outpatient facility that will offer patients a number of specialty services.

Named in honor of civic leaders Lester and Sue Smith, the Smith Clinic will improve patient access to healthcare services, reduce wait times, focus on preventive health care and serve more people in our community. The clinic holds more than 100 exam rooms and will offer general medicine, obstetrics, diabetes and oncology services. It will also include the Lester and Sue Smith Breast Center of Excellence, diagnostic imaging units, facilities for outpatient treatments, and the J. Evans and Mary P. Attwell Radiation Therapy Center.

HCHD’s mobile health units bring breast cancer screening technology, immunizations, health care for the homeless and dental care right into the community. The mobile vans feature state-of-the-art equipment to best serve the patients.

Early detection of breast cancer saves lives. Mobile mammography vans make it easy for women to be proactive. HCHD’s immunization van plays a major role in keeping kids healthy by increasing the percentage of children who are fully immunized in Harris County. HCHD provides immunizations for children in non-traditional settings, such as daycare facilities, Medicaid offi ces and pre-schools. The program helps reduce the likelihood of childhood diseases like measles, educates parents about the importance of sibling immunization and reduces potential inpatient care costs. HCHD’s dental van travels to shelters in the area, providing an array of dental services, such as regular examinations, cleanings, fi llings and extractions.

ESSENTIAL Women’s Health

Gynecology (GYN), Mammography and Breast Care

Providing the women of our community with access to quality health care is essential. Through prenatal and postnatal care, mammography vans and the Lester and Sue Smith Breast Care Center of Excellence, women are receiving the high-quality care they deserve with the convenience they need.

The new Lester and Sue Smith Breast Care Center will offer critically needed diagnostic mammography imaging, breast MRI’s, bone density testing, ultrasounds and other specialty healthcare services. The new GYN clinic will provide on-site procedures and testing for GYN patients, including colposcopy and ultrasounds. By moving GYN patients out of Ben Taub to the new Smith Clinic, capacity is doubled in both places, as Ben Taub will serve high risk obstetrics patients and the Smith Clinic will serve GYN patients.

HCHD is the fi rst healthcare system in Houston to achieve the Physician Practice Connections-Patient-Centered Medical Home designation by the National Committee for Quality Assurance. The prestigious recognition of the medical homes is a major milestone in healthcare delivery for patients. The medical home model is the primary-care component of a coordinated continuum of services that is held clinically and fi scally accountable for the health status of patients served.

ESSENTIAL Women’s Health

Gynecology (GYN), Mammography and Breast Care

Providing the women of our community with access to quality health care is essential. Through prenatal and postnatal care, mammography vans and the Lester and Sue Smith Breast Care Center of Excellence, women are receiving the high-quality care they deserve with the convenience they need.

The new Lester and Sue Smith Breast Care Center will offer critically needed diagnostic mammography imaging, breast MRI’s, bone density testing, ultrasounds and other specialty healthcare services. The new GYN clinic will provide on-site procedures and testing for GYN patients, including colposcopy and ultrasounds. By moving GYN patients out of Ben Taub to the new Smith Clinic, capacity is doubled in both places, as Ben Taub will serve high risk obstetrics patients and the Smith Clinic will serve GYN patients.

The Foundation works to help HCHD maintain the access to preventive services such as health screenings and immunizations, physical exams, women’s wellness exams, well-baby and pediatric care, diabetes care and education, adult weight management, as well as smoking cessation classes, chaplain services and medication education, so that the Harris County community is receiving the most convenient, quality health care possible.

EXPANSION of Community Clinics

Increasing Access Throughout the Community

As our community grows, HCHD will continue to increase the access to community health programs and build convenient specialty clinics and pediatric outpatient clinics. In the last two years, HCHD opened three new pediatric and adolescent health centers.

Future capital initiatives are to expand and renovate the emergency centers at Ben Taub and LBJ, build an Outpatient and Specialty Care Center at LBJ and renovate and expand several existing community health centers for primary care. The Foundation will be there helping to provide the support the Harris County Hospital District needs to fulfi ll its mission and goals in the community.

The Foundation works to help HCHD maintain the access to preventive services such as health screenings and immunizations, physical exams, women’s wellness exams, well-baby and pediatric care, diabetes care and education, adult weight management, as well as smoking cessation classes, chaplain services and medication education, so that the Harris County community is receiving the most convenient, quality health care possible.

Keeping our community educated and active and giving them direct access to the health care they deserve is our ultimate goal. After all, improving the health of our community is our mission, because we know that a healthy community is a happy community.

EXPANSION of Community Clinics

Increasing Access Throughout the Community

As our community grows, HCHD will continue to increase the access to community health programs and build convenient specialty clinics and pediatric outpatient clinics. In the last two years, HCHD opened three new pediatric and adolescent health centers.

Future capital initiatives are to expand and renovate the emergency centers at Ben Taub and LBJ, build an Outpatient and Specialty Care Center at LBJ and renovate and expand several existing community health centers for primary care. The Foundation will be there helping to provide the support the Harris County Hospital District needs to fulfi ll its mission and goals in the community.
