(Information Bulletin 106)

Registration & Renewal Checklist Important

Details

• First time license holders must apply in person at the Development Business and Services Center.

• All paperwork must be submitted to get license processed.

• Certificate of Insurance must have the license holder’s name and DBA listed on the policy.

• Bond shall have the license holder’s name and DBA listed.

• A demolition

license is valid for a period of two years from the issued date.

Insurance and Bonding Information

• Insurance Requirements:

o Workers’ Compensation – Statutory limits

o Automobile Liability Personal injury -

$100,000/person Accident -

$300,000/accident Property damage -

$50,000/accident

• Comprehensive General Liability: o Bodily Injury -

$300,000/Occurrence o Property Damage -

$100,000/Accident o Property Damage -

$200,000/Aggregate o The policy shall include

the hazards of explosion and collapse coverage

• Indemnity Bond - $5,000* Note*: Must submit original bond. Copies will not be accepted.

Registration and Renewal Checklist

This checklist offers guidance for registering or renewing a Demolition Contractor’s License. The following information is required to complete the application:

• Name of License Holder • Current Home Address • Current Business Address • Date of Birth • Copy of Driver’s License • Fees:

o Registration - $85 o Renewal - $35

• Current Letter of Authorization • Certificate of Insurance • Demolition Bond with Power of
