almosthomekids.org Spring/Summer 2018 1 The Littlest Rose Home News Spring/Summer 2018 Jen (RN) working with Antonio and Ana Laynez during a checkup for Rosamaria When Antonio Laynez heard his wife cry out in pain one morning last March, he felt a moment of confusion. “It couldn’t be the baby,” he remembers thinking. Ana was just 23 weeks pregnant with their first child. “But I quickly saw what was happening and called 911.” Their daughter, Rosamaria, arrived at Carle Memorial Hospital in Urbana weighing only 1 pound 7 ounces. Doctors immediately intubated her to enable breathing. Her skin was so delicate that her parents could not touch her. “It was the most difficult time you can imagine,” Antonio says. The couple struggled to understand their newborn daughter’s condition. As natives of Guatemala, they speak a rare dialect of Mayan. Antonio, who came to the U.S. in 2004, also speaks Spanish and English, but with so many medical terms coming at him, he found it hard to translate for Ana. Friends at the Apostolic Life Church in Champaign, where Antonio works full-time, helped with meals and support so he could take a leave of absence. After two months, Rosamaria moved to the NICU at Ann and Robert H. Lurie Children’s Hospital of Chicago, where doctors placed a tracheostomy and treated her for diabetes insipidus, a condition affecting fluid balances throughout the body. In late 2017 Almost Home Kids welcomed Rosamaria and took on the challenge of teaching her parents to handle her trach, feeding tube and diabetes care. “There are no medical translators who speak their dialect of Mayan,” explains Social Worker Molly Kim, “so we’ve taken a team approach. Antonio learns in English and Spanish with Ana observing, and he translates for her.” Visual learning has worked well, says AHK Staff Nurse Amera Al-Ali. “We draw a lot of pictures! It’s a constant triangle of communication. Since Antonio’s work schedule only allows him to be here for two weeks of every month, we are very focused in our sessions.” Amera says she felt a surge of satisfaction the first time she watched the couple disconnect Rosamaria’s equipment from the wall and move the child and equipment to the other side of the bed. “Just to be able to provide them with comfort, skill and confidence to do a safe transfer is so rewarding.” Rosamaria now weighs a healthy 18 pounds and is nearly ready to make the journey home. As the family celebrates her first year of life, they are reflecting on all they have learned together. “When we came here, we needed emotional support as much as medical training,” Antonio says. “Almost Home Kids gave love not only to our child, but to us too. We are so grateful.”

AsOdetteandWilkinsgetreadytotransitiontheirson,AveryMarc,homefortheveryfirsttime,PreferredHomeHealthRespiratoryTherapistRomaWeilandremarksonhowwellpreparedthefamilyisthankstotheirtrainingatAHK.“ThefamiliestransitioningtheirchildhomethroughAHKaremorethan90%betterpreparedthanthosegoinghomedirectlyfromthehospital,”shesays.“Itisasignificantdifference.”AverywasbornprematurelyandwastransferredtoAnn&RobertH.LurieChildren’sHospitalofChicagofollowingchronicrespiratoryfailureandtheneedforatracheostomy.Hespentalmostayearinhospitalashisdoctorsstabilizedhisrespiratorydifficultiesandlookedforwaystotransitionhimhome.TheMarcfamilyliveinPontiac,Illinoisandhavebeenfacedwithaprolongedstayduringeachstageoftheirson’smedicaljourneyduetoashortageofin-homenurses.TheyhadheardaboutAHKfromtheircasemanager,andwerekeentotransitionAverytherewhenhewasready.

OnceAverytransitionedtoAHK,itwasabigreliefforthefamily,sincehewasnowmuchclosertohome.Prettysoonmomsawsomedefinitegainsinhisdevelopment– gettingregularPTandinteractionswithvolunteersandclinicalstaffdefinitelymadethedifference.“[AHK ]hasbeenamazing–thenursingstaffhaveshownsomuchloveandcare –theyarelikefamily,”saidmom.PreferredHomeHealtharethefamily’schosenin-homehealthproviderandhasarrangedforthetransitionhometogosmoothly.AmyGelsomino,thecompany’sPediatricClinicalDirector,stressedthatthechallengesaresignificantastherequisitehome-healthRNsarejustnotthatreadilyavailableinthisruralarea.“We[Preferred]struggledtofindnurseswhowereinthePontiacareaandwhowouldworkforlowerpaythantheywouldreceiveinhospitalsorotheracutesettings.Ournursesareveryhardworkinganddedicatedtoprovidingexceptionalcareandactuallybecomeapartofthefamily”shesays.

AtAlmost Home KidsmomanddadhavecompletedtheirtraininginAvery’smedicalcare,andfeelveryconfidentandcomfortableinhiscare.TheyarefortunatethatPreferredfinallysucceededinidentifyingthenursingtheywillneed.

“Weareready–the24hourstaywheretheclinicalstaffmakesureweknowwhatwearedoingwasreallyhelpful.ItreinforcedthatwearereadytocareforAveryathome,andknowexactlywhattodoinanyemergency,”saysmom.

