Agenda for Change – Knowledge and Skills Framework (NHS KSF)

“The NHS Knowledge and Skills Framework (NHS KSF) defines and describes the knowledge and skills which NHS staff need to apply in their work in order to deliver quality services. It provides a single, consistent, comprehensive and explicit framework on which to base review and development of staff.” (October 2004)

Ref no.

CYTOG3 Title Prepare chromosome spreads for cytogenetic analysis

OVERVIEWThis standard relates to the preparation of metaphase chromosomes in order to determine the constitutional or acquired karyotype of normal, abnormal or malignant tissue. It embraces (1) conventional analysis including solid staining, G banding, C banding and other differential or selective staining techniques for visualizing chromosome banding or structure; (2) fluorescence in situ hybridisation (FISH) of mitotic chromosomes; and (3) comparative genomic hybridisation (CGH). See function C NOS

You must be able to: You must know and understand:

a) Prioritise cases for preparation of chromosome spreads by reason for referral and other influencing factorsb) Pre-treat slides or coverslips as appropriate for the intended analysisc) Clearly identify factors affecting the quality of the stained preparationd) Take appropriate action to enhance the quality of the stained preparatione) Prepare FISH probes suitable for the investigation type and in accordance with relevant protocolsf) Generate and capture image of stained chromosomes g) Locate cells ready for analysish) Record process accurately, legibly and with sufficient detail to meet audit requirements

1.Application of appropriate SOPs2.How to prioritise cases for preparation of chromosome spreads and the factors influencing this3.Characteristics of the reagents used for staining4.Staining characteristics of the chromosomes under the different conditions that may occur5.The range of methods applicable to different types of specimen and/or investigation and their limitation6.The relationships between chromosome structure and staining patterns7.When and how to apply adjustments and corrective action to improve the quality of staining 8.Internal QA standards9.The types of preparation that require special storage and relevant storage conditions10.Documentation relevant to sample processing and its completion, including audit requirements11.Appropriate timescale to report results and professional guidelines12.How to produce a permanent image using image capture equipment13.Health and Safety requirements of cytogenetic investigations14.Range and requirements for associated documentation
