Dear Friends,

A s I look through the list of individuals and organizaions that contributed inancially in support of Community Health Center’s programs, I am

impressed and deeply grateful on behalf of the 9,840 paients who ben-eited from our services. They are grateful too, as evidenced elsewhere in this report and on our web site.

As a primary care physician myself, now reired, I was personally afected on those occasions when paients thanked me. Not everyone did, of course.

Most accepted my treatments as an expected mater of course and felt they owed me nothing more than payment for service. I was even more touched on those occasions when thanked by someone who, although enduring a diicult and unpleasant illness, was nevertheless grateful in belief that I had done my best.

When I donate to charitable organizaions, it maters to me whether or not I get an acknowledgment of the git along with a litle feedback about what good is made of it. That’s what I am trying to do here. Your inancial gits are translated, in so many ways, into primary health care services, including prevenive, mental health, and school-based services for our Jackson County neighbors who might otherwise not get them.

Our paients have “skin in the game” by contribuing what they can to the services they receive, and by cooperaing and taking interest in their own care with support of our health-care teams. That’s the model that Community Health Center operates on, and it has proven efecive in prevening un-necessary use of emergency rooms and hospital admissions. That is why Community Health Center expects to be a key player in State-directed Cooperaive Care Organizaions that you may have heard about by now.

Which brings up a quesion I someimes hear from supporters: “Won’t the Afordable Care Act and the recently State-enacted Health Transformaion Act eliminate the need for private donaions?” The plain answer is no. While there are potenial beneits, like expanded enrollment in the Oregon Health Plan, reimbursement models based on prevenion and results, and more access to a compei-ive insurance market, we know only too well that even if these things come true, many will be let out. While we welcome some of these changes, and prepare for them, we must always be alert for unpredictable poliical forces and budget strictures that could erase these promises.

Community Health Center strives to be prepared to take care of its own and to make room for new paients. With your coninued help, the uninsured child with asthma, the uninsured worker with atrial ibrillaion who is not yet eligible for Medicare, and the middle-aged waitress with a breast lump can turn to a professional and compassionate resource for help.

In January 2011, in partnership with Eagle Point School District #9 and

with the help of foundaions and indi-vidual donaions, Community Health Center opened its irst School-Based Health Center (SBHC) at Eagle Point High School.

Open during regularly scheduled school days and hours, the SBHC provides a wide-

range of health services for students including immunizaions, sports physicals, prevenive health exams, diagnosis and treatment of minor illnesses and injuries, as well as management of certain chronic illnesses such as asthma and diabetes in collaboraion with the student’s primary care provider.One of the biggest beneits of the SBHC is that students have

quick, easy access to health care. They do not have to miss a half day of classes to drive to and from a medical appointment. Keep-ing kids healthy and in the classroom where they can learn is the goal of the SBHC.

Community Health Center’s Butte Falls Clinic is open

as a full-service delivery site providing access to sliding-fee scheduled primary and preven-ive health care services to an esimated 2,634 rural residents. The opening of the clinic was made possible by a Federal Bureau of Primary Health Care grant award. Support from the community as well as the vision and leadership of the Bute Falls School Board made this clinic a reality. Rebecca Vose, PA-C, who

was a provider at the Medford site prior to the opening of the clinic, requested a transfer to the new Bute Falls clinic immediately ater hear-ing the clinic was opening. This was a career move for Rebecca, whose lifelong dream has been to provide medical care in a rural seing.

In May 2011, Community Health Center in Medford embarked on a year-long renovaion project to rebuild and modernize its aging clinic.

The new 10,792 sq. t. facility constructed on the footprint of the old buildings will provide team-based primary and prevenive health care services, prevenive dental care, brief mental health counseling and paient educaion services all under one roof -- a true medical home.

Financial support for the $3.5 million project which is scheduled for compleion in June 2012, includes a $1,877,872 Federal Afordable Care Act – Capital Development Grant and a $180,948 City of Medford Community Development Block Grant.

With the support of grants from the Oregon Community Foundaion, Collins Foundaion, Naumes Family Foundaion, and Rogue Federal Credit Union, combined with lead investments from community-minded individuals, Community Health Center’s Board of Directors iniiated a community campaign to raise the balance of the funds needed to furnish and expand the services of the fully renovated facility.

Donors can designate “Named Git Opportuniies” for certain exam and other rooms at the clinic. Individuals and groups interested in discussing their support for the project or who would like to tour the facility can contact Peg Crowley or Karen Elliot at 541-842-7711.

C ommunity Health Center implemented Epic, an electronic health record (EHR)

system, to meet the current and future needs of patients, providers and other medical partners in the community. An EHR makes it possible to share information between provider teams assuring coninuity of care, paient safety and cost savings as duplicated diagnosic tesing is eliminated. Through the MyChart program, paients can electronically access their lab results, medication list, research health concerns and ask quesions of their provider team.