“ParentsneedtoknowthatcomingtoAHKcanreallyhelpinsomanyways.WehavebeengivennursingskillsthatwillhelpuscareproperlyforAvery,andtheconfidencetobringhimhome.”It’salwaysabittersweetmomentwhenfamiliestransitiontheirchildhome.AmyBramwell,AHKSocialWorkerexplainsmore.“IhavebeeninspiredbythelevelofcourageandbraverydemonstratedbyAvery’sparentsfromdayone.ParentscometousonwhatIimaginetobeanoverwhelmingly,apprehensivejourneyandleaveuswithconfidence,smiles,andapresencethatisremarkable!Avery’sfamilyhasbeendeartoAHKandtherehasbeennothingmorerewardingtoseethantheirfamilygoinghome.”Avery’sdadadded,“Nomanisanisland,andwhenyouasafamilyarefacedwithcaringforachildwhohasallthesecomplications,youcometorelyonhelpandsupport,andmostimportantlylove,frommanydifferentplaces– ourchurchgivesthatloveandsupportinourcommunity,ashaseveryoneatAlmost Home Kids.Theyarelikeasecondfamilyandit’swhatmakesthedifference.Weareblessedtohavethisinourlives.”

Fornearly7years,executivesfromGrantThornton(GT)havebeeninvolvedatAlmost Home Kidsinastrategicandmeaningfulway.Wehaveseenfirsthandthepowerofbringingdifferentbackgroundsandexperiencesintoourhomeandthedynamicandinnovativevolunteerenvironmentthatresults.Intheearlyyears,GTemployeesfirstparticipatedinourGolfMarathon,completing100holesandraisingmoneyforthecause.Thisyearmarksthe6thyearthattheyhaveattendedthegolfeventandwereallyappreciatetheirconsistentsupport.Thecompanyhasalsoprovidedcorporatesponsorshipforoursignatureblacktieevent,TheSharetheCareBall,whichincludesintroductionstoimportantnewsupporters.GTemployeeshaveparticipatedinafewStoryHourswithourchildrenwhichisagreatwaytohaveone-on-onetimewiththechildrenreadingbooks,engagingtheminplaywithtoysorsimplycuddlingwiththeinfants.ThisissucharewardingexperienceforbothourchildrenandGrantThorntonemployees.Thechildrenandstaffreallylookforwardtotheirvisits.Corporatevolunteersalwayswalkawayknowingthey’veaccomplishedneededprojectswhiletheyalsohonetheirteambuildingskills.GrantThorntonPartner,ChrisSchenkenbergservesonAHK’sBoardofDirectorsandPaulBogdanski,GTDirector,volunteersonAHK’sPresident’sCouncil.Eachhavebeendedicatedadvocateswithintheauditandtaxcompanytorecruitotherswithaheartforchildren’shealthcare.Whileoutinthecommunity,ChrisandPauloftenmakestrategicintroductionstoAlmost Home Kidsandadvocateforchildrenwithmedicalcomplexities.Theyaretrulywonderfulambassadorsforourmission.Forthesecondyearinarow,GrantThorntonhasorganizedateamofexecutivestorunintheShamrockShuffle,an8Kwhichcelebratesthebeginningofrunningseason.GivingbacktothecommunityisavaluethatpermeatesGT’scorporatecultureandisembeddedintheirvaluesandbehaviors.

OurPhysicalTherapyprogramlaunchedin2014andtodayweareenhancingtheprogrambyaddingSpeechTherapy.Ourcommunity-basedmodelofcarebringstherapiesdirectlytochildrentoremoveanybarriertoreceivingtheservices.Childrenidentifiedwithspeech-languageand/orfeeding/swallowingchallengesduringtheirstayatanyofour30referringhospitalsoftenreceiveevaluationsandinterventionfromaspeech-languagepathologistinthehospitalsetting.Todaywearepilotingtheabilitytoprovidespeech-languageandfeedingorswallowingservicesatourhomeinNaperville.Earlyidentificationwillleadtoimprovedoutcomes.Interventionsfocusedonfunctionalcommunicationinadevelopmentallyappropriatewaycanbesharedandtaught.Functionalcommunicationskillsareformsofbehaviorthatexpressneeds,wants,feelings,andpreferencesthatotherscanunderstand.SincethemajorityofchildrenstayingatAHKarenon-verbal,thiswillhelpparentsandcaregiverslearntocommunicatewiththeirchild.OurSpeechTherapistalsofocusesonidentifyingthesafestfeedingplan,increasingthetoleranceoforalintake,increasingsuccessfultransitiontomorefeedingsbymouthandlessrelianceonalternatefeedingsourceslikenasogastrictubeandgastrostomytube.ThankyoutoFTCaresFoundationandNorthwesternMedicineFoundationfortheirgrantstothisimportantprogram.SpeechTherapyisthenextstepinbuildingthecontinuumofcareinthecommunityforchildrenwithmedicalcomplexities.If you would like to support this new initiative, please contact Raeann Olsen-Jackson, [email protected] or 312.282.7081.
