Thank you for taking time to read the Columbus Community Hospital Foun-dation’s 20�3-�4 Annual Report, which highlights the Foundation’s work in pro-viding support to our patients and their families. We hope you find the informa-tion in the following pages a testament to our on-going commitment to provide healthcare that embodies our values of Integrity, Compassion, Accountability, Respect and Excellence.

Columbus Community Hospital Foun-dation helps the Hospital achieve its mission through raising funds to support infrastructure, clinical services and health promotion programs of the Hospital. As our Hospital is one of the remaining stand-alone, not-for-profit healthcare institutions in our region, the Founda-tion assists in increasing the margin of excellence in healthcare delivery at the Hospital by finding funding to aug-ment net revenue generated through the Hospital operations to meet patient and clinician needs.

If you are concerned about the health of your family and friends and want their care to be delivered in a professional, safe and caring environment, you have some-thing in common with the mission of our Foundation. We endeavor to protect and sustain our community’s healthcare asset so that it can maintain a patient-centered hospital that responds to the needs of the community while at the same time provides a professional environment to attract and retain the best clinicians.

Foundation donors can say that they have helped bring the most current medical technology and programs to the Hospital and by doing so have saved and changed lives. Contributions have supported a wide variety of items. Everything from equipment needs such as a cardiac rehab telemetry system, therapy tools, pediatric cart, Nu-Step, thermometers and totes to building needs such as Behlen Memorial Hospital Cornerstone project, artwork pieces, awards board, signage, to health promotion activities such as SHAPE-

DOWN and mammograms, to edu-cational assistance such as scholarships to students and employees advancing in the medical field. Every dollar given makes a positive impact.

I know there is a great need in our community and that valuable health care organizations work hard day-in and day-out to meet those needs and increase the quality of life for everyone. I hope you include the Hospital on that list and consider making a gift to join in the Foundation’s goal to help CCH in caring for generations to come.

It was my honor to serve as chairman of the Columbus Community Hospital Founda-tion for the 20�3-�4 year. It is wonder-ful to be affiliated with an organization dedicated to ensuring quality health care for our community. With the increase in support we have received from our em-ployees and donors, plus the exceptional stock market returns on our investments, the foundation is in an excellent financial position.

The board is not here to sit on our money! Our mission is to “build goodwill and financial support for the hospital”. Dur-ing 20�3-20�4, the foundation spent $�52,238, which supported funding the following programs: for mammograms and follow up treatment for low income or uninsured women; scholarships for stu-dents and employees to further their health care educations; Cardiac Rehab Telemetry System; SHAPEDOWN – a weight man-agement program for children.

Our annual golf tournament raised

$93,4�5, netting $76,335 after ex-penses. This is the 3rd best in the event’s �2-year history. If you ask the major sponsors of the event why our tournament is so successful they respond, “Because it’s so much fun!” That directly relates to Shane Fleming, other volunteers and our staff! Shane, along with Whitey Walgren has very successfully chaired the golf tourna-ment for a number of years. Thank you Shane, Whitey, Denise, Carol and everyone who helped!

The Foundation was startled this year when our leader, Carol Keller, experienced a health issue. Admirably, Denise Schnieders stepped up and al-though Carol was missed, the Founda-tion was kept on course! We’re happy to see Carol back on the job on nearly a full time basis. Unfortunately, Denise has chosen to leave the Foundation and take a job within the hospital in hu-man resources department! We’re here

to support the hospital but this is taking that to a new level! Thank you, Denise. We’re glad you’re down the hall to help us, hopefully, when needed.

We need to thank three outgoing board members: Carroll Novicki, Bob Mead and Dr. Luke Lemke. Their leadership and support proved invaluable. Bob Irby, Tom Jarecke, and Marty Nore have taken up the challenge as new board members.

Eagles Radiology Scholarship Endowment This fund provides an annual cash award in the name of the Columbus Eagles Club to Columbus Community Hospital’s Radio-logical Technology Student of the Year.

Endowment Growth Fund The proceeds of this fund are used to increase the endowment and to purchase medical equipment where needed.

FLEXcon Endowment The proceeds from this fund purchase, through the CCH Auxiliary, medical equip-ment which is used to advance the chari-table mission of Columbus Community Hospital.

George Prochaska Scholarship Endowment This fund provides an annual cash award in the name of George Prochaska to help foster educational achievement and service excellence among Columbus Community Hospital employees.

Hospice Endowment The proceeds of this fund are used to benefit Columbus Community Hospital Home Health & Hospice Program.

S C H o l A R S H I p SWith the help of many generous donors, the Foundation is honored to award scholarships to deserving students who are pursuing careers in the healthcare field.

Eagles Radiological ScholarshipJessica Marie Heithuff of Columbus was the winner of the 20�4 Eagles Radiological $�000 Scholarship. The Eagles Radio-logical Scholarship Fund was established in 2006 by Columbus Eagles No. �834 Aerie and Auxiliary to promote excellence in the field of prevention, diagnosis, and treatment of heart disease and cancer in the Columbus area.

A graduate of Elgin (NE) High School, Heithoff is currently a Radiography stu-dent at the University of Nebraska Medical Center and is continuing her education by working at the Columbus Community Hospital. Heithoff plans to graduate from the Med Center in May 20�5 with a Bach-elor’s Degree in Radiation Science Technol-ogy. She then plans to further her educa-tion by specializing in the study of MRIs, CTs and Mammography. Upon completion of her education, Heithoff hopes to work in rural northeast Nebraska.

Two Prochaska Scholarships AwardedMark L. Jarecki of Platte Center and Dani-elle (Hill) Reab of Columbus were recently announced as the recipients of the 20�4 George Prochaska Scholarships through the Columbus Community Hospital Foundation. Both will be awarded a $500 scholarship to advance their educations in the medical field. The George Prochaska Scholarship Fund was established in 2005 to help promote educational achievement and service excellence among area health care professionals.

