Enrollment for your voluntary benefits begins at 9:00 a.m. Eastern Time (ET), September 23, 2013, and ends at 5:00 p.m. ET, October 18, 2013. You will be able to enroll, disenroll, or make changes to your Hyatt Legal Plans and EyeMed Vision Care for 2014 during these dates only. Payments for voluntary benefits are made through convenient post-tax payroll deductions, which are broken out evenly over your 2014 payroll cycle.

What’s ChangingEyeMed Vision Care Rates Are Increasing Effective January 1, 2014, there will be a slight increase in the EyeMed Vision Care rates. Please see the chart on page 7 for the new rates.

Please note, at 5:00 p.m. ET, October 18, 2013, the open enrollment period for voluntary benefits will end, after which you will not be able to enroll in coverage for legal or vision services, change your coverage level, or cancel coverage for 2014 unless you experience a qualified status change.

During this time, you may view your 2014 coverage and costs, and enroll or make changes to your 2014 coverage — online only — using the Added Benefits® website.

Enrollment Period (by phone and online):October 7, 2013 – October 18, 2013You may enroll in and/or change your 2014 Voluntary Benefits coverage elections online at www.addedbenefitsaccess.com or by calling Added Benefits® at 1-800-622-6045 starting on Monday, October 7, 2013, at 9:00 a.m. ET, through Friday, October 18, 2013, at 5:00 p.m. ET. Be sure to take action before Friday, October 18, 2013, at 5:00 p.m. ET. Late enrollments will not be accepted.

With the Hyatt Legal Plans, for an affordable price, you and your family will have access to professional legal representation for specific covered services whenever you may need them. Consider the following:

• If you owe money, would you like harassing calls from bill collectors stopped? Your plan attorney can help enforce your rights.

In-NetworkAll attorney fees for covered services are paid in full – there are over 12,000 in-network attorneys to choose from. You’ll pay no deductibles or copayments, and there are no claim forms. Out-of-NetworkYou may choose a non-plan attorney and be reimbursed for covered services according to a set fee schedule. Exclusions

Excluded services are those legal services that are not provided under the plan. No services, not even a consultation, can be provided for the following matters:

• Employment-related matters, including company or statutory benefits

• Matters involving the company, MetLife® and affiliates, and Plan Attorneys

• Matters in which there is a conflict of interest between the employee and spouse or dependents in which case services are excluded for the spouse and dependents

• Appeals and class actions

• Farm and business matters, including rental issues when the Participant is the landlord

• Patent, trademark and copyright matters

• Costs or fines

• Frivolous or unethical matters

• Matters for which an attorney-client relationship exists prior to the Participant becoming eligible for plan benefits

In an effort to better serve you and your family, the Hyatt Family Matters Plan allows you to extend the benefits of Hyatt Legal to your/your spouse’s parents. Now the benefit of a nationwide network of over 12,000 attorneys providing personal legal counsel is available to more of your family.

Covered Services

The Family Matters Plan covers telephone advice and office consultation for personal legal matters and the preparation of the following legal documents:

IMPORTANT: Personalized ID cards are printed with the employee’s name only; eligible covered dependents should use the card for identification purposes. Member ID will be the employee’s HR ID.

There are two options for you to choose from:

> STANDARD PLAN > ENHANCED PLAN Both plans provide you with vision care and prescription eyewear through a vast network that includes independent providers as well as retail locations, such as Sears Optical, Pearle Vision, Target Optical, LensCrafters, and JCPenney Optical.

ID cards will be mailed to new participants in January. If you are continuing your coverage from last year, you will not receive a new ID card. Print Your ID Card for Safekeeping

EyeMed Vision Care

Value Added Features

Members will receive a 20% discount on items not covered by the plan at in-network providers, which may not be combined with any other discounts or promotional offers. The discount does not apply to EyeMed providers’ professional services or contact lenses. Retail prices may vary by location.

At an in-network provider, once the funded benefit has been used, members also receive a 40% discount on the purchase of a second complete pair of eyeglasses and a 15% discount on conventional contact lenses.

Members also receive 15% off the retail price, or 5% off the promotional price, for Lasik or PRK from the U.S. Laser Network, owned and operated by LCA-Vision. Now Offering Retinal Imaging

Catch degenerative eye conditions early with retinal imaging. Retinal Images are obtained quickly and easily and give your eye care specialist the ability to monitor your eyes through the years. Early detection and treatment of eye conditions are crucial for your eye health.

If you are already enrolled in this program, your election will automatically renew. Why vision care coverage? Regular eye exams are important to the overall health of your family. A comprehensive eye exam can be the first indicator of health concerns, such as diabetes, high blood pressure, and glaucoma. Your vision plan offers coverage for eye exams, lenses, frames, and contact lenses. This valuable coverage is available to you and your eligible dependents. How do I locate a provider? Visit www.addedbenefitsaccess.com and select “Benefit Providers” under Vision Care to locate a provider in your area. How do I know what is covered under my vision care coverage vs. my medical plan?Your EyeMed benefit provides coverage for routine eye care only. If you have a condition that requires treatment with medication or possible surgery, these may be handled by your physician through your medical plan. Here is a list of the most common conditions of the eye that may be covered under your medical plan:

• Cataracts • Ocular Hypertension • Glaucoma • Retinal Detachment • Conjunctivitis • Retinitis Pigmentosa • Diabetic Retinopathy • Strabismus • Macular Degeneration • Uveitis

What if I decide to see a non-participating provider? If you choose to use a non-participating provider, you will be required to pay in full at the time of service and obtain itemized receipts. A copy of your receipts and a completed claim form should be submitted to EyeMed for reimbursement. You will be reimbursed up to the amounts listed on your schedule of benefits. Claim forms can be found at www.addedbenefitsaccess.com. What�does�a�routine�eye�exam�entail?� EyeMed Vision Care’s panel of optometrists and ophthalmologists conduct comprehensive eye examinations to diagnose or detect existing conditions of the eye and vision system. The tests and procedures performed may vary by patient because of age or specific conditions, but typically include an assessment, diagnosis, and treatment plan. EyeMed Vision Care monthly premiums are paid through convenient post-tax payroll deductions.
