• Health records library is a storehouse of chronological written account of patient examination and treatment that include the patient medical history, complaint, the physician physical findings, the result of diagnostics test, and procedures, medication and therapeutic procedures are required, preserved, stored, and disseminated upon request.

• Health Records Library is also a section or unit in Health Information Management which is responsible for storing all live and deceased patients health records and supplying medical records for all emergency and elective inpatient admission and clinic appointment.

Sorting patient records numerically. Entering patient hospital number into

library dispatch register in numerical sequence.

Filing the folders chronologically and consciously into its rightful position on the shelve.

Documenting request(s) made by legitimate users sequentially for easy point of reference.

• To preserve and make health records accessible for continuity of treatment.

• For research, teaching, training of medical and Para-medical students, staff and scholars.

(H/R library holds patients’ folders in custody and retrieves folders for legitimate users. Requests for folders are released in batches and tracer card must be filled for every case note; which will indicate the patient hospital Number, patient’s name, date borrowed, name of borrower and destination).
