R.N. Care Management

Registered Nurses are available to provide ongoing Care Management, which can include such things as:

• Assessment of client condition

• Medication Management and Set–Up

• Family Conferences and Care Conferences

• Arranging services, such as Physical Therapy

• Ordering medical equipment

• Coordinating with doctors and other providers

• Attending doctor appointments

Our Home Care Services

• Possible schedules: Hourly care for 3-24 hours/day, overnight care, or 24-hour “live-in” care

• For adults 18-100+

• Exceptional 24-hour “live-in” care

− Typically two consistent caregivers

• Nursing Home-Level care

− Bathing and toileting

− Transfers and mobility

− Personal care

− Blood pressure, blood sugar

− Medication administration

− Oxygen administration

− Catheter care

− Tube feeding

• Care for people with Memory Loss or Dementia

• Household tasks: Meals, routine cleaning, laundry, changing beds

• Pets: Clients can be at home with their pets!

• Driving: Errands and driving clients for social or appointments, shopping, etc.

• Caregivers: Experienced, nursing assistant training

• Every client has an experienced R.N. Care Manager (no extra fee)
