Dental Benefits

Dental coverage is provided for Class 5 members and their eligible dependents. The maximum annualfamily benefit is $2,750.00.

There are participating dental care providers that provide service at negotiated rates that accept theFund’s fee schedule. However, services can be obtained at a provider of your choice but you will have topay the difference, if any, between the Fund’s fee schedule and what your provider charges.

Covered Expenses

Diagnostic examinations Cleanings, once every 6 months X-rays, bitewings as needed, panoramic or full mouth once every 3years Sealants for children up to age 15, once a year Fluoride for children up to age 19, once every 6 months Fillings, composites (tooth color) fillings are not downgraded to amalgam (silver) fillings Dental prosthetics (crowns, bridges, dentures and implants), replacement allowed 5 years from date

of insertion Dental Implants, patient can be balanced bill by a participating provider for any charges over the

$550.00 allowable Surgical Extraction of fully bony or impacted wisdom teeth will be paid under medical as follows:

Occlusal guard, replacement allowed 5 years from date of insertion Orthodontia treatment is covered at 50% of the charged amount up to a maximum lifetime

benefit of $2,000 per person Periodontia treatment is covered at 50% of the Fund’s reasonable and customary

Expenses not covered (including but not limited to)

Teeth whitening or bleaching Dental bonding Veneers without a root canal being done Dental contouring Flippers Enamel shaping Gum lift Work or Auto related injuries