Mark L. Jarecki, son of Kevin and Ileana Jar-ecki, is a graduate of Lakeview High School and will attend the University of Nebraska - Lincoln this fall to earn an undergraduate degree in health science and then study for a career in physical therapy.

Danielle (Hill) Reab graduated from Co-lumbus High School. She attends the Uni-versity of Nebraska Medical Center College of Nursing- Northern Division in Norfolk, Nebraska. Her career plans include earning her BSN ( Bachelor of Science in Nursing.)

and possibly becoming a Family/Mental Health Nurse Practitioner.

Reab will complete an externship in the Intensive Care Unit at Columbus Com-munity Hospital this summer. She is active in several student organizations at UNMC. She studies Spanish and looks forward to becoming fluent in the language, in order to better serve her community.

Stempek Receives Gerhold ScholarshipStaci Stempek has been named the re-cipient of the $500 20�4 William and Barbara Gerhold Scholarship through the Columbus Community Hospital Founda-tion. The William and Barbara Gerhold Scholarship was established in 2005 to help promote educational achievement and service excellence among area students going into the health care field.

Stempek is currently employed as a regis-tered nurse in the Maternal Child Health Dept. at Columbus Community Hospital. She is a student in the Doctor of Nursing Practice (DNP) in Family nurse Practi-tioner program at Creighton University in Omaha.

HDR Scholarships AwardedLeslie Adame and Cole Fullner, both of Columbus have been named the 20�4 H D R Scholarship winners through the Columbus Community Hospital Founda-tion. Each year, the H D R scholarships assist two students who are pursuing careers in healthcare, one of whom is proficient in Spanish. Preference is given to students who plan to attend Central Community College – Columbus Campus.

Adame will be a student in the nursing program at Central Community College in Columbus in the fall of 20�4 and has been a Volunteen at Columbus Community Hos-pital all during her four years at Columbus High School. Adame plans to obtain an associate’s degree in nursing and then pur-sue a career as a Nurse Practitioner.

Fullner, a 20�4 graduate of Columbus High School, will attend the University of Nebraska at Omaha in the fall of 20�4 majoring in neuroscience. He aspires to become a surgeon. He was part of the 20�3 Nebraska Medical Mission Team that trav-eled to Nicaragua, where he was slated to serve as an interpreter and work in youth ministry. However, he was soon helping with tasks in the operating room where his interest in becoming a surgeon grew.

Mohr Receives CCH ScholarshipAmy J. Mohr of Genoa is the recipient of the 20�4 Columbus Community Hospi-tal student scholarship, awarded through the Columbus Community Hospital Foundation. The Columbus Community Hospital Scholarship was established in 2008 and is awarded to a qualified high school student who is taking a Certified Nursing Assistant or other entry level healthcare course.

Mohr graduated from Twin River High School in Genoa in 20�4. She will attend Wayne State College this fall where she will major in Pre-Nursing. Her career plans include receiving her bachelor’s de-gree in nursing and also to earn her degree as a nurse practitioner.

Dwayne Willers Scholarship AwardedAaron A. Alexander of Columbus is the recipient of a $300 Dwayne Willers Me-morial scholarship, awarded through the Columbus Community Hospital Founda-tion. This scholarship was established in 20�3 by Dwayne’s family and is awarded to a qualified student who is pursuing a career in the field of radiology.

A 20�2 graduate of Scotus Central Catho-lic High School in Columbus and a 20�4 graduate of Central Community College in Columbus where he is studied Radiol-ogy. Alexander has been accepted at the University Of Nebraska Medical Center where he will continue his study of Radiol-ogy with MRI and CT certification.

“People are like stained-glass windows. They sparkle and shine when the sun is out, but when the darkness sets in; their true beauty is revealed only if there is a light from within.” This quote by the late Dr. Elisabeth Kubler-Ross speaks to the hearts of the volunteers and Auxiliary members of Columbus Community Hospital. Every day, in every action, whether assisting patients or coordinating a fundraising event, our volunteers and Auxiliary members shine their inner light for those when they need it the most.

In 20�3, Columbus Community Hos-pital was proud to be the recipients of the energy and talent of 3�3 individual Volunteers and Auxiliary members.

Volunteers contributed 22,609 hours of service to Columbus Community Hospital in 20�3. Ranging in age from �4-94, volunteers serve in more than 20 areas throughout the Hospital. With patients’ needs at the forefront of every volunteer interaction, the service volun-

teers provide allows nurses, therapists, and others in the medical team to remain close to their patients.

The efforts of the Columbus Com-munity Hospital Auxiliary are focused on raising funds that are used to purchase or assist with the purchase of new equipment, technologies, or programs for the Hospital. In 20�3, the CCH Auxiliary committed to $�54,000 in financial support for the following equipment and programs: • C-Arm (Diagnostic Imaging) • Lymphedema Program (Rehabili-

tative Services) • Mindray Monitor (Maternal /

Child Health) • Resuscitator Infant Warmer (Ma-

ternal / Child Health) • R-Series Zoll Monitor Defibrillator

for Pediatric Code Cart (Maternal / Child Health)

Contributions from the CCH Auxil-iary partner with financial resources of

the CCH Foundation to maximize the Hospital’s budget and to help the Hos-pital stay on the leading edge of medical and technical advancements. Volunteers and Auxiliary members collaborate with Columbus Community Hospital to help make quality health care services avail-able right here at home.

The heart of our Volunteer Services and Auxiliary programs is illuminated by the inner light of our individual volunteers and Auxiliary members. We are grate-ful that they choose to shine their light on us.
