I. Create User Log In A new contact is created when a user registers, signs in, and enters their profile information. Steps for creating a New User Log in:

A. Steps

1. Access the EHAN Sign-Up page: https://secure.hcphes.org/EHANSign/User/Login.aspx

2. Click on “Not a member? Register here!” link.

3. On the “Create User” screen, complete the fields below. Please make a

note of the newly created user name and password for future use.

• Username • Password • Confirm Password • Email Address

4. Click “Create User” blue button. The profile page will show.

II. Enter Profile Information

A. Steps 1. Enter the user information.

Note that (*) Indicates fields that are mandatory or required. 2. Review the information on the drop-down lists for Role, Medical Specialty, and

Facility and select accordingly. 3. Click the “Submit” button to save all information to the User’s Profle.

A confirmation message will be displayed to verify that the user profile has been updated.

III. Logoff

A. Steps

1. Click the “Logoff” link on left navigation.

2. This will take the user back to the Log in screen.

IV. Editing User Profile Information from the EHAN Sign-Up Page. (If a user wishes to edit their profile information from the EHAN Sign-Up Page)

A. Steps

1. Access the EHAN Sign-Up page: https://secure.hcphes.org/EHANSign/User/Login.aspx

2. Enter the Username, Password, and click “Log in”

3. Click “Edit My Profile”

Make the necessary changes to the fields or selections from the drop down categories.

4. 4. Click Submit and Click Logoff.
