As part of our continuous effort to provide our colleagues with the best possible services, HSHS partnered with LifeWorks and has launched a newly customized pension portal . The online portal and access center allows you to be in total control, ensuring you have the resources you need to make the most of your pension benefits.

This personalized online gateway will give you the advanced tools and resources you need to make better, faster decisions. The Pension portal is truly a one-stop source to all of your HSHS Pension information; providing you with a wealth of data at your fingertips.

When navigating through the online portal, colleagues will have access to the following features:

• Automated approach to calculating pension benefits• Real-time and unlimited accessibility to produce pension estimates• Pre-retirement assistance• Integrated communications on all pension benefits• Ability to designate or update beneficiary/beneficiaries• Retirement knowledge center• Social Security resources

In addition to the online portal, colleagues will also be able to gain access to support from experienced pension service representatives, both online and by phone. These representatives will be available to answer all questions pertaining to pension benefits and help colleagues navigate through the retirement process. We are confident that these enhancements will make it easier than ever to manage your financial security.

Your personal and financial information is important and should be kept confidential, that is why Hospital Sisters Health System and LifeWorks have a strict policy in place to protect your privacy rights. More information on the Privacy Policy can be found at

MULTIFACTOR AUTHENTICATION

As part of our efforts to keep your personal informa�on secure, we have adopted Mul�factor Authen�ca�on (MFA), which uses a two-step process to beter protect your account from unauthorized access. MFA combines a username and password with an addi�onal security factor - a temporary numeric code sent to you using a phone number or email address you provide. Once you receive this numeric code, you will enter it in the website to confirm your iden�ty. MFA is a widely accepted security measure used by banking and social media pla�orms and ensures an addi�onal level of iden�ty verifica�on and data protec�on.

ELIGIBILITY

ARE YOU ELIGIBLE TO USE THE HSHS-PENSION PORTAL?

Colleagues who have not been employed at an HSHS ministry for a minimum of 12 months and/or have yet to work 1,000 hours or more in a calendar year will not have access to the online tool, until participant requirements have been met.

REGISTERING YOUR ACCOUNT

WHAT IS REQUIRED?

In order to access this website, you will need to register your account. As part of the registra�on process, you will be prompted to set up a username and password. Your username must be an ac�ve email address that you are able to access, and your password will need to meet a minimum set of requirements, which will be specified during the registra�on process. As part of our Mul�factor Authen�ca�on process, you will also be asked to provide at least one secondary contact method which can be either another email address or a phone number which can be used to send a temporary numeric code to validate your iden�ty. Quick �ps on how to register your account are available on the next page.

1. Login at http://benefits.hshs.org/pensionand click “GET STARTED.”

2. Register your account and enter in therequired fields:- Last 4 digits of Social Security number- Last name- Date of birth (MM/DD/YYYY)- Postal/zip code

4. Complete the two-step authentication.

7. Congratulations! Your registration is complete.

The portal allows you to designate a beneficiary(s) at any time.

1. Login at http://benefits.hshs.org/pension.You must register your account before you can begin. See quick tips on registering your accountfor additional support.

2. To view, manage, or add beneficiary data, you can either click on the “Add Beneficiary Data” tileor use the following path: Menu >> My Account >> Beneficiaries.

Your Pre-Retirement Beneficiary will receive any survivor benefits that may become payable if youdie before commencing payments.

3. If you are single, skip to step 6 (edit beneficiaries)If you are not married, you may designate any beneficiary. If you become married, your spouse will automatically become your beneficiary and any prior beneficiary designations will no longer be valid.

4. If you are married and designating your spouse as your beneficiary, skip to step 6.If you are married, your beneficiary is automatically your spouse. You may designate a different beneficiary, subject to spousal consent.

5. If you are married but selecting someone other than your spouse as your primary beneficiaryspousal consent is required.

In order to make this type of election, you must download the Pre-Retirement Beneficiary Designation form from the Forms and Documents page, complete it, and mail it to the HSHS Pension Service Center for processing.

You can also upload the form using the upload documents function found in the general documents page.

6. Add a Beneficiary: You may only enter one Primary Beneficiary. Remember to set thepercentage = 100%. If you enter multiple Primary Beneficiaries, your election will not be valid.

7. Edit personal information and select add to list.SSN, DOB, First Name, Last Name and Relationship are required.

Please note: If you have a benefit under both the traditional and cash balance pension plan, your pre-retirement beneficiary is not universal, therefore, you will need to manage your beneficiaries separately under each plan.

Your Pre-Retirement Beneficiary will receive any survivor benefits that may become payable if you die before commencing payments.

If you are married, your beneficiary is automatically your Spouse. You may designate a different beneficiary, subject to Spousal Consent.

If you are not married, you may designate any beneficiary. If you get married, your Spouse will automatically become your beneficiary and any prior beneficiary designations will no longer be valid.

You may only enter one Primary Beneficiary. Remember to see the percentage = 100%. If you enter multiple Primary Beneficiaries, your election will NOT be valid.

The pension portal allows you to have unlimited accessibility to run estimates at any time. 1. Login at http://benefits.hshs.org/pension.

You must register your account before you can begin. See quick tips on registering youraccount for additional support.

