How to Make an application as a Mentee

Fields marked with red Asterix are compulsory fields– please fill all of them or else the applicant would not be able to move to the next page

1st Page of Mentee Application: Personal Details- Part 1

1 .Enter your FIRST and LAST name (Middle name is optional)

2. Enter Father’s Name,

Mother’s name and Father’s Occupation (and similarly mother’s occupation)- you will have following options to choose from (drop down list)

3. Enter Gender and Date of

Birth

The application has 3 sections: 1. Personal

Details 2. General Details 3. Attach Documents


5 . If you belong to Particularly

Vulnerable Tribal Group (PVTG) select YES otherwise select NO----- For detail information about what is PVTG please click on

button

6. Select your state from the drop down

7. Similarly, select District, Sub-District and Village from their drop downs

Please select your State- District-Sub District and Village correctly from the comprehensive list. If not available, please enter correctly in address box

8. Enter complete address in the space provided below


IF NOT

9. Select your state from the drop down and similarly, select District, Sub-District and Village from their drop downs- and enter complete Address

10. Enter email ID- example : [email protected] Mobile number where the GOAL team can connect you easily

If your residential address ( Where you are staying currently) is same as your permanent Address ( Where you and your family permanently reside) for the period of next 12 months (1 year) please select ‘Same as residential address and move to the Email ID section .

Conformation of Application and any communication of selection would be done through your Email ID or mobile number- Kindly enter them correctly

11. Kindly fill your answers for these 3 questions. They are not mandatory questions but answering them would help increase your selection chances and support selection committee to know you and your application better

Provide information whether you are nominated by some authority or institutions

If yes ,Choose from drop down which authority/institution has nominated youThe list contains names of all the institutes/authorities who can nominate any tribal youth

Provide information whether you are working or not – employed or unemployed

If yes ,Choose from drop down the years of experience you have.Follow that with the type of occupation you currently engaged in:

1. Skilled and employed- If you work in skilled category2. Skilled and unemployed - If you are skilled but currently unemployed3.Unskilled and unemployed – If you come under the worker/labor force and currently unemployed4. Unskilled and employed – If you come under the worker/labor force and are currently employed5. If you run a business or are self employed.

Write the name of your employer in the text box

In case you don’t have work experience, please select ‘No’ and move on to next question


Put ‘Yes’ if you are differently abled and keep certificate ready. If not, put ‘No’

Put ‘Yes’ if you are member of Self Help Group. If not, put ‘No’

Provide name of your tribe – as mentioned in your caste certificate

Provide details of your family annual income from drop down details as below

Provide details about the language you speak and understand. It is mandatory to provide all

three preference using drop down menu –These language preference will determine the language in which you will receive course content


Provide information about level of education you have received using the drop down menu

Provide information about the field and occupation in which you are interest & related sub-interest in applying the training using the drop down menu


Providing interest will help mentor to guide you strategically on applications and case studies

related your field of interest

Provide information whether you have taken loan form either NSTFDC* or MUDRA**

Provide loan documents scanned copy if loan availed from them-*NSTFDC: National Scheduled Tribes Finance and Development Corporation**MUDRA: Micro Units Development and Refinance Agency Bank

Provide ‘Yes’ if you have a bank account otherwise ‘No’ if not

Provide ‘Yes’ if your village has mobile internet access otherwise ‘No’ if not

Provide ‘Yes’ if any of your family member has registered for GOAL otherwise ‘No’ if not

Provide ‘Yes’ if you are employed otherwise ‘No’ if not

Tick once you fill the form to self certify that all information is correct

Click on ‘Next’ to go to ‘Attach Documents page


Upload the document required as per the given category

This page will have provision to upload all documents related to fields which you will fill in ‘Personal Details’ and ‘General Details’ page.

1. For proof of caste upload Caste certificate2. For proof of age, please attach Birth Certificate/ Ration Card/ 10th Class

Certificate3. For education proof provide certificate till you have studied and selected in

form4. If you are differently abled, please attach the Physical Disability Certificate5. If you are nominated upload Nomination letter scan copy6. Upload MSME certificate if your business is MSME certified7. Upload Loan documents if you taken loan using NSTFDC scheme or MUDRA

scheme8. Upload scanned copy of Award if you have received any award and

selected yes9. Upload Disability Certificate if you are differently abled10.Upload work experience certificate if you are doing a job

Click on submit after uploading all the relevant documents

Uploading relevant documents will help in determining

genuineness of application and also level of mentorship required-Make sure you upload the correct

document

Click on choose file

Select file from relevant folder

After selecting file click on ‘Open’

The selected file will be reflected in that bar