2. Click on the Estimates tab

3. Select your Benefit Commencement Date (i.e. Pension Start Date):• Early retirement: Age 55• Normal retirement: Age 65• Custom: Enter a custom date any time after the earliest date shown above

You can run estimates based on commencing your pension benefits at different ages, or you can choose your own. You may also enter in additional assumptions which could impact your pension benefits. Such as annual pay increases. These options can help you determine the most appropriate benefit commencement date and payment form for your situation. When you estimate your pension benefit, you will see results for all of your payment options. This will include several monthly annuity payment options and in some cases, a lump sum option.

4. Enter your last day worked (the date you expect to end your employment).

5. Select your Marital Status• Single: The Joint and Survivor Options will only be calculated if you provide a beneficiary

DOB.• Married: If you are married, you must enter a beneficiary DOB. You may designate any

person as your beneficiary, regardless of marital status. However, if you are married andyou designate someone other than your spouse as your beneficiary, your spouse mustconsent to your election.

6. Additional Assumptions (optional)Weekly Scheduled Hours Projection: Weekly Scheduled Hours will be used to calculate your projected service. If you leave this field blank, we will use your current schedule in the projection. If you anticipate a change in your schedule, please select the weekly scheduled hours to be used in the projection from the drop-down menu.

Annual Pay Increase: The rate at which you expect your pay to increase each year. If you do not make a selection, your current base pay will be used in the calculation.

7. Name Your Estimate (optional): Give your estimate a name for easy recall; otherwise, a defaultname will be assigned.

Keep in mind, the results displayed are estimates only and do not guarantee any pension benefit amount. Your accrual amount will be calculated when you commence your pension benefit.

On the HSHS-Morneua Shepell pension portal, you will be notified when you can begin receiving your pension benefits. Payments cannot be made until after you terminate employment.

1. Login at http://benefits.hshs.org/pension.You must register your account before you can begin. See quick tips on registering youraccount for additional support.

2. If you are eligible to begin receiving your pension benefit and you would like to commenceyour benefit, select “Get Started.”

3. If your pension benefit is subject to a court order dividing benefits as a result of adissolution of marriage, you must provide information regarding your Qualified DomesticRelations Order. Choose the option that best applies to you.

a) If a QDRO exists and has been submitted, you will see your QDRO on file. If there are noadditional QDRO’s pending, click “next” to advance to your benefit commencement details.

b) If a QDRO exists and has not been submitted, you must upload your QDRO after selecting theappropriate option outlined in step 3.

c) If no QDRO exists, click “next” to advance to your benefit commencement details.

4. Benefit Commencement Date: You must choose a date that falls on the first of a month, isafter your last day is worked, and is no more than 180 days in the future.

5. Designate a beneficiary:You may designate any person as your beneficiary. However, if you are married and you elect aform of payment other than a Joint and Survivor Option or if you designate someone otherthan your spouse as your beneficiary, your spouse must consent to your election.

6. Once your payment options have been calculated, view your results.

7. Select payment typeCarefully review your payment options below and select the option that best meets your needs. Be sure to review your benefit commencement date before proceeding.

8. Post-Retirement Beneficiary InformationIf your beneficiary’s information is incomplete, fill in what’s missing and proceed to the next step.

9. Payment Method: Choose how you would like to receive your pension payment(s):a. paper checkb. direct deposit

Please note: If “check” is chosen, proceed to the next step. If “direct deposit” is chosen, add your banking information and proceed to the next step.

10. Tax Withholding: Enter your elections for federal and state withholding.If you need to change your state for tax purposes, go to Benefit CommencementOverview and select Personal Information.

11. Review Your Information: If you need to change anything that is not editable you willneed to cancel commencement on the Overview Page and start over. You mustacknowledge and agree to all items in order to continue.

a) 30 Day Waiver Notification

b) Suspension of Benefits: If you retire and begin receiving a monthly pension benefit, yourpayment will be suspended if you return to work and are scheduled to work at least 1,000 hoursper year.

c) Consents: You must acknowledge and agree to all terms in order to continue.

12. Benefit Commencement Overview: Review to see progress and manage next steps.a. If actively employed, your benefit will be processed when your employer sends

official confirmation of your last day worked.b. If you are no longer actively employed, the online commencement process is complete,

and your payment will be initiated as soon as administratively feasible.

13. Termination Date Received: Once your termination date is received, your pension portaldashboard will state “the next step is ours, your payment will be initiated.”

IMPORTANT FACTS

1) If your last day of work does not differ from whatyou entered during the retirement online process, noadditional action is required beyond this point.

2) If your last day of work differs from what you enteredduring the retirement online process and there is noimpact in your benefit as a result, no additional action isrequired beyond this point.

3) If your last day of work differs from what you enteredduring the retirement online process and there is animpact in your benefit as a result, you will be notified viaphone to start over with the retirement online process.

14. Payment Started: Your first payment and all upcoming payments will be visible onyour personal dashboard.

a. The issue date is not the date when you may receive payment, but the date that wasprocessed in the system.

b. The amount shown for the next payment is the gross payment before taxes or anydeductions.

For more details, click on “view details” to see an overview of your most recent payments, tax withholding elections, and payment methods.

Congratulations! You have reached the end of the retirement online process!

2. Scroll to the bottom of the landing page and select “Update Security Settings.”

3. Click on the “Click Here” link to manage your user name, password, and contact methods.

4. Enter your current password and select continue.

7. You should see the following message at the top of your screen. Your new authenticationmethod should be available the next time you log in to the portal.
